The marine inspector must check the setting on the relief valves on all pressure vessels twice every 5 years. No more than 3 years may elapse between tests. Unable to check the relief setting on the 3 forward pressure vessels. Provide certificates for bench test, or replace with new reliefs prior to use.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel reported a half inch pinhole leak in the starboard #6 ballast tank piping in the pump room. Vessel shall conduct temporary repairs, then provide documentation and attestations by Master and Chief Engineer once temporary repairs are complete.

The towing vessel reported ballast water migration between PORT BT #7 and PORT BT #8. Vessel is authorized to proceed to Grand Haven, MI for further evaluation. Vessel is to provide repair plan from class to this OCMI zone prior departure from Grand Haven.

At this point, we are not comfortable with the plan to move the vessel with a tarp over the access hole or with the vessel departing port without full completion of the welding of the access patch. The vessel will be required to conduct all work at one location and the entirety of the welding must be completed prior to departing that location.

Vessel reported grounding outside of Waukegan Harbor. ATB was able to come off grounding after several attempts, and reported no damage. Conduct hourly rounds or as deemed appropriate for the safety of the vessel by the Master to ensure no water ingress or changes in vessel stability. After de-ballasting tanks on 5/11/2024 at Indiana Harbor provide proof of no damage to any of the vessels voids/ballast tanks to MSU Chicago at below contact information.

ELECTRIC INSTALLATIONS AND ELECTRIC EQUIPMENT MUST BE INSPECTED AT THE INSPECTION FOR CERTIFICATION AND PERIODIC INSPECTION TO DETERMINE MECHANICAL AND ELECTRICAL CONDITION AND PERFORMANCE. PARTICULAR NOTE MUST BE MADE OF CIRCUITS ADDED OR MODIFIED AFTER THE ORIGINAL ISSUANCE OF THE CERTIFICATE OF INSPECTION. VESSEL HAS NUMBEROUS UNSUPPORTED WIRES, DEAD END WIRES, INPROPER WIRE PENITRATIONS,EXTENTION CORDS AND UNAPPROVED WIRE RUNS THROUGHOUT THE ENTIRE VESSEL. IT SHALL BE THE DUTY OF THE MASTER AND [name withheld] THAT THE VESSEL, AND, IN PARTICULAR, THE QUARTERS ARE IN A CLEAN AND SANITARY CONDITION. THE [name withheld] RESPONSIBLE ONLY FOR THE SANITARY CONDITION ON THE ENGINEERING DEPARTMENT. NOTED NUMEROUS CARDBOARD BOXES, TRASH, OPEN TOP CONTAINERS OF OIL, TOOLS AND PRECARIOUSLY STACKED ITEMS THROUGHOUT THE VESSEL CREATING AN UNSAFE CONDITION. PROPERLY STOW ITEMS AND DISPOSE OF WASTE OIL BY APPROVED METHODS. 46CFR 97.15-10(A)

VESSELS OPERATING IN FRESH WATER FOR AT LEAST 6 MONTHS IN A 12 MONTH PERIOD SINCE THE LAST DRYDOCK ARE SUBJECT TO DRYDOCK, INTERNAL STRUCTURE, AND CARGO TANK INTERNAL EXAMS AS PER THE TABLE IN 46CFR91.40-3(b). THE VESSEL'S BALLAST TANKS AND VOIDS WERE NOT READY FOR INSPECTION AT THE SCHEDULED TIME FOR THE INTERNAL STRUCTURE EXAM, DUE TO ACCUMULATION OF WATER, MUD, AND SCALING. MAKE BALLAST TANKS/VOIDS READY FOR INSPECTION.

Vessel has a weeping pinhole leak in #3 starboard ballast tank at the weld at the vertical longitudinal bulkhead above the slope connection leaking into the #2 cargo hold. Submit repair proposal and effect repairs to the satisfaction of the Coast Guard Marine Inspector.

Vessel's thruster is not functioning. Troubleshoot and effect repairs to the satisfaction of the Coast Guard Marine Inspector.

