Watertight integrity above the margin line in a vessel of 100 gross tons or more. Each opening in an exposed weather deck must— (1) Have a coaming that complies with the height requirements in table 171.124(d); and (2) Have a means for closing it weathertight. Provide a repair proposal for renewing the damaged sea gate to include the name of the vessel within 30 days.

It shall be the duty of the master or other person in charge of the vessel to see that all of the provisions of the certificate of inspection are strictly adhered to. Nothing in this subpart shall be construed as limiting the master or other person in charge of the vessel, at his own responsibility, from diverting from the route prescribed in the certificate of inspection or taking such other steps as he deems necessary and prudent to assist vessels in distress or for other similar emergencies. Due to the loss of the sea gate, vessel is to be operated in seas less than 8'.

Vessel must meet all applicable stability and subdivision requirements of sub chapter S and Classification Society Rules. Make permanent repairs to the area on the stern damaged by the marine Casualty that occurred on 17 SEP 2025.

On vehicular ferries, suitable chains, cables, or other barriers shall be installed at the ends of the vehicle runways. In addition, suitable gates, rails, or other devices shall be installed as a continuation of the regularly required rails. Install temporary rails while underway to prevent persons from falling overboard until the reinstallation of the sea gate.

During each inspection a demostration of the proper condition and operation of rescue boat shall be demostrated. Rescue boat did not function as intended repair rescue boat to the satisfaction of the marine inspector in a timely manner.

Fire doors 10 and 12 did not properly close.

Emergency lighting in engine boiler room is no longer designated with required markings.

Lifejacket stowage in engine boiler room were not marked with indicator or quantity.

Each launching appliance must be fitted with brakes that are capable of stopping the descent of the survival craft or rescue boat securely when loaded with its full complement. Starboard forward life boat davit winch slipped before coming to a stop when tested.

The installation of all systems of an electrical engineering or interior communications nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Found an oil leak on the strbd side turbogenerator.

Position indicator at remote closure for WTD #4 is broken. Prove proper operation of indicator.

Escape ladder in upper boiler room has been cut 2 rungs short Fabricate and install a way to access the existing ladder without impeding the hinged grating for hoist access prior to 2023 COI inspection.

Lifeboat #3 is corroded and wasted through the bottom in way of aft stbd side outboard from drain plug. Vessel is restricted to carriage of not more than 540 passengers. Make permanent repairs as original to the satisfaction of the attending Marine Inspector.

Fire detection panel did not alarm properly for all zones. Prove proper operation of fire detection.

Upon USCG Sector Lake MI approval, repairs shall be made to all Cabin Deck plates corroded or wasted more than 25%. All repairs to be verified and to the satisfaction of the attending CG Marine Inspector. Repairs must be verified by an attending CG MI on or before May 15, 2022 and before carrying passengers for hire. As required by 46 CFR 71.55-5

Observed from upper vehicle deck, numinous existing cabin deck plates corroded or wasted more than 25% of original. In addition, noted numinous existing doublers previously installed. Company rep provided correspondence between Bay Engineering Inc., USCG MSC, MSD Sturgeon Bay, and MSO Milwaukee OCMI dated October 1996 thru October 1997, reflecting ¼ inch ABS grade A or ASTM A-36 plate, doubler plates were allowed to be installed per BEI SS BADGER Cabin Deck Corrosion-Patching Arrangement & Details drawing 110-01, Rev B. Drawing approved per USCG MSC Ltr H2-9702438, dated July 22, 1997. Considering documentation provided, company required to re-submit a revised drawing to USCG Sector Lake, MI., which identifies cabin deck plates wasted and required repairs.

General Alarm bells in various spaces were found not working or found with tone inadequate. Repair or adjust and prove proper operation.

Sanitary conditions of crew accommodations were on poor condition. Showers throughout were rusty, missing paint or had not been cleaned. Rectify sanitary condition.

Floor tile and condition of flooring throughout many crew spaces were detached or raised from the deck. Company to survey and submit corrective action plan to Sector Lake Michigan to address.

Conduct a full load weight test on davits for lifeboats No. 1, No. 3 and No. 5 upon arrival in Manitowoc. Wi in the presence of a Coast Guard Marine Inspector.

Provide copy of the service reports for the fire extinguishers, Inflatable Buoyant Apparatus's (IBA's) and Great Lakes Radio agreement annual inspection.

Replace worn vent fan flexible boot on vent aft of pilothouse upper deck.

Master was not aware of who the Company Security Officer (CSO) was as the vessel had gone through change of ownership. Update security plan to reflect new company CSO with name and contact number.

Fire screen door No. 8 does not fully close when magnetic release is activated. Prove proper operation.

