Port side genset inoperative due to faulty starter. Company is awaiting replacement part. Complete repairs and demonstrate proper operation to attending CG Marine Inspector. Vessel must be provided with and maintain battery back-up power for vital systems until second genset is repaired & operational.

Crew is unable to provide accurate documentation/attestation to the amount of cargo/baggage that is carried. This is in direct contradiction to the vessels issued stability letter. The carriage of passengers and cargo/baggage combinations is not authorized until accurate weight calculations can be obtained for the vessels crew/shore-side facility personnel and presentable to the OCMI.

1 pilothouse window is cracked and must be replaced.

All dead-end wiring must be properly terminated &/or removed. Any new wiring installations must be properly approved prior to installation.

Vessels engine room has accumulation of oil on the machinery and oil/trash on deck plates and in the bilges, all must be properly cleaned.

Auxiliary heating boiler has been installed by reputable vendor with Marine Inspector oversight, however submitted installation plans & equipment list have yet to be approved by the MSC. Until approval is granted, installation confirmed to be in compliance with these approved plans (to include an emergency fuel shut-off valve), hydrostatic test & operational testing of all the installed safety devices are witnessed by CG MI, operating the boiler is prohibited.

Maindeck overhead sprinkler system diffuser heads must be standardized, both relating to type and position. Correct prior to "winter season" operations.

Pilothouse emergency comms battery requires proper cover.

Gensets do not have emergency fuel shut-offs. Install & demonstrate proper operation. Correct prior to "winter season" operations.

Cover missing from electrical J-box under stbd side of helm. Replace.

Several wasted areas sighted at maindeck forward area outboard by loading doors. Correct prior to "winter season" operations.

While underway carrying passengers en route to Mackinac Island, Captain reported feeling a resistance in the steering system. Upon investigation deckhands found the forward-most chain knuckle in the steering shaft to be cracked. This is the second time in three days the steering had bound up and caused this same knuckle to crack under heavy torsion. Upon inspection, MI found that the steering shaft had shifted approximately 1/4 to 3/8's of an inch at numerous spots along the joints and unions. As well as the key had rolled in the key-way at the section where the shaft reduces from 1 1/2 down to 1 inch. Submit repair proposal to this office for approval in correcting failures occurring in the steering system and, once approved, affect repairs to the satisfaction of the attending Coast Guard Marine Inspector.

Mechanics found flattened cam lobe on MDE. Effect repairs and confirm safe operation via underway trials with Coast Guard Marine Inspectors.

Locally fabricated steering gear pillow block/bearing must be replaced with a properly manufactured and system appropriate pillow block/bearing. Installation and operational testing must be witnessed by attending CG Marine Inspector.

Replace rejected (burst) maindeck fire hose.

Sighted many areas of concern relating to corrosion/wastage. Areas included: lazarrette, stairs to 2nd deck, port side mooring bits, holding tank compartment, accommodation space outer edges, pilothouse landing and deck, along with port qtr damage.

Engineroom – sighted MDE oil leak, MDE exhaust leak, throttle/transmission linkage needs to be professionally assembled with proper hardware. Oil leak on genset at dipstick requires repair/replacement. Transmission oil temp alarm constantly on in pilothouse, sensor/transducer on order.

Found 1-50 person IBA servicing expired April 2022.

Found all 4 IBAs painter lines not properly secured. Instructed crew how to properly install & regularly inspect.

Vessel experienced a loss of steering while transiting between Mackinac Island and St. Ignace, MI. The cause appears to be damage sustained to the rudder by ice. Assess damage, submit repair proposal to OCMI, repair IAW approved plan, demonstrate proper operation prior to carriage of passengers.

Vessels 4-50 person permanent IBAs servicing has expired; IBAs must be sent out for annual servicing. Vessel is not authorized carriage of passengers until IBAs are serviced or replaced & the OCMI is notified.

Make vessel available for underway operational testing, demonstrate emergency drills. All to be witnessed by CG Marine Inspectors.

Clean and descale port side sewage void and schedule CG Marine Inspector to attend and inspect.

Fire extinguisher bracket is loose on the deck in engine room. Secure fire extinguisher bracket.

Vessels 4-50 person permanent IBAs have been sent out for annual servicing. Replacement IBAs total capacity is 160 persons (2-50 person & 2-30 person). Vessel is limited to 320 total persons allowed until permanent IBAs are re-installed.

A quarter sized crack is present on the stbd side, 6 inches from the keel, in the void aft of the forepeak. Submit repair proposal for approval and effect permanent repairs to hull plating to the satisfaction of the attending marine inspector prior to carrying passengers.

Bilge suction piping wasted, leak caused loss of suction, corrected during inspection.

The vessel is currently broadcasting (1) Improper MMSI Number, (2) Improper Call Sign, (3) Improper Ship Type. Ship Type should be a two digit number (60) Per the US Official Encoding guide.

Wastage identified in internal structural members. Areas identified to be cropped and renewed.

Provide oil resistant insulation to prevent combustible materials from coming into contact with hot surfaces of engine exhausts for main engine and ship service generators.

On the hot water heating boiler, relocate safety valve directly into boiler casing, and provide servicing report that documents testing and proper operation of all safefty devices and operating controls.

Main fuel oil tank top severely wasted and epoxy coatings have not been maintained. Tank top was cleaned and new epoxy was applied to maintain remaining integrity until permanent repairs can be completed, related to outstanding requirement #10 issued 10/23/13 per Activity 4748242.

Make permanent repairs/ weld 1" fracture in port quarter steering compartment just below rubrail.

Replace missing quadrant nut on bracket.
