Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB battery found expired. Vessel restricted to operations within 3 miles from shore until replaced.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Flammable/combustible cans found on shelf adjacent to wheelhouse. Remove all flammable/combustible items from vessel.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Prove proper operation and general condition of the life float, by placing in the water for a period, to ensure watertight integrity.

Each life jacket carried on board the vessel must be marked in accordance with § 185.604 of this chapter. 01 lifejacket found without required markings. Mark lifejacket.

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Renew vessel COD and provide proof to the Coast Guard, Sector Detroit.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: (1) Inspection of each portable fire extinguisher, semi-portable fire extinguisher, and fixed gas fire extinguishing system to check for excessive corrosion and general condition Portable fire extinguishers not inspected monthly. Educated operator on requirement.

Abandon ship drills, man overboard drills, and fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel was not logging drills in the logbook. 46 CFR 185.520 ; 46 CFR 185.524

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date- charts; coast pilots; light lists; and notice to mariners. Vessel was not keeping up-to-date charts, coast pilots, light lists, and notice to mariners. (185.20-35 OLD T)

Lifefloat floating waterlight must be approved under approval series 161.010. Light did not have an attached approved battery. (46 CFR 180.175 (f)(4))

Each item of survival craft equipment must be of good quality and efficient for the purpose it is intended to serve. Vessel's lifefloat paddles and retroreflective tape were wasted and worn. 46 CFR 180.175(a) ; 46 CFR 180.175(f)(2) ;

If the life float does not have a painter attachment fitting, a means for attaching the painter must be provided by a wire or line that encircles the body of the device. Vessel's painter did not encircle the body of the life float. (46 CFR 180.137(d)(1))

Lifefloat waterlight must be attached around the body of the life float. Light was not attached around the body of the lift float. (46 CFR 180.175(f)(4))

The marine inspector shall examine the vessel's watertight bulkheads. Each vessel must meet the applicable subdivision requirements in Subchapter S. Vessel had multiple watertight bulkhead penetrations that were not watertight on both the foremost and aftermost watertight bulkheads.(46 CFR 176.25-5 OLD T; 46 CFR 177.13-1 OLD T)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel's first aid kit had expired asprin. (46 CFR 184.710)

Vessels of less than 20 meters in length, the sidelights shall be fitted with matte black inboard screens. Vessel's matte black paint was faded to gray. (33 CFR 84.05(a))

A vessel must carry adequate and up-to-date (1) Charts of large enough scale to make safe navigation possible. Update charts to notice to mariners weekly. This is second time deficiency issued (previously in 2021). CITE 46 CFR 184.420(a)(1)

A vessel must carry adequate and up-to-date (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Update publications to notice to mariners weekly. This is second time deficiency issued (previously in 2021). CITE 46 CFR 184.420(a)(2-5)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Donning placard for each type of pfd on board. This is second time deficiency issued (previously in 2021). CITE 46 CFR 185.516(a)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Provide detailed servicing report showing portable fire extinguishers are maintained IAW NFPA-10. 6 Year intervals not completed. This is second time deficiency issued (previously in 2021). CITE 46 CFR 176.810(b)(1)

The pump is equipped with a strainer that can be readily inspected and cleaned without removal. Provide suction strainers for bilge piping. This is second time deficiency issued (previously in 2021). CITE 46 CFR 182.520(e)(4)

One ship station license will be granted for operation of all maritime services transmitting equipment on board a vessel. Vessel does not have a valid FCC station license. Obtain license and provide copy to local inspector. CITE 46 CFR 184.502 47 CFR 80.13(b)

Each U.S. flag vessel subject to the Great Lakes Agreement must have an inspection of the required radiotelephone installation at least once every 13 months. Vessel did not have Great Lakes Radio Agreement inspection completed. Complete inspection and provide copy of certificate. CITE 46 CFR 184.502 47 CFR 80.953(a)

Each ship subject to the Great Lakes Radio Agreement must have on board an officer or member of the crew who holds a marine radio operator permit or higher class license. Provide copy of marine radio operator permits that work on vessel. CITE 46 CFR 184.502 47 CFR 80.161

The master of the vessel shall ensure that each EPIRB must be tested monthly. EPIRB not tested monthly. Test EPIRB monthly. CITE 46 CFR 185.728(a)

Drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: General description of the drill scenario and training topics. Drills were not logged not carried out as required. Log and complete drills per regulations. CITE 46 CFR 185.520(f) 46 CFR 185.524(d)

A vessel on a Great Lakes route must carry - Six hand red flare distress signals; and Six hand orange smoke distress signals. Vessel did not have flares. Provide 6 day and 6 night flares. CITE 46 CFR 180.68(a)

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 0.5 inches high: “DISTRESS SIGNALS”. Distress flare container was not labeled. Label distress flares per CFR. CITE 46 CFR 185.614

Each floating water light must be approved in accordance with § 161.010;and must have a lanyard. Ring life buoys reconnect light line and replace cracked battery. CITE 46 CFR 180.70(d)

Each life jacket must be marked in clearly legible block capital letters with the vessel's name. Life jackets not stowed separately; label with name of vessel and remove previous name. CITE 46 CFR 185.604(b)

Fill pipes and sounding pipes must be so arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. Fuel fill line STBD side is dry rotted and needs replacing. CITE 46 CFR 182.445(d)

All fuel tanks must be electrically bonded to a common ground. Port and STBD lines to be bonded (electrically). CITE 46 CFR 182.440(b)(4)

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Provide pre employment testing records and proof of enrollment of employees in a drug testing program. CITE 46 CFR 16.260(a)

A vessel must comply with the applicable requirements for any radio. Vessel had incorrect MMSI number input in the VHF-DSC radio. Input correct MMSI number. CITE 46 CFR 184.502

The EPIRB test required by paragraph (a) shall be logged. EPIRB test not logged. Provide copy of log showing completed test. CITE 46 CFR 185.728(c)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Alterations that affect the safety of the vessel include, but are not limited to repair or replacement, other than replacement in kind of electrical wiring, fuel lines, steering, alterations affecting stability, etc. Vessel owner installed hydraulic steering and an electric hatch opening device for the engine room hatch and did not notify the local [name withheld] modifications to the Marine Safety Center for review and approval. code changed to 50c, removed requirement to submit for steering, as it is soley operated hydraulically.

Any solid ballast used to comply with this part must be installed to the satisfaction of the [name withheld] crates of solid ballast in the form of cement bricks were found on port and starboard in the steering compartment. No indication of the presence of this ballast was found on the stability documents issued to the vessel in 2007. Stability must be verified.

Vessel operator contacted USCG for inspection on 22 Sept 2020. Vessel COI inspection was due no later than 28 Jul 2020, and was not completed. Vessel must undergo COI inspection prior to movement.
