A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel reported an pinhole leak in the bulkhead between the starboard #8 and #9 ballast tanks. Vessel shall proceed to Buffalo, NY to discharge cargo then proceed to Erie, PA for winter lay-up. Make appropriate repairs/renewals to bulkhead during winter lay-up 2025 and prior to returning to service.

An inspection either general or partial depending on upon the circumstances shall be made whenever any important repairs or alterations are undertaken. Master reported a pin hole leak in the #9 stbd ballast tank adjacent to the #5 cargo hold. Vessel is to make repairs within 14 days to the satisfaction of class and/or USCG.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Affect repairs to the crack in the forepeak sideshell to the satisfaction of class and/or USCG. 46 CFR 92.01-10(a) / 15 / ac

Vessel rep could not provide proof of proper operation of the generator.

Lifejackets were not marked with the vessels name

Generator CO2 shutdown was not tested during fixed fire fighting system service

Fuel oil reach rod Shut off for the Bow Thruster prime mover fuel tank was not labeled. Vessel owner to Label Fuel Shut offs in accordance with the regulations. Due date June 04, 2020.

If, at any time after a Certificate (COFR) has been issued, a certificant becomes aware of a change in any of the facts contained in the Application or supporting documentation, the certificant must notify the Director, NPFC, in writing within 10 days of becoming aware of the change. A vessel or operator name change or change of a guarantor must be reported by the operator as soon as possible by telefax or other electronic means to the Director, NPFC, and followed by a written notice sent within 3 business days. (See, §138.45, Where to apply for and renew Certificates, for contact information). Obtain change through NPFC on COFR & submit to this OCMI

Electric installations on vessels must ensure, Maintenance of services necessary for safety under normal and emergency conditions, and the protection of passengers, crew, other persons, and the vessel from electrical hazards. Vessels forward winch power is being bypassed to fwd electrical panel. Have winch installation made properly as per manufacture specifications. Elec. must be installed per methods as incorporated by reference and applicable in 46 CFR 111. Once rectified please send photographic proof along with statement from C/E.

Closures for hull openings at or below the freeboard deck must be as strong as the structure to which they are attached and must be watertight. Port light cover broken. Repair/replace port light cover. Once rectified please send photographic proof along with statement from Captain.

Wiring Materials and Methods must be made in a way where wire must be in an enclosure. No dead end wiring even if from a non powered source. Fwd quarters areas still found to have exposed dead end wiring. Conductors must be removed or made properly in an enclosure using acceptable install methods as incorporated by reference and applicable in 46 CFR 111. Once rectified please send photographic proof along with statement from C/E.

Temporary repairs, two four foot doublers were placed over 20 inch Port & Stbd Ballast cross over line; located in Aft Lower Machinery space, under and in way of the old auxiliary boiler. Prior to 31 March 2019 remove doublers and make permanent repairs. Repairs to be satisfaction of and verified by a Coast Guard Marine Inspector.

Temporary repair weld placed on a 3 inch fracture between, in and on the #8 & #9 Port Ballast tank water tight bulkhead. The fracture & welds are located in way of the inboard longitudinal bulkhead & slope plate. Permanent repairs shall be made to satisfaction of ABS Class Surveyor on or before May 8th, 2018. To clear this requirement submit ABS Surveyor report to attending CG Marine Inspector.

Observed water leaking and fracture in 20 inch ballast cross over pipe (Aft Lower Machinery room centerline). Make temporary or permanent repairs prior to vessels movement. 46 CFR 56.50-50

Water leaking from #9 Port Ballast Tank into unloading tunnel. Suspect fractured dredge pump line. Invest cause and make temporary or permanent repairs prior to vessels movement. 46 CFR 56.50-50

Side shell plate at port auxiliary 8 inch ballast suction (aft machinery space, lower level port side) clad welded at hull. Repairs holding and effective. Make permanent repairs during vessels next dry-docking or before March 31, 2019. 46 CFR 56.50.95

(3) cargo boom support cable end thimbles have cracks, at the aft ends of the port outboard and starboard inboard and outboard cables. Repair and conduct a load test on the cargo boom at the next unloading port, to the satisfaction of the attending ABS surveyor.

Shock hazard present at outlet to aft side of cargo hatch #17, j-box to starboard aft winch boarding ladder controls, and open j-boxes in starboard generator room, where missing covers. Fit and install correct cover.

Oxy-acetylene tanks not properly stowed and secured in Conveyor man's workshop. Properly secure

Observed loose reach rod assembly to valve in bow thruster room fuel tank. Inoperable reach rod assembly aft, opposite of generator fuel oil shutoff valve. Repair

ABS Surveyor must complete hatch inspections on 8, 12, 18, and 21 to ensure proper seal.

AC unit and bulkhead opening in crew lounge does not meet Load Line specifications and must be removed and bulkhead brought to Load Line standard. Install steel plate and fully weld.

