Every fuel oil pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank...shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which the tanks are situated. During the test of the vessel's quick closing valves in group 10, one (01) valve failed to operate and close upon activation from the ECR. Deficiency was corrected during the examination and cleared on the spot. 74 SOLAS 97 II-2/15.2.5

The bulkhead deck or a deck above it shall be weather tight. All openings in the exposed weather deck shall have coamings of ample height and strength and shall be provided with efficient means for expeditiously closing them weather tight. The master, owner, operator, agent, or person-in-charge of all vessels equipped with ballast water tanks that operate in the water of the U.S. must...remove fouling organisms from the vessel's hull... PSCO's observed excessive growth on hull beneath the waterline. 33 CFR 151.2050

Fire doors in main vertical zone bulkheads and stairway enclosures, other than power-operated watertight doors and those which are normally locked, shall be of self-closing type capable of closing against an inclination of 3.5 degrees opposing closure… PSCOs observed fire screen door 7/V/36 incapable of closing and latching properly. 74 SOLAS (81 Amend) Ch. This prevented lifeboat #2 from returning to the mothership under its own power and forced an emergency mooring to the pier, aft of the mothership. Lifeboat #7 reduction gear would not clutch ahead. Maintenance, testing and inspections shall be carried out in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. After any survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in the structural arrangments. machinery, equipment and other item covered by the survey, without the sanction of the Administration. Found 02 cracks: one on the Fire Screen Door 6/Ill/37 interior of the frame inside of A.C. Station 3.6 and one on the exterior frame of Fire Screen Door 5/||1/18. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Objective evidence discovered in the expanded ISM examination revealed the following: The vessel failed to fully implement the requirements of the vessel's maintenance procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules. This is made evident by deficiencies #01 and #02 as shown in the satisfactory inspection of their lifeboats as logged on 05JUL2023. Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO discovered damaged panel doors and exposed wiring in A.C. Stations: 2.8, 3.5,and 3.10. Specifically, an electrical panel in A.C. Station 3.10 was completely removed, exposing live wires and electrical equipment. A.C. Stations 2.8 and 3.5 had missing blanks that led to exposed electrical hazards. Additionally, PSCO discovered a broken speaker on deck 9 in the vicinity of cabin C627 exposing internal wires and components. 74 SOLAS 04 II-1/40.1.3 09209 50ac

A Passenger Ship Safety Certificate shall be issued after an initial or renewal survey to a passenger ship which complies with the relevant requirements of chapters II-1, II-2, III, IV and V and any other relevant requirements d the present regulations. The issued Passenger Ship Safety Certificate Form P has a total persons allowance based on life-saving appliances of 3196 persons. In order to meet the requirements of 74 SOLAS II-2/21.1.1.1, the vessel is required to have a minimum of 37.5% lifeboat capacity on each side. The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identif the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. All steering gear components shall be of sound and reliable construction to the satisfaction of the Administration. Port steering gear piping has an active hydraulic leak.

Basic principles are minimization of possibility of ignition of flammable cargo vapour. There was approx 5 gallons of standing diesel in the ten inch fuel vent drain pipe.

Severe wastage and holes in the deck throughout the entire evaporator room.

Port side Rescue boat suffered casualty while being lowered for routine operations. Vessel has hole punched in the starboard side aft approximately 15 inches long. Vessel must repair rescue boat or recieve flag exemption letter prior to departure.

Ship was unable to lower No. 4 lifeboat due to discrepancies in the breaking system on the launching apparatus.

Laundry space (No. 4237) found lint behind washers & dryers. Pile of white towels on deck aft corner, fire hazards.

Sprinkler system lockers randomly check found rubbish & other personal items. E.G. No. 44 & 45 portside Deck 4, a good housekeeping needed on all lockers (fire hazards).

Bingo equipment secure on main escape route portside Vista lounge, one of the main passenger evacuation area.

3-55 gallon empty drums in stairway portside Fiesta deck 4. Found not ready for sea & blocking stairway.

Provide class report attesting to proper operation of No.1 and No. 2 bow thrusters.

Lifeboat No. 11 found with windshield scratched in way of the wiper. Visibility of operator platform restricted, safety item. Replacement required with consideration with different material resistant to ultraviolet rays..

Found steam piping leak in way of FS 01-11-03. Insulaion lagging soaking wet.

Found the two Emergency Diesel Generator front counter balance without safety guards on rotating machinery.

Missing A-60 insulation on padeyes, approximately 8, located in a way of the sewege discharge room STBD side and 2 areas overhead that were disturbed in a way of the Aft Fire Pump / chlorine injection system & aft of the FM pressure vessel F.W. room.
