Enclosure (2), section 2.a of NVIC 01-16 CH-3 states that the inspector should review the ECS display system type approval documentation or a manufacturer's declaration of conformity to verify it is the appropriate display type. Vessel was found operating with electronic charting system (Rosepoint Coastal Explorer) that does not have a type approval or manufacturer’s declaration of conformity and therefore does not meet the chart equivalency requirement. O/o is required to have up-to-date paper charts or utilize an electronic charting system that has a type approval or manufacturer's declaration of conformity. 46 CFR 184.420

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed 18 life jackets to be in unserviceable condition. O/o is to replace life jackets and destroy the unserviceable ones. 46 CFR 176.808(b)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Identification of the items may be in words, or the appropriate symbols in IMO resolution A.760(18). Letters must be at least 2 inches high. Symbols must be at least 4 inches square. Marine Inspector observed missing the number and identification on all life jacket stowage locations.

When using DSC techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the Commission or its designees. Marine inspector observed the vessel did not program vessel's assigned MMSI number indicated on its FCC Station License into vessel's Starboard radio telephone that has DSC capabilities.

Penetrations and openings in watertight bulkheads must have a means to make them watertight. Marine inspector observed a penetration in the FWD engine room bulkhead with a single wire passing through to be open to the adjacent compartment.

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date (a) Charts; (b) Sailing directions; (c) Coast pilots; (d) Light lists; (e) Notices to mariners; (f) tide tables; (g) Current tables; and (h) All other nautical publications necessary. Vessel was missing Light List, Coast Pilot and Navigation Rules. Cite: 46 CFR 185.2035

On systems in which the CO2 cylinders are not within the protected space,these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served. The CO2 schematic diagram of the system was missing. Cite: 46 CFR 181.20-15(d)

Any vessel of at least five net tons which engages in coastwise trade, must have a Certificate of Documentation (COD) bearing a valid endorsement appropriate for the activity in which engaged. As indicated on the vessel's COD, the vessel is restricted from coastwise trade due to current owner not citizen qualified for endorsement.

Extracts from the publications listed for the areas to be transited may be provided instead of the complete publication. Vessel was missing up-to-date chart, Coast Pilot, and Light List.

Chemical testing of personnel must be conducted as required by 46 CFR part 16 and in accordance with the procedures detailed in 49 CFR part 40. Documentation for completing a Drug and Alcohol Program Inspection (DAPI) was unavailable for inspection.

13101 - A vessel must have two independent means of controlling each propulsion engine. Control must be provided for the [...] direction of shaft rotation [...]. Stbd M/E shift linkage disconnected at outer bowden cable sheath, preventing the R/G from shifting out of fwd shaft rotation. - 46 CFR 184.620(a) - Deficiency immediately rectified

Vessel has bare wires coming from the compression block located on the port side of the engine room ceiling.

Vessel does not have the Coast Guard approved fuel lines on the main engines.

Vessel had several cracks in the welds of the gussets attached to the frames in the dry stores room. Due prior to next credit dry dock.

Repair or replace ring buoy where lifeline has deteriorated.

Remove dead end wiring from tank compartment bilge.

Effect repairs to discharge bilge piping on port side engine room compartment to prevent leaks.

Investigate cause of coolant leak from the vent tube of the expansion tank on the STBD engine. Make repairs and operationally test to the satisfaction of attending marine inspector.

Alarm panel indicates all bilge levels are activated when only one bilge alarm was triggered.

Vessel's buoyant apparatuses had deteriorated and needed reservicing. Vessel may not operate until the buoyant apparatuses are repaired.

Retro reflective material on the lifejackets was deteriorating and needs to be replaced.

Provide a proper length lanyard for the lifebuoy's floating waterlight in accordance with 46CFR 180.70 (d) (2) & (3).

Fix inoperable installed Hydraulic bilge pump.

Add one type A-II portable fire extinguisher to the vessel's upper passenger accommodation.

Hose was sun damaged.

Bilge pump inoperable.

Shift control cable parted. Shift control cable replaced and controls were tested in all modes. Tested SAT.

Repair fractured frames port side IWO chine 15' aft of stem.

#2 space: Port #1 frame. Fractured weld in lower reinforcement plate in way of chine (forward side of plate). Provide proposal and effect repairs NLT 8JAN00.

#2 space: Port #1 frame. Fractured weld of upper wedge in way of mouse hole. Provide proposal and effect repairs NLT 8JAN00.

#3 space: Port #1 frame. Small weld fracture in way of mouse hole._x001C_ Provide proposal and effect repairs NLT 08JAN00.

#2 space: Port #2 frame. Fracture in transverse frame in way of chine. Provide proposal and effect repairs NLT 8JAN00.

#2 space: Port #2 frame. Fracture in weld of aft side reinforcement plate.Provide proposal and effect repairs NLT 8JAN00.

#2 space: STBD #1 frame. Small fissure in weld of reinforcement plate. Provide proposal and effect repairs NLT 8JAN00.

Weld/renew to original condition, cracks found on main transverse frames intersecting chine areas in the following locations; Port/stbd engine room #1 and #2 frames. Port/stbd tank room #2 frame. Stbd tank room #1 frame._x001C_ IAW 46 CFR 57.02-4/176.600.

Replace plastic zip ties which secure electrical wiring throughout vessel with metalic fasteners IAW: 46 CFR 183.340.

Repair/renew to original condition, (2) cracked welds, stbd frame #2, forward of collision bulkhead. IAW 46 CFR 57.02-4/176.600.

Provide and install bonding wire on non-metallic sections of fuel fill hosebetween fuel tank and fill pipe.

Effect permanent repairs to cracked main transverse frames 5 through 8 on port and starboard sides.

CORRECT WATERTIGHT BULKHEAD NONMETALLIC PIPE PENETRATIONS TO COMPLY WITH THE REQUIREMENTS OF 46 CFR 182.720(D).

EFFECT REPAIRS TO THE HYDRAULIC LEAKS TO THE STEERING HYDRAULIC PIPING IN THE FORWARD STORES SPACE.