4 foot applicators would not attach to the navy all purpose nozzles and chains where found detached on the nozzle cones. Survey all fire stations and ensure all chains are attached and prove applicators will attach to the nozzles.

Painters and weak links were not attached to IBA's.

Various lights were found removed and unwired throughout the passenger cabin. Reinstall and prove proper operation of lights and emergency lighting throughout ship.

Fire alarm ilumintation light was found inop. Repair and demonstrate proper operation.

Vessel had onboard new EPIRB. New EPIRB was missing registation decal although record of registration was provided. Upon reciept provide proof EPIRB registation decal is installed.

Aft port boiler after drum would not fully lift after multiple attempts within set range. Prior to placing boiler in service prove proper operation.

No. 3 feed pump is out of service due to a faulty impeller. prove proper operation and demonstrate operation of overspeed trip.

Due to change of ownership vessel security plan must be audited. Provide documentation of Audit, Drills and last exercise.

During attendance a small fire was discovered by Marine Inspectors consisting of oil soaked lagging on the starboard aft draft fan turbine. Clean Fan space and remove all oil soaked lagging on all turbines in the space and renew lagging.

Ship Service Air compressor was out of service. Prove proper operation.

Pitting and wastage noted in tanktop of #3 ballast in way of coal conveyor/crusher equipment. Crop and renew as per repair proposal dated Oct 19, 2020.

Wastage noted on bilge floor frame #79 and bracket port side coal conveyor space. Crop and renew to satisfaction of attending Marine Inspector.

Corrosion wastage noted on trip brackets of side frames to deck at vehicle deck stbd side #24 & #36. Crop and renew to satisfaction of attending Marine Inspector.

Observed low water level alarm on the forward stbd propulsion boiler did not reflect correctly on monitoring and control system history. Investigate and resolve issue on or before November 26, 2019.

Counterweight on brake lever for #1 lifeboat contacts the gearbox housing and does not properly engage when released by operator. Adjust counterweight to prevent contact with gearbox housing when released.

Observed gasket(s) missing for doors to sideport opening stbd side for emergency egress to IBA/rafts. Install proper gaskets/seals to maintain weather/watertight integrity of sideport.

Each self propelled vessel must have at least two electric generatoring sources i.a.w. 46 CFR 111.10-3 Vessel's Stbd SSTG was unable to maintain adequate electrical load due to failure of voltage regulator.

Vsl is broadcasting: Call Sign from expired FCC License, invalid IMO #, and NAME with Vsl type precursor (NAME must not include Vsl type precursors, e.g. F/V, M/V).

Disassembly and inspection of forward horizontal falls sheave on Lifeboat Davit #4 indicated bearing failure. Renew/repair sheave, retest in presence of CG Inspector. Vessel passenger count reduced to 540 until resolved.

Bilge manifolds port and starboard manifolds in boiler room not properly labeled i.a.w. 46 CFR 78.47-38

Conduct hydro test of fwd port boiler to 750 psi in presence of CG Marine Inspector. Side wall tubes leaking during initial season light off of fwd port boiler. It was estimated that 03 tubes required renewal and subsequently 08 were renewed. Boiler was not returned into operation until a satisfactory hydro test conducted with ABS and CG present.

Prove proper operation of port fwd boiler and set safety valves. Boiler shall not be placed into operation until after a hydro test conducted with ABS and CG present. Side wall tubes leaking during initial season light off and operation of fwd port boiler. It was estimated that 03 tubes required renewal and subsequently 08 were renewed.

Prove escape scuttle hatch to #6 Hold space on car deck is watertight.

Conduct full weight test and light weight test of starboard lifeboats #1, #3, #5 in accordance with 46 CFR 199.45

Power supply for gyro compass and repeater must be independent of the EDG start battery or general alarm battery i.a.w. 46 CFR 113.25-7(b)

Unlicensed crew assigned abandon ship duties are not completely familiar with the operation and deployment of IBAs. Implement additional training or other methods to improve knowledge of crew members assigned to emergency duties.

Re-install soft patch on coal day-bunker feeder system piping on the upper level in between boilers.

Install grab rail on front of galley stove for the safety of personnel.

Lifeboat #6 was holed in way of bottom drain. Submit repair proposal and make proper repairs in accordance with approved procedure. Vessel is restricted to carriage of 585 passengers until reapirs completed to the satisfaction of an attending Marine Inspector.

Conduct full load weight test of #6 lifeboat/rescue boat, davit and falls in accordance with 46 CFR 199.45(b).

Make proper repairs to fire main piping in way of frame #157 sbd side vehicle deck at staion #33 and conduct hydrostatic test of repairs to the satisfactio of an attending Marine Inspector.

