Every fuel oil pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank...shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which the tanks are situated. During the test of the vessel's quick closing valves in group 10, one (01) valve failed to operate and close upon activation from the ECR. Deficiency was corrected during the examination and cleared on the spot.

Stations: 2.8, 3.5,and 3.10. Specifically, an electrical panel in A.C. Station 3.10 was completely removed, exposing live wires and electrical equipment. A.C. Stations 2.8 and 3.5 had missing blanks that led to exposed electrical hazards. Additionally, PSCO discovered a broken speaker on deck 9 in the vicinity of cabin C627 exposing internal wires and components.
