At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. During man overboard crew did not demonstrate proficiency in responding to and retrieving a person in the water. Vessel crew did not communicate and coordinate effectively, did not follow emergency instructions and don life jackets during recovery, and had difficulty in maneuvering to recover the rescue dummy. Conduct training and drills as necessary and demonstrate a satisfactory man overboard drill to the attending USCG marine inspector.

Fire fighting, abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel was not able to provide records showing completion of drills. Conduct drills and provide records.

A supply duct must be provided with a cowl or scoop having a free area not less than twice the required duct area. Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Vessel modified port side engine room ventilation, and opening does not meet the size requriements (at least 24 square inches). Vessel does not have the means to close the new openings. Provide properly sized engine room ventilation inlets with means to secure them in case of a fire.

On vessels described by 46 CFR 175.110(d) of this chapter, a passenger safety bill must be posted by the master in each cabin or stateroom, and in passenger accommodation spaces. Provide passenger safety bill.

The Certificate of Inspection (COI) must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. The COI was not posted. Post certificate.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's stern light was inoperable at time of inspection. Vessel replaced the bulb and demonstrated operation the same day.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not install an interconnected fire detection system by 27DEC2022. Vessel is restricted to a LIMITED COASTWISE route and is prohibited from allowing passengers to utilize overnight accomodations until it's compliance with the interconnected fire detection requirements of 46 CFR 181.405 have been verified by a USCG marine inspector.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Starboard runing light was found inoperable, and stern light was found mounted incorrectly (rotated 90 degrees) and intermittently operable.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Hydrostatic Release Unit (HRU) was found with a June 2022 expiration date. Provide new HRU to ensure float free arrangement.

Oceans, coastwise, limited coastwise, and Great Lakes routes. A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry - (1) Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel was found with six orange and six red flares, however three of the red hand flares expired in July 2022. Provide adequate number of unexpired distress signals.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. One of the three required buckets was found on deck, broken and missing a lanyard. Provide 3 fire buckets in satisfactory condition.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel was missing one bucket and needs to re-stencil another.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI.

A vessel must comply with the applicable requirements for any radio and electronic position indicating radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel does not meet requirements for MF/SSB installation, as determined by attending FCC licensed inspector. FCC inspector issued FCC Safety Radio Telephony Cert with note stating it met requirements for a 20 nautical mile route. FCC exemption request filed. Vessel limited to 20 nautical miles from land until exemption granted or denied. Vessel restricted to the following route until MF/SSB carriage issue resolved: PACIFIC OCEAN, NOT ON AN INTERNATIONAL VOYAGE, BETWEEN THE SAN LUIS OBISPO / MONTEREY COUNTY LINE: 35–47.5 DEGREES NORTH LATITUDE AND 32 -15 DEGREES NORTH LATITUDE, NOT MORE THAN 20 MILES FROM THE MAINLAND SHORE. A VOYAGE FROM THE MAINLAND TO SANTA CRUZ ISLAND AND ANYWHERE WITHIN 2 MILES OF SANTA CRUZ ISLAND IS CONSIDERED TO BE WITHIN THE SCOPE OF THIS ROUTE.

A vessel must comply with the applicable requirements for any radio and electronic position indicating radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel failed to obtain an FCC Safety Radio Telephony Certificate. Vessel is restricted to 1000 feet from shore until a successful FCC safety radio telephony certificate inspection has been completed.

Make engine room bulkhead vapor tight,

Provide a valid COD.

Provide a valid FCC station license and a FCC safety certificate. Vessel is restricted to no more than a 1000' from shore.

Provide emergency/muster instructions.

Provide a waste management plan.

Repair anchor and forward mast head lights as well as provide day shapes.

Prove proper operation of bilge pump in bunk room.

Repair bow that was damaged from allsion with scorpion rock.

Provide additional 40 B:C fire extinguisher for machinery space.

Provide FCC radio station license for current owner.

Provide (06) hand and (06) smoke flares.

Provide current NOAA epirb registration.

Provide coast pilot, light list, tides and currents.

Demonstrate proper operation of VHF DSC radio: screen inoperable.

Implement drug and alcohol program under new owner / marine employer.

Replace damaged life ring buoy.

Replace smoke detector battery.

Provide emergency instructions.

Provide (03) 2.5 gallon fire buckets with lanyards.

Overnight passengers authorized for (11) eleven persons. Deficiency to be cleared upon COI amendment.

When passengers are below decks a roving watchman will be designated to patrol spaces. Deficiency to be cleared upon COI amendment.

Provide a plan to replace or demonstrate fire retardant properties of mattresses.

Prove proper operationd of the bilge high level alarm in the laz prior to carrying passengers.

Secure water filter bracket on MDE

Repair stern light.

Remove out-of-service battery cable in E/R.

Renew EPIRB registration

Install retaining device on E/R hatch cover.

Replace expired EPIRB hydro release

Provide proof of proper logging of drills.

Provide proper marking and stowage of adult and children lifejackets.

Provide proof of paper navigation charts for area of operation.

Provide Electronic or paper copies of navigational publications IAW 46 CFR 184.420.
