Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Containers for life jackets were not correctly marked. Make containers and stowage as required by regulation and attending Marine Inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date tide tables. Current tide table Publications were not available for inspection. Make publications available for inspection.

Employers must ensure that they and their crew members meet the requirements of Part 16 - Chemical Testing. Employees were not able to provide any proof of enrollment in the vessel's chemical testing program. Provide proof of crew members enrollment in chemical testing program with confirmation from consortium that vessel is in current good standing to attending Marine Inspector.

Vessel must meet survival craft requirements as per the table in 46 CFR 180.200(c). Vessel is currently missing 01 (30 person) IBA as required on the vessel's COI due to the vessel's route and condition. Vessel has a loaner (25 person)IBA currently installed on the vessel. Vessel is operating in cold water months for its operating area as dictated in NVIC 07-91. The vessel is limited to 37 total persons allowed (this number includes crew) due to requiring aggregate capacity primary lifesaving that will accommodate at least 67% of the total number of persons permitted on board. This CG-835 will remain in effect until such time that the vessel is able to prove satisfactory primary lifesaving for additional persons.

Employers must ensure that they and their crew-members meet the requirements of 46 CFR Part 16. Vessel could not provide proof of crew members enrollment in a drug and alcohol program required by 46 CFR Part 16. Provide proof of crew enrollment in required drug and alcohol program as required by 46 CFR Part 16 to attending Marine Inspector.

Vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have original Safety Radiotelephony Certificate onboard. Provide proof of satisfactory Safety Radiotelephony Certificate to attending Marine Inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. Vessel did not have electronic versions or hard copies of these publications. If vessel intends to use electronic versions of these publications they must be downloaded for immediate use.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Missing donning placard for a type of life jacket available on-board. Display placard as required and to the satisfaction of attending Marine Inspector.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather,or man overboard conditions. Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in §185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel must update emergency procedures as addressed during vessel's annual inspection to address the particular equipment, arrangement, and operation of the vessel.

Strong and effective rudder stops must be provided to prevent jamming and damage to the rudder and its fittings. These stops may be structural or internal to the main steering gear. Starboard rudder stop had large gap at hard over (port & starboard). Make rudder stops as required as per regulation and to the satisfaction of attending Marine Inspector.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of 46 CFR. Waterlight had incorrect type battery. Replace battery as per manufacturer requirements.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, and equipment. The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Vessel's crew is not available off-season for a satisfactory and sufficient inspection of the vessel to take place. Vessel is currently not operating under its COI. Vessel has provided drug and alcohol program information as objective evidence to begin COI renewal process prior to COI lapsing and will make vessel available for inspection, with full crew, for underway operational testing at 1000 on 23 March 2022.

Ring buoy dry rotted and not serviceable. Provide serviceable ring life buoy.

First aid kit missing items. Provide current and complete first aid kit.
