The lifeline and pendants must be as furnished by the manufacturer with the approved life float or buoyant apparatus. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter: Each pendant must be made up in a hank, and the hank secured by not more than two turns of light twine. Pendants were not made up into hanks. 46 CFR 180.175(f)(1)

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant [name withheld] shackle securing anchor rode to vessel was wasted. 46 CFR 184.300

The fixed power bilge pump shall be self-priming and may be driven off the main engine or other source of power. MI observed a deteriorated belt for the engine driven bilge pump, operator replaced on the spot. 46 CFR 182.25-10 Old T

Each survival craft must be: secured to the vessel by a painter with a float-free link permanently attached to the vessel. Life floats float-free link was not connected, install to satisfaction of MI. 46 CFR 180.130 (a)(1)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Life floats had damage exposing the inner portion of the rafts, repair to MI satisfaction. 46 CFR 185.175(a)(4)

Filling and sounding pipes shall be arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. Sounding pipes may be omitted where sounding can be accomplished through the fill pipe or where marine type fuel gages are installed. Where sounding pipes are used, their openings shall be at least as high as the opening of the fill pipe and they shall be kept closed at all times except during sounding Vessel's filling and sounding piping was allowing overflow into the space. Replace the hoses to the satisfaction of the MI. 46 CFR 182.20-30(b) Old T

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Vessels guard on the aft belt of the engine was found broken and not properly covering the exposed belt. Repair the guard to the satisfaction of the MI. 46 CFR 117.35-15 OLD T

At least one ring life buoy must be fitted with a lifeline... Each lifeline on a ring must: be buoyant. MI observed forward life line was not of buoyant type. Replace line the line to satisfaction of the MI. 46 CFR 180.70 (c)(1)

Each bouyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The material must be in strips at least 55 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. For peripheral body apparatus, each strip must extend completely over the top and bottom surface of the body. For box type apparatus, the strip must extend at least 300 mm (12 in.) inboard from the edge over the top and bottom surface. Each strip must be positioned near the center of the side or end, but so that it is not obscured by any strap. Multiple sections of tape were found not properly installed as required. Replace the tape to the satisfaction of the MI. 46 CFR 160.010-4(p)

Each first-aid kit must meet the requirements in ISO 18813:2006(E) paragraph 4.12 (incorporated by reference, see § 199.05).(i) A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12, except that it does not contain the burn preparations. It must be clearly marked on the first-aid kit that it does not include the burn preparations.(ii) The active ingredients in medicinal products must conform to over-the-counter (OTC) drug regulations set out in 21 CFR part 330. Vessel had several items in their med kit, bring med kits up to ISO 18813:2006 (E) standard. 46 CFR 199.175(b)(10) 46 CFR 184.710

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; Tide tables. Vessel charts and tide tables were outdated, update to MI satisfaction. 46 CFR 184.420 (a) (1)/(4)

Each flare carried to meet this section must be stowed in one of the following: A portable watertight container marked as required by § 185.614 of this chapter, carried at the operating station. Flare box paint appeared faded, repaint to MI satisfaction. 46 CFR 180.68 (e)(1)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks; A crack was discovered in the port side sanded area within void space 4. Diagnose extent of the damage of the crack and repair to the satisfaction of the Marine Inspector. 46 CFR 176.405(a)(2)

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Vessel has a removable section of its bulkhead that has been found removed and allows vapors to escape to other areas of the vessel. Make the removable section permanent to the satisfaction of the Marine Inspector. 46CFR177.405(e)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Vessel weather deck surrounding the hatch into the lazarette was found to be delaminating allowing water into the space. Submit temporary repair proposal to the satisfaction of the MI with a permanent repair to be conducted at the vessels next drydock. 46 CFR 176.802(a)(1)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel did not have the fire buckets on board of the vessel during the inspection. Vessel must obtain the required fire buckets of this regulation to the satisfaction of the attending Marine Inspector. 46CFR181.610

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF–DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2), except that a VHF radiotelephone installation without DSC capability is permitted until one year after the Coast Guard notifies the Commission that shore-based sea area A1 coverage is established. Vessels in this category must not operate more than 20 nautical miles from land. Vessel's VHF is not properly programmed with DSC which includes the vessel's MMSI and GPS coordinates. Vessel must upload and integrate all three of these systems into their radios to the satisfaction to the attending Marine Inspector. 46CFR184.502 47CFR80.905(a)(1)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel's Radiotelephone certificate showed incorrect information and radios tested did not meet the requirements outline in this part. The vessel radio telephony certificate is deemed invalid, vessel must obtain a new radio telephony certificate. 46CFR184.502 47CFR80.903