CO2 discharge piping to forward rope locker leading from adjacent paint locker is not adequate for space. Correct

Reach rod for fuel cutoff valve near port transfer pump requires repairs; demonstrate proper operation.

Make repairs to the crack (15") in the hull ivo(6'forward) of frame 252. Crescent shaped crack is 13' from the main deckon the unload/generator deck, stbd side in the notch. All repairs to the satisfaction of the local OCMI.

There is a temporarily-repaired hair-line fracture located on the stbd pin-box hull (inboard notch) that extends approx 12" in length (located ~6 fwd of FR-252, 13' from the main deck on the unload/gen deck). Make permanent repairs by cropping and renewing as original. Repairs must be examined by a USCG Marine Inspector prior to the vessel's return to service after winter 2017 lay-up period.

The vessel's cargo hatch clamps/securing hardware is unusable/corroded. Ensure all cargo hatch securing hardware is replaced if found to be unusable IAW 46 CFR 97.15-20.

An 8-10" split riveted lap seam was reported in the #1P ballast tank in way of FR32, approx 30" above the bottom of the hull, on the curved section of the plate. The plate appears to be inset approx 2" and is weeping. Vessel is authorized to proceed to Toledo, OH to affect repairs. Extent of repairs to be determined upon arrival by USCG and ABS. Repairs to be completed prior to departure.

A cement box extending 3 frame bays in way of FR32 in the #1 port-side ballast tank was installed to affect a temporary repair for inset damage noted/reported on 22JAN15. This damage is located on the bilge turn and resulted in a weeping split riveted lap-seam. Vessel to inspect cement box quarterly, and make permanent repairs, by no later than next drydock period in 2018.

The vessel's cargo hatch clamps/securing hardware was unusable and corroded. Required O/O to ensure all securing hardware was in good condition and replace any corroded elements.

Install temporary box patch between frames 19 and 17 in the fore peak tank on the port side. 46 CFR 91.30-1

Complete permanent repairs to damage in fore peak tank on the lap seam between side shell and bottom plating approximately between frames 19 and 17. 46 CFR 91.30-1

Port and Starboard anchors inoperable due to misaligned chain and frozen brake, corrected during inspection.

Most of the main deck cargo hatches appeared non-weathertight as they were twisted and warped. Corrected during inspection.

Forward Thruster Space bilge contains excessive oil, fuel, antifreeze and bilge water. Prior to departure from port, remove all contaminated liquid from bilge as per 46CFR 91.25-45

The batteries in all all float ights have expired dates. Install new batteries with experation dates beyond the date of this exam

Prior to operation remove /repair the multiple damaged electrical fixtures which could result in personnel contacting energized prior to sailing. Currently the vessel has multiple broken light fixtures which all appeared to be energized readily accessible to personnel. These are operational circuit, post conversion.

Remove accumulations of oil which could create a fire hazard in the bow thruster room. Currently excessive amounts of oil are in the bilges due to poor housekeeping.

Prove proper operation of the Luddel deck wash down thermal fluid heater automation and shut downs or provide a servicing report to an attending Marine Surveyor prior to operation of the thermal fluid heater. Currently the thermal fluid heater cannot be operated due the draft of the vessel and the location of the raw water suction.

Prior to operation of the Ship Service Generator prove proper operation or provide a servicing report certifying proper operation of the generator including all safety features as designed to verify satisfactory condition for the service which it is intended. Currently the vessel personnel were no familiar enough with the complex control system to prove it would operate as designed.

Submit proper plans for approval for the lighting system on main deck prior to energizing the unauthorized circuits. Currently the vessel has a modified deck lighting system with components that are not do not appear to approved for marine use, the components appear to residential house hold electrical components.

Provide proper non-conductive matting in the machinery space IAW 46 CFR 111.31-11. Currently the vessel does not have matting behind the switch board.

Provide an approved embarkation ladder at the life raft launching station IAW 46 CFR 199.110(f).

Provide a servicing report for the deicing boiler verifying proper operation of the alarms and safeties or conduct testing in the presence of a CG Marine Inspector IAW 46 CFR 61.30-20. Currently the deicing boiler is not operable due the draft of the vessel in light ship condition.

Provide a servicing report for the generator verifying proper operation of the alarms and safeties or conduct testing in the presence of a CG Marine Inspector IAW 46 CFR 61.30-20. Currently the vessel crew does not have the expertise to test the generator with its computer control system.

Provide proper battery boxes to the both generators to prevent damage from falling objects IAW 46 CFR 111.15-5. Currently both the ship service and the unloading generator have no covering on the batteries.

Provide proper fuel hoses meeting SAE J1942 standard IAW 46 CFR 56.60-25 on the ship service generator. Currently the package generator has non approved collapsible fuel lines.

