DISCOVERY

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List. Vessel did not have up-to-date Coast Pilot and Light List. Obtain and keep aboard vessel up-to-date nautical publications.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Vessel's first aid kit had expired medications in it. Renew all out of date medications and have all required items onboard.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. Vessel was missing one of its 3 required fire buckets. Replace missing fire bucket.

Each survival craft must be: (1) Secured to the vessel by a painter with a float-free link permanently attached to the vessel. Vessel's liferaft painter was not attached to the weak-link. Attach painter to weak-link.

Each ring life buoy with a floating water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the ring life buoy. Ring buoy light lanyard was attached to life line. Secure line around body of ring buoy.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry— Six hand red flare distress signals and Six hand orange smoke distress signals. Vessel's distress signals were expired. Provide new distress signals on vessel.

n vessels described by 46 CFR 175.110(d) of this chapter, a passenger safety bill must be posted by the master in each cabin or stateroom, and in passenger accommodation spaces. Each passenger safety bill required by this section must list: (1) The embarkation station and the number and location of the survival craft to which each passenger is assigned, if applicable; (2) The fire and emergency signal and the abandon ship signal; (3) Essential action that must be taken in an emergency; and (4) If immersion suits are provided for passengers, the location of the suits and illustrated instructions on the method of donning the suits.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not have life jacket donning placard instructions for both types of life jackets. Post donning placard instructions for all types of life jackets.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. The aspirin in the first aid kit was expired. Re-supply the first aid kit with aspirin.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. The vessel did not have a life jacket donning placard for both types of lifejackets onboard. Post life jacket donning placards for all types of life jackets on board.

Each hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or to other devices to prevent its loss. The fore peak hatch did not have a device to secure it in place. Provide securing device for fore peak hatch.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel's EPIRB's battery was expired. Vessel is limited to within three (03) miles of coast until new EPIRB is installed on board vessel.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel does not have an installed interconnected fire detection system in required areas of vessel. Operator smoke must install UL 217 interconnected smoke detectors in all required spaces prior to carrying passengers in 2024.

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessels with a keel laid date after March 28, 2022, must include plans for the monitoring device(s) within the plan submissions required in 46 CFR 177.202. Operator has not submitted plans for installation of watchmen system. Operator must submit plans to include timeline for installation of watchmen system.

Passenger egress drills must be logged or otherwise documented for review by the Coast Guard upon request. Passenger egress drills were not logged as required. Operator to log passenger egress drills and provide proof of log to Marine Inspector after 30 days.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Operator was not logging emergency drills as required. Operator to log passenger emergency drills and provide proof of log to Marine Inspector after 30 days.

Adequate emergency lighting automatically actuated upon failure of the main lighting system shall be fitted along the line of escape in vessels having lounge areas below the main deck. Emergency lighting did not extend through alternate escape routes below deck. Ensure all escape routes from lower level are sufficiently illuminated with emergency lighting.

Each generator shall be protected with an overcurrent device set at a value not exceeding 15 percent above the full load rating of generators. The two ship service generators were connected to a disconnect switch but were not protected by an overcurrent device. Submit a plan for installation of overcurrent devices for generator and shore tie to local OCMI to include one line electrical drawing and materials list. Operator to repair prior to carrying passengers in 2024.

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Replacement of existing equipment, not including wire or cable, installed on the vessel prior to March 11, 1996 need not comply with the regulations in this part. Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Cabling for the lube oil change out pump and MSD were improperly spliced using unapproved electrical plugs and utilizing portable cord in a permanent installation. Remove plugs and portable cord and reinstall the correct size and type electrical cable for in accordance with 46 CFR 183.340. Operator to repair prior to carrying passengers in 2024.

Each ring life buoy on a vessel must be approved. Starboard aft ring life buoy not servicable.

The name of a vessel must be marked or painted in clearly legible letters and numbers on ring life buoy. Port aft ring life buoy markings are faded.

Each child size life jacket must be stowed in a location that is appropriately marked and seperated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. No marking for child size life jacket stowage and not separated from adults.

A vessel must comply with the applicable requirements for any radio and EPIRB installation, including installation certificates to be3 issued by the FCC as set forth in 47 CFR Part 80. FCC Safety Radiotelephony Certificate expired 02Sep2019.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information specific ot the individual vessel.

No MOB was conducted

Owner advised that CG marine inspector must be present to (1) examine the welding procedures; (2) witness the marking of the location of the cut-outs; (3) witness the fit-ups after the tack welding has occurred; (4) final inspection; and (5) NDT.

**NOV ISSUED** Vessel owner/operator advised via email on 29MAR19, 31MAR19 & 4APR19 that the CG marine inspector must be present to witness the cutouts of the vessel's hull. Did not heed this requirement and conducted cutouts prior to arrival of CG marine inspector. NOV mailed 4/18/19.

A vessel on a Coastwise route must carry six hand red flare distress signals. Vessel only had 4.

Each ring life buoy must be marked in clearly legible block capital letters w/ the vessel's name. Stbd aft ring life buoy illegible.

Crew overnight accommodations must be fitted w/ suitable equipment to provide for the safe operation of the vessel. Stbd aft bunk light extinguished.

Submit a written repair proposal for the proper installation of the vessel's four (4) overboard through hulls. Use USCG NVIC 7-68 for guidance for your proposal. Proposal must account for Coast Guard inspection of cutout, fit up, root pass/back gouge and final inspection with NDT. Proposal must be reviewed and approved by the OCMI prior to commencement of the repair. New installation must be completed 1 October 2018 or prior to carrying passengers after this date. **THIS CG-835 amends the CG-835 issued [name withheld] on 5/15/2018**

Provide specifications of generator set removed and generator set installed prior to carrying passengers.

Vessel not reporting to OCMI when conducting repairs or alternations to hull, machinery, or equipment that affect the safety of the vessel as per 46 CFR 176.700

Provide proof of all employees, crewmembers are enrolled in a drug testing program.

Replace deteriorated 2 inch cooling hoses in the engineroom.
