On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette; (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. MI observed a shared visual alarm for both the engine room compartment and forward bilge when they should be separate. O/O to replace light to distinguish between high alarm in separate spaces.

A power-driven vessel underway shall exhibit a masthead light forward. Masthead light found inoperable; operator to repair/replace. Operations restricted to daytime only until light restored.

On a vessel of more than 65' in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. PA and General Alarm system found inoperable; no overnight passengers until PA/GA system repaired.

A vessel on an oceans route must carry 6 hand red flare distress signals and 6 hand orange smoke distress signals or acceptable substitutions as approved by [name withheld] 01 orange or red hand flare; operator to obtain.

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Hydrostatic unit not marked with expiration; operator to show proof of expiration or replace unit.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. No placard installed; operator to provide proof of emergency broadcast instructions posted at operating station.

Fuel shutoff valves must be fitted in the fuel supply lines, and must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Fuel shutoffs on weather deck were not easily accessible through hatch; operator to install access on hatch door for quick emergency response.

The inspection and maintenance of onboard survival craft shall be logged or otherwise documented for review by the Coast Guard upon request. No maintenance log found for visible maintenance to life rafts; operator to provide maintenance record.

Each buoyant apparatus must have Type II retroreflective material on each side and end. The material must be in strips at least 2" wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. Retro on buoyant apparatus found insufficient; operator to replace faded strips.

Life floats and buoyant apparatus, when stowed in tiers, the separate units must be kept apart by spacers. Operator to place spacers between buoyant apparatus.

Each ring life buoy must be marked with Type II retroreflective material. Retro on life buoy faded; operator to replace.

A ship greater than 26' in length must have a placard of 5" by 8", made of durable material fixed in conspicuous place in each machinery space, or at bilge and ballast pump control station, stating Discharge of Oil Prohibited clause. Placard was not installed; operator to install.

Ventilation of spaces containing diesel machinery- A supply duct must be provided with a cowl or scoop having a free area not less than twice the required duct area. Vessel missing cowl/scoop on ventilation- operator to install 01 port side and 02 starboard side to prevent water intrusion.

A vessel to which this subchapter applies may not be operated without having on board a valid USCG Certificate of Inspection. Vessel is overdue for annual inspection and restricted from operating until COI is satisfactorily completed.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1" high indicating purpose of the valve and direction of operation. Fuel shutoffs were not labeled; operator to label.

The master or person in charge of a ship shall ensure that one or more placards are displayed in prominent locations and in sufficient numbers so that they can be read by crew and passengers. These locations must be readily accessible to the intended reader and may include embarkation points, food service facilities, garbage handling spaces, living spaces, and common areas on deck. Each placard must be at least 8" wide by 5" high, made of a durable material, and legible. At a minimum, each placard must prominently state:(1) The discharge of all garbage is prohibited into the navigable waters of the United States and into all other waters except as specifically allowed;(2) The discharge of all forms of plastic into all waters is prohibited; (3) A person who violates the above requirements is liable for civil and/or criminal penalties; and(4) Regional, state, and local restrictions on garbage discharges also may apply. No garbage placards posted on board; operator to install.

If your vessel is not operated on international voyages and does not meet the conditions in paragraph (d) of this section, it must undergo a drydock and internal structural examination as follows unless it has been approved to undergo an underwater survey (UWILD) per § 176.615 of this part: A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel passed drydock examination drop dead date. Vessel to complete drydock exam prior to operating.

Portable fire extinguisher in berthing area was found empty and not usuable. Service or replace portable fire extinguisher.

Life buoy wast found wasted and unserviceable. Replace.

Liferaft light was missing pin to hold top on. Repair or replace lifefloat light.

7 lifejacket lights were inoperable. Fix or replace lights.

4 orange smoke distress signals expired in August of 2021. Replace expired signals

Vessel has a toilet but no installed MSD. Provide Type 1 or Type III MSD

Missing sound signaling device.

Flexible fuel hoses were found dryrotted and cracked. Replace fuel hoses

Vessel didn't have an approved First Aid kit onboard.

Vessel must have nav lights that are in compliance with the Nav Rules. Vessel's anchor light is inoperable and broken from mount.

Each vessel must have adequate emergency lighting fitted along the line of escape. Vessels emergency light from passenger accommodations to main deck is inoperable.

Any vessel of at least 5 net tons must have a valid Certificate of Documentation. Vessel did not have a valid COD onboard.

Provide a valid FCC station license onboard.

Provide NOAA registration sticker on EPIRB

Dry dock due date is 29FEB2020. Vessel not permitted to operate until satisfactory dry dock inspection is conducted to the satisfaction of the Coast Guard, which is scheudled for 19MAR2020.

Floating waterlights for both life floats and ring buoy have expired batteries. Replace with new batteries.

STBD generator is rated for 40 degrees C and is derated to 80%. Mount plackard or sticker showing generator is derated.

Sewage tank overboard discharge valve must have a means of locking installed and be locked at all times.

Obtain current FCC radio station license.