Annual full load weight test of starboard lifeboat davits and falls unable to accomplish due to mooring arrangement to stbd side. Conduct full load weight test of #1, #3, & #5 davits and falls at earliest opportunity when vessel docks in Manitowoc.

Deck rails in vicinity of port and starboard raft (IBA) stowage and release racks were wasted and broken in several spots. Make suitable repairs to same.

#1 lifeboat release hook was repaired and satisfactorily weight tested but requires non-destructive testing (dye penetrant) subsequent to full load weight test.

Launching appliance for number 1 lifeboat failed and aft davit would not release.

Gyro was hunting and not operationing properly.

Two required Class "A" fire doors would not properly close.

Sheave mounting bracket for #5 lifeboat davit falls is fractured. make proper repairs and conduct 110% load weight test. Vessel restricted to not more than 540 passengers.

Emergency lighting for STBD circuit not operating de-energized for repairs to adjacent piping in overhead Texas deck. Prove proper operation of all "E" lights.

Aft wheel house helm binds at points through wheel rotation. Make repairs and prove proper smooth and even operation of afte helm wheel.

Deck plates in machinery spaces not properly secured. Secure loose deck plates in machinery spaces.

Aft masthead/range light not operational, disconnected for maintenance. Prove proper operation of aft masthead/range light.

Coal ash retention system not operational/under repair. Prove proper operation of ash retention system.

Superheater casing cover for stbd fwd boiler is missing studs/nuts for cover mounting. Install/repair studs for casing cover mounting.

Steam stop valves at boiler room to engine room bulkhead are not insulated. Install suitable insulation where necessary to preclude injury to personnel i.a.w. 46 CFR 56.50-1(k).

Fire main piping leaking in way of overhead line crew's berthing passageway, Texas deck just forward of fire station #6. Make permanent repairs to satisfaction of an attending marine inspector.

Your request to employ a 1st AE Motor/2nd steam in lieu of 1st AE steam & promote 1A/E to C/E is approved in accordance with 46 CFR 15.825. You may temporarily adjust manning until end of the operating season in October 2014. You may be manned as follows: 01-C/E, 02-2nd (steam), 01 3rd.

Test releasing gear for the #2 Lifeboat at waters edge.

Repair #1 and #2 lifeboat aft sheave brackets that are warped and fractured. Reduce passenger capacity by 120 persons until repairs have been made to the lifeboats in the presence of an attending CG MI.

Batteries for fire alsarm system have excessive corrosion on terminals and connections. Clean and prove good electrical contact.

Fire main above overhead ceiling tiles along port passageway on the "Texas" deck had leak along weld seam. Cropped and renewed satisfactorily during inspection.

Fwd sheve/block on #1 life boat installed at improper angle resulting in binding and improper operation. Master stated that sheve/block will be removed and installed correctly during winter lay up.Proper operation demonstrated on/before 31May2012.

PROPERLY PLACARD PORT AND STARBOARD CABIN DECK LIFESAVING GEAR STOWAGE LOCKER TO SEPERATELY INDICATE THE NUMBER OF STOWED PFDS AND THERMAL PROTECTIVE AIDS.

FOLLOWING EACH COAL TRANSFER, CLEAN SEATING SURFACES OF THE #3 WATERTIGHT DOOR. MAKE PERMANANT ARRANGEMENTS TO KEEP SURFACE CLEAN AND ENSURE PROPER SEATING OF DOOR.

DO NOT OPERATE THE NEW AIR CONDITIONING SYSTEM UNTIL SYSTEM PLANS AND INSTALLATION HAVE BEEN APPROVED BY THE USCG.

IAW MSO CHICAGO I&I POLICY #4-99, SUBMITT AN IBA CREWMEMBER TRAINING PROCEDURE FOR REVIEW BY MSD GRAND HAVEN.

FOLLOWING EACH COAL TRANSFER, CLEAN SEATING SURFACES OF THE #3 WTD. MAKE PERMANANT ARRANGEMENTS TO KEEP SURFACE CLEAN.

DO NOT OPERATE, FOR PROPULSION OR POWER, THE STBD AFT BOILER UNTIL SAFETY VALVES ARE SET BY THE USCG MARINE INSPECTOR. AS AN OPERATING CONDITION, VESSEL MAY OPERATE NORMALLY (IE. DEPART LUDINGTON) W/ AT LEAST 3 BOILERS INOPERATION, & MAY RETURN TO LUDINGTON W/ 2 BOILERS.

Weight test the numbers 1, 3 and 5 lifeboats in the presence of USCG marineinspector.

