Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Vessel Master informed that vessel engine would not start in July and was inoperable for approximately 30 days. Owner/operator/master failed to notify the OCMI of condition and repair proposal.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Each fire drill must include:(1) Summoning passengers on a vessel on an overnight voyage to muster or embarkation stations;(2) Summoning the crew to report to assigned stations and to prepare for and demonstrate assigned duties; and (3) Instruction in the use and location of fire alarms, extinguishers, and any other fire fighting equipment on board. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. New hire deckhand, serving onboard since June 2025, was unfamiliar with emergency duties in event of fire and was unable to describe responsibilities. Demonstrate competence in presence of marine inspector.

Each Employee assistance program must include education and training on drug & alcohol policy. Provide a list/log of training, including who attended.

Each owner must ensure that a copy of the approved Vessel Security Plan (VSP) are kept on board. Provide the Coast Guard a copy of the VSP approval letter.

(b) Each abandon ship drill must include:(1) Summoning the crew to report to assigned stations and prepare for assigned duties;(2) Summoning passengers on a vessel on an overnight voyage to muster stations or embarkation stations and ensuring that they are made aware of how the order to abandon ship will be given;(3) Checking that life jackets are correctly donned;(4) Operation of any davits used for launching liferafts; and(5) Instruction on the automatic and manual deployment of survival craft. (c) Each abandon ship drill must, as far as practicable, be conducted as if there were an actual emergency. (f) Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel Master unable to produce record of drills conducted through 2024 operating season. Perform drills at prescribed intervals determined by owner/operator and submit each drill record to USCG Sector Detroit for review.

All (ventilation) ducts shall be of rigid permanent construction of fire proof material and reasonably gastight from end to end. Exhaust piping leaking gas from within segment inside sewage compartment. Repair leak.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Port and starboard generator windings found excessively oil soaked. Clean windings and verify soundness of insulation.

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electric shock. Junction box directly above main engine missing cover. Replace cover.

No marine employer shall engage or employ any person to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. List of P.R.-employment tested crew not available at inspection. Provide list of all crew who were drug tested and enrolled in the company's DAPI program.

Security drill and exercises. Drills shall be preformed every three months. Exercise shall be preformed every 18 months. Drills and exercise has not been preformed. Conduct drill and exercise and provide documentation.

Fire Service report not available. Provide proof of record of portable fire extinguishers service.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The vessels first aid kit contained expired items, renew/replace expired items.

Spanner wrenches must be provided for each fire hydrant required by this regulation.

Bilge suction shall be fitted with suitable strainers having an open area not less than 3x the area of the bilge pipe. Install strainer on bilge suction in sewage room

A robust worklist has be issued and can be found in documents and the narrative.

Upper deck capacity must be indicated by a durable marking in at least 1" lettering at entrance to that deck. Upper deck capacity incorrect.

Magnetic compass is missing deviation card.

Life ring buoy grab line found to be dry rotted and needs to be replaced.

Each vessel must have at least two operable portable battery lights, 1 at the operating station and 1 at the access to the engine room. Provide lights as required.

Mark both sides of emergency escape in at least 2" letters "EMERGENCY EXIT_- KEEP CLEAR". Found not labeled and with obstructions, clear and label.

Clean bilge suction in fuel tank space and operationally test bilge pump from this space.

Remove dead end wiring from port generator.

Secure mount for starboard generator.

WHERE FLEXIBLE NONMETALLIC HOSE IS PERMITTED FOR USE IN PIPING SYSTEMS, IT MUST MEET SAE J-1942 COMPLETE WITH FACTORY ASSEMBLED END FITTINGS COMPLYING WITH SAE J-1475. FLEX HOSE FUEL LINES MUST BE RENEWED.

An adult life jacket must be provided for each person carried on board, in addition, a number of child size lifejackets equal to at least 10% of the number of persons permitted, or such greater number as necessary to provide a life jacket for each person being carried. Several life jackets destroyed and removed from the vessel. Owner advised that 100% of life jackets would be inspected until a full renewal has taken place given the age of the life jackets.

Update VHF radios with DSC capability - see CG-CVC policy letter 15-06(CH2). New radio to be installed with DSC capabilities as soon as possible but not later than the next annual inspection. Please contact Marine Inspector upon installation.

The bilge suction shall be fitted with suitable strainers having an open area not less than 3x the area of the bilge pipe. Bilge strainer in forepeak was found to be severely corroded.

Ensure lifejackets are stowed in accordance with labels

Each ring lifebuoy on a vessel must be approved with subchapter Q of this chapter. Port and Starboard life rings were found to have surface cracking and deteriorated line.

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Electric solenoid valves must not be used unless used in addition to the manual valve. Shut off valve did not operate, potentially due to failure of 9V battery. Replace with manually operated valve.

On a vessel of at least 26' in length, a visual and audible alarm must be provided at each operating station. Bilge alarm for freezer space is not working.

Provide new, legible emergency broadcast placard

As appropriate for the voyage, a vessel must carry adequate and up to date charts. Charts and Navigation rules were not up to date.

Contact Sector Detroit to conduct drills and underway portion of exam to include operational test of bilge pump.

Renew retro-reflective tape on 26 child life jackets.

Provide new chart, light list, and coast pilot.

Vessel is not conducting security drills and not logging security training. Vessel shall conduct quarterly security drills and log. Log all future security training.

Placard for max passengers is labled wrong with 70. Change to 60 IAW stability letter.

Replace port side pressure relief valve on air flask.

Replace STBD pressure gauge on air flask.

Install gasket in hatch in fore-peak.

Install suitable bracket on fire extinguisher in pilot house.

Stern light was extinguished.

Port upper steering quadrant bolt too short. Replace with suitable sized bolt.

Reefer flat bilge high water alarm is inoperative. Repair or replace unit.

Vessels aft stern ring buoy removed from service due to un-serviceable condition. Renew aft ring bouy and affix proper marking and retro-reflective tape. 60 ft of line to be attached to aft ring bouy.

Waterlight is cracked in a number of citical areas. Renew as appropriate. Renew Water light line and ensure corrosion resistant clip is provided.

Fuel shutoff control valve shall be installed and accessible for closure from outside of the space.