Vessel experienced a grounding while transiting to Stoneport quarry in Alpena, MI. Vessel shall submit a repair proposal prior to departure from Stoneport, MI to the satisfaction of Class or local Coast Guard Marine Inspector. 46 CFR 91.30-1(a)

Crew was initially unable to find various life-saving equipment, and once found multiple deficiencies lead us to believe maintenance is not being conducted. Produce log entries of required maintenance and inspections.

Two Lifebuoys are required on-board with lifelines and float-lights. Only one lifebuoy has a float light but its battery is expired. Provide two lifebuoys with required operational float lights.

The vessel is required to carry six (6) Type I PFDs, and only has five (5) on-board. Of the five on-board several were missing required whistles, and lights. Provide six approved Type I PFDs with whistles and lights.

The vessel is required to carry six (6) Immersion suits, and only has five (5) on-board. Of the five on-board several were missing required lights. Provide six approved immersion suits and lights.

The life-raft's painter and hydrostatic release are not connected to the cradle or vessel, which would cause the raft not to deploy in an emergency. Properly attach.

Provide documentation of approved fixed fire fighting systems on-board, and for those systems not previously approved, submit plans for review and approval to MSC. within sixty days.

Update fire control plan to accurately reflect systems and equipment on-board the vessel, within sixty days.

6" x 12" hole in the side-shell, located in the port wing-tank, approximately 25' above the waterline.

Issued requirement to affect permanent repairs to bottom plating in way of cement box patch in #6 port ballast tank at main transverse watertight bulkhead separating #6 and #7 port ballast tanks at next dry dock.

Provide copy of Lloyd's Condiion of Class addressing cement box patch in #6 port ballast tank.

Engine shut-down pressure switches integral to CO2 fixed fire suppression systems for port and STBD Conveyor Engine Spaces have been bypassed by disconnection of plungers. This constitutes an alteration not approved by OCMI. Prior to departure from port, place system back into approved configuration and prove proper operation IAW 46CFR 109.425(a).

Provide gauge report for stbd longitudinal bulkhead plating (at & apx 8 feet above slope plate transition) within the #1 cargo hold (below #1 thru #4 hatch covers) and #2 cargo hold (below #7 thru # 10 hatch covers). Plating in question separates cargo holds & ballast tanks. Areas gauged shall be to the satisfaction of the attending CG Marine Inspector; with possible repairs pending.

Crop-out 4 temp. patches fitted within the #2 & #4 stbd ballast tanks (two in each tank-total of four) and make permanent repairs. Patches installed on stbd longitudinal bulkhead (above slope plate transition) between the #1 cargo hold to #2 ballast tank & #2 cargo hold to #4 ballast tank. Patches vary from 1 to 2 square feet at frames 42-43, 48-49, 53, 79, 81-82.

Complete and correct all outstanding items from the final survey report of 06Jan2012 (attached in documents section of activity # 4209617).

Make permanent repairs to cargo hold screen bulkheads between 2-3 and 3-4.

Renew ten-20 inch and two-10 inch to 8 inch non-metallic expansion joints fitted in the bilge and ballast piping system as required by 46 CFR 61.15-12. All non-metallic expansion joints are located in the lower aft machinery space.

Remove all improperly installed PVC piping penetrating through the ballast tanks watertight bulkheads and install penetrations which are designed and installed in accordance with 46 CFR 171.111 & 46 CFR 172.245(e).

Make repairs & demonstrate the four most forward remote reach rods are operational; serve Bilge and Chain locker, IAW 46 CFR 56.50-50

Install remote closing-reach rod on aft main fuel oil service tank, IAW 46 CFR 56.50-70&75

Blank all "open" Circuit breaker openings in electrical panels as observed, throughout vessel, per 46 CFR 111.01-1(a)(2)

Remount loose light fixtures within the starboard unloading tunnel IAW 46 CFR 111.01-1(a)(2) & 111.75-20

Install missing glass light fixture globe; mid level starboard aft air-dryer compartment, IAW 46 CFR 111.01-1(a)(2) & 111.75-20

Service vessels fixed CO2 systems, provide service report IAW 46 CFR 91.25-20

Remove old electrical cables and fixtures (as noted) throughout the vessel. Note all electrical circuits shall be properly dead ended IAW 46 CFR 111.01-1

Provide 4 type I PFD's prior to underway permissively manned operations.