Each floating water light installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the water light to be quickly disconnected from the ring life buoy. Vessel's water light was tied onto the life buoy. Vessel must install the water light to the satisfaction the the attending Marine Inspector. 46CFR180.70(d)(3)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel has multiple publications listed that were out of date. Vessel must obtain up to date publications to the satisfaction of the Marine Inspector. 46CFR1184.420(a)

The intent of this part is to insure that the scantlings, material, fastenings, workmanship and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection. Vessel reported damage to hull after allision with dock. Make necessary repairs to the hull and submit all repair proposals to the satisfaction of the Coast Guard marine inspector. 46 CFR 177.01-05(a) “OLD T”

Watertight integrity above the margin line in a vessel less than 100 gross tons.(a) Each hatch exposed to the weather must be watertight, except that the following hatches may be weather tight: (1) Each hatch on a watertight trunk that extends at least 12 inches (30.5 cm) above the weather deck. (2) Each hatch in a cabin top. (3) Each hatch on a vessel that operates only on protected waters. (b) Each hatch cover must- (1) Have securing devices; and be attached to the hatch frame or coaming by hinges, captive chains, or other devices to prevent its loss.(c) Each hatch that provides access to crew or passenger accommodations must be operable from either side. Access cover leading into the space aft of collision bulkhead is not watertight. 46CFR121.124 (OLD T, OLD S)

The breaking strength of each (float-free) link must be between: (1)450 N (100 lbs) and 600 N (134 lbs) for links intended for life floats and buoyant apparatus of 10 persons and less capacity.(2) 900 N (200 lbs) and 1200 N (268 lbs) for links intended for life floats and buoyant apparatus of 11 to 20 persons capacity. Vessel's weak link leading to 6 person life float is not compatible with current installation. In addition, 22 person life float's weak link is missing required identifying information.

"All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections." 46 CFR 183.01-15(a). Bundle electrical installations in bridge.

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. The vessel's main propulsion engine exhaust lagging is torn. Repair the exhaust lagging to the satisfaction of the Marine Inspector. 46 CFR 177.10-5

There are multiple bulkhead penetrations in the forward bulkhead of the Engine Room that are not vapor tight. Seal the bulkhead penetrations to restore the vapor tight boundaries of the Engine Room compartment. 46 CFR 177.10-5

At each inspection, the marine inspector shall examine all weather tight closures above the weather deck. The gasket for the watertight hatch on the No. 2 compartment is damaged. Repair the gasket to restore the watertight integrity of the main deck. 46 CFR 176.25-5 (5)

Crop and renew rotten port underdeck beam and deck on forward most main deck hatch at next credit dry dock in 2021.

Replace the fuel line support (currently zip lines) with metallic/fire resistant material.

Replace the wing nut connections on each battery with standard nut connections.

Replace fwd battery broken connection (port fwd, for the anchor windlass).

Replace deteriorated life ring.

Install visual bilge alarms on the bridge.

Install machinery guard to cover the alternator belts on the main engine.

Identified crack in the vertical corner structure of the laz, port aft side - 3 planks of wood each approx 1/2" comprise the corner post, only the outer post appears to be cracked and also has an approx 1" spot of rot in the same vicinity. Prep the area for inspection prior to the upcoming drydock in December and be prepared for probable replacement of structural member. Ref: NVIC 7-95

Provide screw down valve for the forpeake bilge suction that is operable from the weather deck IAW 46 CFR 182.510(d)

Vessel's anchor light inoperable. Make proper repairs to the anchor light.

Renew primary lifesaving equipment for a minimum of 25 persons, 50%. If changing equipment from lifefloats, provide equipment information and arrangement for approval.

Installed weak link on lifefloats is for 11-20 persons. Replace with proper weak link for 28 person capacity.

All waterlight batteries are expired, renew with proper new batteries.

Provide proof of obtaining 1 adult PFD to meet COI requirements.

Provide current list of crewmembers enrolled in drug and alcohol program

Installed lifering damaged from exposure, renew with new.

Provide current and corrected charts and publications for area of transit.

Bilge suction in the forepeak and #4 void inoperable. Make proper repairs and prove proper operation.

Renew the attachements for the identified deck hatches.

Vessel had a loss of propulsion with passengers onboard. Identify and correct cause of the engine failure. Prove proper operation of the MDE to the satisfaction of the attending marine inspector.