Repair or replace the generator day tank transfer pump to prevent leakage at the shaft seal IAW 46 CFR 50-75(b)(5). Currently the shaft seal is leaking diesel and sorbents are being used to remove the fuel from the containment area.

Properly remove and dispose of diesel and waste oil from bow prop bilges to prevent fire hazards IAW 46 CFR 91.25-45. Currently the vessel has excessive amounts of diesel and waste oil in the bilges due to changing fuel filters and oil filters.

Remove remaining debris between both boiler casings (inboard of both mud drums). To be completed for next years lay-up inspections.

Provide a proposal to this office for bringing the forward life raft launching arrangements into compliance with 46 CFR 199.130(a)(7) and 46 CFR 199.130(c)(3) as required by 46 CFR 199.03(b)(7). Each life raft must be arranged to permit it to drop into the water from the deck where it is stowed. Presently the forward life raft position prohibits the raft from being launched into the water

Launch, release and recover both lifeboats in accordance with 46 CFR 199.180(d)(3), shall include a full load weight test (filled with water, lowered, brake applied) & a light load test, involving lowering & release of the lifeboat. After completing the full load weight test; davits winch, falls, & boat shall be thoroughly inspected, Master shall submit report to MSD StBay of test results.

Provide approved plans for the newly installed auxiliary boiler automation/control panel. Plans shall be submitted to USCG Marine Safety Center for review and approval. 46 CFR 91.55-5

Remove the two boiler mounts (main steam stop and relief) for inspection as required by 46 CFR 61.05-10 (table). Currently the Auxiliary boiler mounts removal requirement is two years past due.

Renew the aft EPIRB hydrostatic release (HAMMER Release). Current hydrostatic release is expired. 46 CFR 199.510

Fix or replace the #2 fireman's outfit scott air pack (SCBA) low O2 alarm, or remove the SCBA from the vessel.

Update fire control plans to meet the requirements of 46 CFR 97.36-1

Move forward life raft to the new proposed location at the bow just aft of the jack staff (aft of the bulwarks) to bring the forward life raft launching arrangements into compliance with 46 CFR 199.130(a)(7) and 46 CFR 199.130(c)(3) as required by 46 CFR 199.03(b)(7). Each life raft must be arranged to permit it to drop into the water from the deck where it is stowed.

Submit vessel response plan to C.G. MSC for amendment/changes IAW 33 CFR 155.1070. Provide documentation to C.G. OCMI duluth prior to 24 May 06

Provide documentation of EPIRB registration to OCMI duluth.

Install exhaust lagging on bow thruster engine prior to 24 May 06.

PROCEED DIRECTLY TO DULUTH/SUPERIOR HARBOR (FRASER SHIPYARD) FOR PERMANENT REPAIRS TO RUDDER DAMAGE. HAVE SUFFICIENT TUG(S) READY IN DULUTH/SUPERIOR HARBOR TO ESCORT/ASSIST VESSEL TO THE SHIPYARD.

PROVIDE A SMOOTHLY OPERATING RUDDER ANGLE INDICATOR ON THE BRIDGE PRIOR TO DEPARTING FRASER SHIPYARD (FOLLOWING RUDDER REPAIR). CURRENT INDICATOR IS STICKY AROUND THE MID-SHIPS POSITION.

Have damaged frames in the vicinity of frame 23 port, between the main and spar decks, surveyed by class surveyor. Notify cognizant OCMI of results to clear this requirement.

Exercise lifeboat underway with crew of eight or more to the satisfaction of the cognizant OCMI.

MAKE PERMANENT REPAIRS TO THE SIDE SHELL, WEB FRAMES AND SECOND DECK, PORT SIDE BETWEEN WEB FRAMES 184 AND 188. REPAIRS ARE TO BE MADE DURING LAY-UP, PRIOR TO NAVIGATION IN 1988 BUT IN NO CASE LATER THAN 15APR88.

PROVE OPERATION OF UNLOADING GENERATOR AND SWITCHBOARD PRIOR TO LOADING CARGO

REPLACE AND PROVE OPERATION OF AUTOMATIC VOLTAGE REGULATOR FOR #2 GENERATOR

INSTALL FLASHING RED LIGHT AND BUZZER IN ENGINEROOM TO OPERATE ON MAIN BOILER SHUTDOWN DURING AUTOMATED OPERATION.

INSTALL THE FOLLOWING ITEMS FOR AUTOMATION OF AUX BOILER: A.PREPURGE AND POSTPURGE, B. LOW WATER CUTOUT ALARM, C. DOUBLE FUEL OIL SOLENOID. ALSO PROVIDE PROCEDURE FOR TESTING THE AUTOMATION ON THE AUX BOILER.

CONDUCT WEIGHT TEST OF STBD LIFE BOAT

PERFORM EMERGENCY DRILLS.