Provide 4 Immersion Suits for crewmembers prior to underway permissively manned operations.

Provide approved electrical matting in front and in back of all switch boards. 46 CFR 111.30-11

Remove the wood 2" x 4" used as a blank in the stbd engine room panel board. Install a proper blank for crew protection. 46 CFR 111.01-1

Remove the piping between the potable water head tank and the air receiver in the forward bosn locker. This is not an approved air receiver.

Provide cover to protect batteries in the stbd engine room - forward of the stbd generator. 46 CFR 111.15-5

Provide proper life jacket light IAW 46 CFR199.70(b)(4(i). Currently the light had a dead battery, from being left energized.

Provide an approved embarkation ladder at the life raft launching station IAW 46 CFR 199.110(f).

Provide this office with a copy of the Cargo Gear Certificated with in the time line assigned by ABS for completion of system review.

Prior to commercial operations demonstrate attachment with tug and releasing. Test shall be completed while moored dockside.

While tug and barge attached, verification of visibility from pilot station as depicted on Bay Engineering drawing (Inboard profile and visibility 601-03 stamped approved by MSC under letter C1-0602126 dated August 16, 2006) shall be completed by a CGMI .

Prior to commercial operation provide this office with a copy of GL LL cert.

Vessel is allowed to proceed to Nanticoke, Ontario to off-load and effect permanent repairs to #2 Portside Ballast tank and framing. Vessel touched bottom while at anchor damage IAW 40 foot bottom set in, 9 tripped frames.

Properly service and cover terminals on the general alarm battery, located in the emergency generator room.

Prove proper operation of the emergency generator control station indicator panel light located at ECC.

Replace broken E-light globes No. 1 and 3, stbd side forward conveyor tunnel. Ensure all tunnel glass light globes are guarded with cages.

Properly secure with metal supports, the overhead wires located above the hydraulic pump inside the emergency generator room.

Ensure proper operation and adequate coverage of the vessel's GEMs system. Provide company's expectation for the system and procedures for how the system will be used by the crew.

Ensure vessels stateroom call system functions properly.

Ensure 4-foot applicators properly mate and lock into the nozzles.

Ensure installed fixed firefighting system pull boxes, that are located at vessel entrances and possibly exposed to temperatures of less than 32 degrees F, properly function.

Upon conducting a hydrostatic test of this pressure control air system pressure vessel (PV) it was discovered that the PV had a hole blown in it at the bottom of the tank. PV needs to be replaced with an approved PV and tested to the satisfaction of the attending marine inspector. LEGACY COMPONENT DESC: Receiver/Bottle

PROCEED TO MILWAUKEE, WI TO EFFECT REPAIRS TO THE STARBOARD BOILER IN BALLAST. REPAIRS TO BE COMPLETED NO LATER THAN COMPLIANCE DATE.

EFFECT REPAIRS TO DAMAGED INTERNAL FRAMING IN WAY OF #1STBD BALLAST TANK. DAMAGE LOCATED APPROXIMATELY 10 FEET AFT OF THE COLLISION BULKHEAD FOR APPROXIMATELY 15 FEET.

PROVE OPERATION OF BOW THRUSTER NOT LATER THAN 26 MAY 1987.

MAKE PERMANENT REPAIRS IN ACCORDANCE WITH NAVIC 5-80 TO ALL DAMAGED OR DETERIORATED ASBESTOS INSULATION IN NEED OF REPAIR.

PROVIDE EVIDENCE OF LIGHT FIXTURES IN CARGO TUNNEL MEETING CLASS II, DIVISION 2 REQUIREMENTS.
