11117 - Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter. The grab lines on the ring buoy were deteriorated. 46 CFR 180.70(b)(1)

13104 - A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The visual indicators for the bilge pumps were inoperable. 46 CFR 182.530(c)

11132 - Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The hydrostatic release unit was expired. 46 CFR 185.740

11118 - The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The number of children's life jackets was not marked on the stowage container. 46 CFR 185.604(f)

11199 - A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. The first aid kit did not contain eye wash. 46 CFR 184.710

14502 - A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station, stating the following: Discharge of Oil ProhibitedThe Federal Water Pollution Control Act prohibits the discharge of oil or oily waste into or upon the navigable waters of the United States, or the waters of the contiguous zone, or which may affect natural resources belonging to, appertaining to, or under the exclusive management authority of the United States, if such discharge causes a film or discoloration of the surface of the water or causes a sludge or emulsion beneath the surface of the water. Violators are subject to substantial civil penalties and/or criminal sanctions including fines and imprisonment. The vessel's oil placard was missing. 33 CFR 155.450(a)

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. No FCC Documents were found on the vessel.

46 CFR 185.212 A marine employer whose vessel is likely to become a serious marine incident as defined by 4.03-2 of Subchapter A of this chapter shall comply with the requirements of 4.06 in Subchapter A of this Chapter. Vessel was missing alcohol testing equipment onboard.

46 CFR 185.728 Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. (c) The EPIRB test required shall be logged or otherwise documented as applicable. There was no record of lifesaving drills or testing of EPIRB onboard.

46 CFR 180.70(c) - A vessel must have one or more ring life buoys as follows: At least one ring life buoy must be fitted with a lifeline. Vessels ring life buoy did not have a lifeline nor was it properly stowed.

46 CFR 181.520 - Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the [name withheld] of vessels portable fire extinguishers was not properly mounted nor did it have the correct mounting bracket.

46 CFR 182.465(h) - Provisions must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Vessel was missing vent covers for the engine room ventilation openings.

46 CFR 182.455(b(6) - A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Vessels fuel filter was missing flame shield on fuel filter.

46 CFR 158.608 - Remote fuel shutoff stations must be marked in clearly legible letters at least 25mm (1 inch) high indicating purpose of the valve and direction of operation. Vessels fuel shutoff valves were not properly marked.

46 CFR 184.420(a(1) - As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel did not have any navigation charts on board.

46 CFR 184.702 - A vessel must comply with the applicable design, equipment, personnel, procedures, and record requirements of 33 CFR parts 151, 155, and 156. Vessel did not have oil pollution prevention placard or garbage handling placard posted as required.

46 CFR 180.71(a) An adult life jacket must be provided for each person carried on board. Vessel was short 4 adult life jackets and will be restricted to carrying 27 persons.

46 CFR 182.455(b)(6) A suitable marine type strainer must be fitted in the fuel supply line in the engine compartment. Vessel has non-marine type fuel filter installed onboard.

Fuel tank corroded and leaking diesel fuel into engine compartment. New tank plan submitted and being built.

An adult lifejacket must be provided for every person carried onboard. Vessel only had 30 operational lifejackets on board, COI was for 31 POB.

Life ring buoys must have vessel's named marked in block letters. Life buoy did not have name marked.

Vessel must have first aid kit approved under 46 CFR 160.041 or equivalent. Vessel's first aid kit was expired.

Portable FFE must be installed to the satisfaction of the [name withheld] in fwd compartment was not affixed to an installed bracket.

Bilge alarm was not mounted to vessel.

Railings or equal protection must be installed on decks accessible to passengers. Port side rails were not permanently affixed to deck.

Vessels must have nav lights IAW applicable sections of Intl and Inland Nav Rules. Vessel's Nav lights were inoperable and running lights/stern light were not affixed to vessel.

Each vessel carrying more than 6 passengers for hire must have an operator who holds a marine operator permit. Master did not have permit

A Cetificate of Documentation is required for vessels of greater than 5NT on trade business. Vessel's COD was expired.

Vessel required to provide for Bilge High water alarms. Vessels H/W alarm were inoperable.

Vessel must carry first-aid kit approved iaaw 46CFR160.041. Vessel first-aid was missing and had expired contents.

Vessel on Limited coastwise route is required to carry 12 flares. Vessel only had 06.

Each disposable hydrostatic release unit must be marked with an expiration date of 2 years after the date of installation. Vessel had expired hydrostatic release unit installed for EPIRB.
