Port side longitudinal in the forward tank space is cracked. Repair cracked weld prior to drydock credit.

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. MI identified several stanchions with broken welds and/or wasted stanchions.

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. Bilge piping serving the Lazarette Compartment was found to be severely corroded and had a section where the piping was completely wasted away creating a 4 inch hole on the bottom of the pipe. 46 CFR 182.25-5(a)(Old T)

Each vessel must be marked as required by part 67, subpart I, of this chapter. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel does not have Hailing Port marked on the stern. 46 CFR 185.602(a) 46 CFR 67.123

Each survival craft required on a vessel must meet one of the following. For a lifefloat - approved under approval series 160.027. The color of the buoyant apparatus must be primarily vivid reddish orange as defined by sections 13 and 14 of FED-STD-595C. All lifefloats onboard faded and in need of repainting. 46 CFR 180.200(a)(2) 46 CFR 160.010-4(n)

The equipment required for lifefloats must meet the following specifications. The light must be a floating waterlight approved under approval series 161.010. Batteries in lifefloat waterlights were expired in 2019. 46 CFR 180.175(d)

Unless other means are provided to ensure that a fire would be limited to the receptacle, waste receptacles must be constructed of non-combustible materials with no openings in the sides or bottom. Waste receptacles found throughout vessel constructed of combustible material. 46 CFR 177.405 (f)

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew and other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electric shock. Power outlet above sink in galley found to be inoperable with live wires to outlet. 46 CFR 183.200(b)

Electrical equipment on a vessel must be installed and maintained to: provide services necessary for safety under normal and emergency conditions. Permanent extension cord installed for microwave in galley. 46 CFR 183.200

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. No operating instructions found near storage space for CO2 cylinders. 46 CFR 185.612 (a)

Each entrance to a space storing carbon dioxide cylinders must be conspicuously marked as follows: “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” No warning sign found at entrance to CO2 storage space. 46 CFR 185.612(g)(1)

The portable hand bilge pump shall be provided with suitable hoses for pumping bilges on the suction and discharge ends. Hand bilge pump does not have suitable hoses for suction and discharge overboard. 46 CFR 182.25-1(e)

The bilge suction shall be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Bilge suction pipe in lazarette was found to be missing a strainer. 46 CFR 182.25-5(b)

Provisions shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. 1.5" opening found on door between accommodation space and Engine Room. 46 CFR 181.20-35

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew and other persons, and the vessel from electrical hazards. Dead-end wire found in Engine Room. 46 CFR 183.200(b)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A shaft alley bilge. Shaft space bilge alarm did not alarm visually or audibly at operating station.

Provide bearing reading for port and stbd shaft. Will determine replacement based on readings.

Identified 03 holes in port and stbd stern tube. Repair to original condition.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel has damaged areas (13' from bow port side, 3' 7" above the waterline, 42" damaged steel hull-cut open) due to an allision with a cleat at the long dock. Vessel must conduct repairs to the satisfaction of a Marine Inspector before carriage of passengers.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel's EPIRB battery was expired.

Provisions shall be made by means of plugs, covers, etc. to prevent the admission of air into the space protected. There are no current known provisions made for closing the ventilation to the E/R, a CO2 protected space. Provide and prove proper operation of ventilation closures for the Engine Room. Vessel's route is restricted to Lakes, Bays, and Sounds only, until ventilation closures are provided. 46 CFR 181.20-35(a)

A vessel must comply w/the applicable requirements for any radio installations including the requirements for a station loc & install certs. Current DSC on VHF radio is inop & vessel FCC certs are expired. Vessel to provide valid FCC certs & operable VHF-DSC radio installation. Vessel limited to 1,000 feet from shore.

The lifeline shall be properly secured around the sides & ends of the life float or BA, festooned in bights not longer than 3 feet. Vessel to renew lifeline on all sides of each BA as indicated.

Provide a FCC type accepted CAT 1 406MHz EPIRB, installed to auto float free & activate & not be required to carry survival craft. Current EPIRB registration expires in 17 days. Vessel to provide renewal registration.

A hatch exposed to the weather must be watertight. Hatches for the following spaces are missing gaskets & are not watertight. Lazerrette, Shaft space, & sewage space. Vessel to install proper gaskets and make hatches weather tight.

Provide proper identification/labeling of bilge valves.

Reduce amount of flammable and combustible liguid in the engine room. Vessel is permitted a max of 5 gallons stored in no more than 1 gallon containers. Vessel had aprox 10 new 5 gal pails of oil stored in the engine room.

Vessel is currently overdue for require dry dock examination and must complete a satisfactory dry dock examination prior to carrying passengers for hire.

Provide required retroreflective tape on all lifejackets.

Provide log of monthly EPIRB testing.

Replace grabline on the STBD side ring buoy.

Replace fire pull station placard on topside deck.
