Electrical equipment must be installed to protect from electrical hazards. Electrical wiring underneath operating station had 2 electrical sockets missing face plates and one breaker box missing its cover. Also, one electrical socket on the starboard side E/R was missing its face place. Properly cover to prevent potential shock or fire hazards.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation. Update radio to include MMSI,LAT,LONG and notify local OCMI. 47 CFR 80.905

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light in Laz was missing light cover. Replace and notify local OCMI.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in engine compartment that are going to be used for an additional camera. Install camera or remove dead end wires and notify local OCMI.

Vessel required to have one, 24" ring lifebuoy fitted with 60' lifeline and floating water light attached with a corrosion resistant clip to allow quick disconnect. Vessel did not have lifeline onboard. Provide proper arrangement of ring lifebuoy, lifeline, and waterlight.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide— ‘‘CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.’’. (2) Spaces protected by carbon dioxide—‘‘ CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.’’ (3) Spaces into which carbon dioxide might migrate— ‘‘CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.’’ Vessel spaces were not marked. Mark spaces appropriately and notify local OCMI.

Vessel has damage to hull port side, amidships, approximately 4' above waterline cause by a vessel collision and enacted temporary repairs. Vessel is permitted one time transit to South Coast Shipyard in Newport Beach to conduct permanent repairs. Vessel is permitted to transit without passengers, in fair weather only. Notify local [name withheld]/LB once vessel arrives and submit a repair proposal to Sector LA/LB for approval.

Vessel reported damage to hull portside, amidships, approximately 4' above waterline cause by vessel collision. Make vessel available for damage survey by USCG marine inspectors and submit a repair proposal for OCMI approval.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry 12 flares. Vessel is short two flares. Acquire two (02) additional flares.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The marking is not required on a life jacket carried to meet a temporary need for additional life jackets, if the life jacket has the name of another vessel or company marked on it. For an immersion suit, the name of the person to whom the immersion suit is assigned is an acceptable alternative to the name of the vessel. 01 of the vessels required ring life buoy did not have the vessel's name on it. Write the vessel name on ring life buoy.

Vessel had 02 dead-end wires in the engine room and 01 in the midship void. Properly repair or remove dead-end wires.

The painter must: Be of at least 100 feet, Have a breaking strength of at least 680 kilograms (1,500 lbs.),(3000 lbs.) if 50 or more persons, Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light; Be stowed in such a way that it runs out freely. Replace deteriorated life float painter lines with appropriate. Ensure that no stowage is in and around life floats so lines and floats can float free.

The floating water light must be attached around the body of the life float or buoyant apparatus by a (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least (18 feet) in length. Attach water lights to floats and ensure they are float free.

The pump outlet must be fitted with a pressure gauge. Replace fire pressure gauge.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Replace wasted retroreflective tape on life rings.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Purchase new approved First Aid kit, with ammonia inhalants and aspirin

A vessel on an oceans, coastwise, limited coastwise, Six hand red flare distress signals and Six hand orange smoke distress signals or 12 hand red flare distress signals. Replace outdated flares.

Each ring life buoy on a vessel must: Be readily accessible, Be stowed in a way that it can be rapidly cast loose. Locate ring bouys around vessel making them accessible be deployed.

Life floats must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers. Place spacers between each float.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Replace wasted retroreflective tape on life rings.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Purchase new approved First Aid kit

Each EPIRB, must be tested monthly, and shall be logged or otherwise documented, as applicable. Test and log EPIRB monthly.

Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. Have FCC-licensed technician issue a new Safety Radiotelephony Certificate.

A fire hose with a nozzle must be attached to each fire hydrant at all times. Replace broken Nozzle on port fire hose.

Vessel port side rudder post found to have excessive leaking. Adjust packing to prevent leakage.

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Junction box identified on stbd side of amidships accommodation space found to be not secured. Secure to prevent movement while underway.

Center Main diesel engine belt drive unit missing guard. Install machinery guard.

Vessel found to lack efficient means of shutting down ventilation to CO2 protected engine space. Provide means of shutting down ventillation to engine space.

Wire identified on stbd side of amidships accommodation space found to have outer protective covering wasted away. Repair, remove, or replace damaged wire.

Wet exhaust on center aft main propulsion engine found to be leaking at flex hose, and at the aft bulkhead. Repair wet exhaust to prevent leakage.

Vessel's EPRIB battery was found to be expired as of 01/2021. Provide EPIRB with valid battery. Vessel is restricted to less than 3 miles from shore until corrected.

The aft stbd fire hydrant was found with a broken handle on the cut-off valve. Replace valve handle and prove proper operation.

02 life floats located on the wheel house cabin top were found heavily coated in bird feces. Clean life floats to restore bright orange color.

Provide all deck hatches with adequate securing devices.

Provide bilge suction piping/flexible hose with double hose clamps. Hose clamps were found missing from flexible hose.

Provide all three main engine #6 injector fuel piping with chafe protection. Replace the stbd engine #6 injector fuel line with factory replacement.

Repair or replace fire main pressure gauge. PSI gauge was found inoperable at time of inspection.

Clean and degrease griddle ventilation hood and ducting. Excess grease buildup was found at time of inspection.

VESSEL IS REQUIRED TO HAVE 6 RED HAND FLARES BUT ONLY HAD 4 ONBOARD. VESSEL IS RESTRICTED TO LAKES, BAYS AND SOUNDS ROUTE UNTIL 6 RED HAND FLARES AS REQUIRED FOR OCEANS ROUTE IS ONBOARD AND VERIFIED.

FORWARD BLKHD IN ENGINE ROOM HAD A RAG STUFFED INTO A PIPING PENETRATION. MAKE PIPING PENETRATIONS WATERTIGHT.

VESSELS FIRE PUMP OUTLET PRESSURE GAUGE IS INOPERABLE. REPLACE GAUGE.

SWITCH BOX IN ENGINE ROOM WAS FOUND WITH 2 SEVERED GROUND WIRES OUTSIDE OF ENCLOSURE. ENSURE WIRES ARE CONNECTED PROPERLY AND CLAMPING IS AROUND THE INSULATION PART OF THE CABLE.

ENSURE FCC STATION RADIO AUTHORIZATION CERTIFICATE IS VALID AND ONBOARD.

ENSURE LIFE FLOATS ARE STOWED WITH SPACERS TO SEPARATE EACH FLOAT.

CLEAN AND PAINT LIFE FLOATS STOWED ON WHEEL HOUSE NOT IN SERVICABLE CONDITION

Vessel taking on water IVO starboard side chine forward; determine the extent of the damage and submit repair plan for approval.

Crop and renew the wood damage to the satisfaction of the attending Marine inspector> prove Sea Trials after the vessel is repaired.

Removed G-10 material at fwd staboard side and remove splash zone on starboard side forward bilge.

High pressure fuel line on starboard engine from filter to main has leak. Needs replaced. Make repair and demonstrate proper operation of the vessel to the satisfaction of the attending marine inspector.

Provide cover or guard on all four engines.

Renew firepump gauge and prove proper operation.
