A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit contained aspirin expired January 2025.

An adult life jacket must be provided for each person carried on board a vessel. Inspector observed 02 life jackets not serviceable.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Current Certificate of Documentation not on board.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title IIIof the Communications Act must carry a radio operator. Operator is currently not holding Radiotelegraph Operator's Certificate. Vessel is restricted to 1,000 feet from land until rectified.

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Lifeline not serviceable.

The Rules concerning lights (§§ 83.20 through 83.31) shall be complied with from sunset to sunrise, and during such times no other lights shall be exhibited, except such lights as cannot be mistaken for the lights specified in these Rules or do not impair their visibility or distinctive character, or interfere with the keeping of a proper lookout. Port and Stbd running lights not operating.

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in 4.03-2 of subchapter A of this chapter shall comply with the requirements of 4.06 in subchapter A of this chapter. The marine employer must have a sufficent number of alcohol testing devices readily accessible on board the bessel to determine the presence of alcohol in the system of each individual who directly involved in the SMI. Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: (1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. 47 CFR 80.159

Gages to indicate cooling water discharge tempertaure and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Vessel did not have any gauges at the operating station. 46 CFR 182.15-5(b) (Old T - keel laid prior to March 11, 1996)

Abandon ship, man overboard, and fire drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel did not log abandon ship, man overboard, or fire drills.

Each lifejacket must be marked in clearly legible block capital letters with the vessel's name. Vessel did not have accurately marked life jackets with the vessel's new name.

Employers must ensure that they and their crewmembers meet the requirements of this part. Vessel does not have proof of enrollment in a maritime consortium for this calendar year. Vessel to provide prood of enrollment for all employees operating the vessel.

Every vessel subject to Part III of Ttile III must have a detailed inspection of the radio installation by an FCC licensed technician every five years. Vessel's Radiotelephony Certificate is expired. Vessel to provide a valid certificate. 47 CFR 80.903

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel exceeded two year interval on 01APR2021. Vessel required to undergo satisfactory drydock examination prior to departure or carrying passengers.

46 CFR 176.404: An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. Due to COVID-19 circumstances beyond the control of the vessel, the vessel is unable to meet the timeframe required by 46 CFR 176.107. Vessel is allowed 90 days to continue operations in accordance with the COI, and is required to make itself available for inspection prior to 24JUL2020. Issued CG-835v code 705c to extend COI 90 days. Due to circumstances created by the COVID-19 outbreak, Sector LA-LB has issued an additional 90 day extension for the COI Renewal (5 Year) Inspection of the small passenger vessel LOOKING GLASS. The Annual Inspection must be completed no later than October 13, 2020.

A vessel to which 46 CFR Subchapter T applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. The vessel failed to have the Certificate of Inspection (COI) renewed within the required time frame. Therefore, the vessel does not hold a valid COI and is prohibited from the carriage of passengers. A inspection for issuance of COI must be completed prior to the vessel returning to commercial service. 46 CFR 176.100(a)