Provide original plating thickness to compare to gaugings taken by Bay Shipbuilding representatives where they are absent to enable a marine inspector to make a determination whether plating is within allowable tolerances.

Nonmetallic expansion joints for main circ system due for renewal in accordance with 46 CFR 61.15-12(b). Renew all nonmetallic expansion joints located in a system which penetrates the hull in which both the penetration and the expansion joint are below the deepest load waterline.

Sideshell frames and longitudinals are wasted in way of the stbd side outboard of the water softeners and on the lower level below them. _x001C_ Crop and renew to good metal wasted areas of framing, decking and associated internal members.

Replace in kind and prove proper operation of the admission valve for the #3 cylinder on the starboard main engine.

Prove proper operation of the port generator governor control valve.

Hydro port forward boiler, and set and seal safety valves prior to operating boiler with passengers onboard.

Place type II retroreflective material on all lifeboats to meet the requirements in IMO resolution A.658(16).

Conduct weight test of all 3 starboard lifeboats.

Properly install acceptable flex fuel hose on emergency generator to replace worn supply and return hose.

Prove proper installation of 2 FI feed tubes from mud drum to fire wall header on port forward boiler.

Vessel may operate with 3 boilers under the following conditions: tow vessels must be reasonably available and shall be used prior to entering a harbor if the vessel loses one of its 3 operational boilers prior to the port forward boiler being placed back in service.

PROVE SATISFACTORY WATERTIGHT INTEGRITY OF ALL WATERTIGHT DOORS ON VESSEL.

CROP AND INSERT AS ORIGINAL DEEP WEB FRAMES #88 AND #94 PORT AND STARBOARD FROM WEB TO WEB AND FROM TANK TOP UP TO GOOD METAL.

Prove proper installation and satisfactory operation of car deck exhaust fans and provide approved plans.

Complete temporary repairs to overhead in car deck per plan proposal in BayEngineering letter dated 29APR96.

Make addition to sprinkle zone 9 to provide adequate coverage under ramps including approved plans.

Repair and make all watertight doors watertight.

Prove proper installation and operation of mid car deck ramps.

Crop and renew back to good metal all wasted overhead areas located above car deck.

PROVIDE CURRENT COMPASS DEVIATION CARD.

REPLACE RETROREFLECTIVE TAPE ON PILOTHOUSE EXPOSURE SUIT.

PROVIDE ACID RESISTANT LINING IN GENERAL ALARM BATTERY BOX. ALSO, REMOVE OLD BATTERIES AND WIRING.

CONDUCT WEIGHT TESTS ON LIFEBOATS #1, #3, AND #5; AND EXERCISE THE RELEASING GEAR ON THE SAME BOATS.

ADJUST CLOSING DEVICES AND/OR DOORS TO PROVIDE FULL CLOSURE INCLUDING LATCHING AUTOMATICALLY ON FIRE SAFETY DOORS #7 AND #8.

PROVIDE RING TERMINAL ON WIRE CONNECTED TO THE POSITIVE TERMINAL OF THE FIRE ALARM BATTERY.

REPAIR OR RENEW ON/OFF SWITCH FOR VENT FAN ON STARBOARD SIDE OF GALLEY.

RENEW DECK PLATING STARBOARD SIDE IWO PASSENGER WALKWAY MAIN CAR DECK BETWEEN FRAMES 142 AND 150. THIS REPAIR TO BE COMPLETED PRIOR TO PLACING VESSEL INTO SERVICE SPRING 93, AND NO LATER THAN COMPLIANCE DATE.

OPEN BOILER MOUNTINGS FOR INSPECTION BY A USCG MARINE INSPECTOR NOT LATER THAN 30 NOVEMBER 1992. IF THE PROPULSION PLANT IS OUT OF SERVICE, THIS REQUIREMENT MAY REMAIN OUTSTANDING UNTIL 30 APRIL 1993.

MAKE LIFEBOAT #5 AND ITS EQUIPMENT AVAILABLE FOR INSPECTION, AND CONDUCT A WEIGHT TEST AS SOON AS THE LIFEBOAT IS PLACED BACK ABOARD BUT IN NO CASE LATER THAN JULY 1, 1990.

REPAIR BOW ANCHOR WINDLASS AND PROVE SATISFACTORY OPERATION.

PLACE #5 LIFEBOAT BACK ON BOARD THE BADGER COMPLETE WITH ALL EQUIPMENT NOT LATER THAN 15 JULY 1989. UNTIL THIS REQUIREMENT IS COMPLETED, THE VESSEL IS LIMITED TO A MAXIMUM OF 560 PERSONS (TOTAL OF PASSENGERS AND CREW).
