Closures for hull openings at or below the freeboard deck must be as strong as the structure to which they are attached and must be watertight. The forward most dog on the chain locker hatch is damaged and requires adjustment. Prove proper operation of hatch dog, due 14May26.

CO2 piping must be supported to prevent damage, provide adequate support for the fixed CO2 time delay. Due 14May26.

The two aft most CO2 bottle control heads were loose, provide service report detailing proper installation of control heads. Due prior to commercial operations.

Periodic safety tests must demonstrate the proper operation of the safety controls; high fuel oil alarm was inoperable. 46 CFR 61.40-6(a) 46 CFR 62.35-40

The master of a vessel fitted with loading doors shall assure that all loading doors are closed watertight and secured during the entire voyage; port and starboard loading doors were not closed. 46 CFR 78.17-33

The master shall enter into the log book the time and door location of every closing of the loading doors; closing of doors was not being logged. 46 CFR 78.17-33(c)

The following shall be logged in the official logbook: (a) Onboard training, musters, and drills: held in accordance with subchapter W; (b) Watertight door operation: Daily and Weekly. (c) Valve and closing appliance operation: Weekly. (d) Loudspeaker system: Weekly. (e) Steering gear, whistle, and means of communication: Prior to departure. (f) Drafts and load line markings: Prior to leaving port (g) Verification of vessel compliance with applicable stability requirements. After loading and prior to departure and at all other times necessary to assure the safety of the vessel. (h) Loading doors. Where applicable, every closing and any opening when not docked. (i) Hatches and other openings: All openings and closings, or leaving port without closing, except vessels on protected waters. (j) Emergency lighting and power systems: Weekly and semiannually. (k) Smoke Detecting System: Once every three months. (l) Fuel oil data: Upon receipt of fuel oil on board. (m) Cargo gear inspections: At least once a month. Not all items were were being logged appropriately; to be verified by MI at next topside prior to clearing. 46 CFR 78.37-5

The master of a vessel fitted with loading doors shall assure that all loading doors are closed watertight; stbd loading door gasket prevented watertight sealing, replace gasket material. 46 CFR 78.17-33

Marine employer must have sufficient number of alcohol testing devices readily available on board; all alcohol testing strips were expired. 46 CFR 4.06-15(a)(1)

Each rescue boat must carry the equipment listed in table 199.175; first aid kit on rescue boat was expired. 46 CFR 199.175(b)

Illustrations and instructions for donning lifejackets must be displayed at each muster station; donning instructions not posted for all types of lifejackets. 46 CFR 199.80

Registers issued with cargo gear certification must include certification of testing of cranes; crane was not load tested, submit plan for correction. 46 CFR 71.25-25(d)(3)

Light and full load tests shall be carried out during each inspection for renewal of certification; could not conduct required load tests on davit. 46 CFR 199.45(b)(1)

Internal combustion driven e-gen shall be operated under load at least 2 hours monthly; e-gen was not being run 2 hours monthly. To be cleared at next re-inspection. 46 CFR 78.17-45(b)

Each side of each rescue boat bow must be marked in block capital letters and numbers with the name of the vessel and the name of the port on the stem of the vessel; markings missing on rescue boat. 46 CFR 199.176(a)

Periodic safety tests must demonstrate proper operation of safety controls/alarms; low lube oil pressure was not operating on port MDE. 46 CFR 61.40-6

Each panelboard must have a decription of the load of each circuit; sampling of distribution panels showed circuit loads were not marked. To be done during winter layup. 46 CFR 111.40-11

Each Passenger vessel must carry a combination of lifeboats and liferafts that have an aggregate capacity sufficient to accommodate the total number of persons onboard. Vessel was missing one of their liferafts because of improper packing of the raft. Total person onboard the vessel reduced from 137 to 100 until the missing liferaft is properly installed. 46CFR77.06 46CFR199.201

Non-conducting deck coverings, such as non-conducting mats or gratings, suitable for the specific switchboard voltage must be installed for personnel protection at the front and rear of the switchboard and must extend the entire length of, and be of sufficient width to suit, the operating space. The mat in front of the main generator switchboard is too small for the space. 46CFR70.25-1 46CFR111.30-11

All portable fire extinguishers and semi-portable fire extinguishing systems must be maintained in accordance with NFPA 10. The Semi-portable extinguisher in the engine room has a service tag from 2023 (over a year old), where all the other extinguishers onboard had a service tag from 2024. 71.25-20(a)(1)

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. The temperature gauge on the port main engine was broken and needs to be replaced. 46CFR70.20-1

Threaded connections are not considered an acceptable method of connection outboard of the shell valves. Several shell valves on the port side of the vessel were connected via threaded connections. Vessel must replace all shell valves connected via threaded connections with a welded or flanged connection at next drydock. 46CFR70.20-1 46CFR58.01-5 ABS Rules 4.6.2.9.13.3

Deck valve on the Valve on #2 fire station is broken and needs to be replaced. 46CFR70.20-1

Lifeboats must have navigation lights that are in compliance with Navigation Rules. The mast light on the rescue boat is not working. 46CFR77.06 46CFR199.202(a) 46CFR160.156-7(b)(27)

Due to hiring challenges, Master requests to temporarily sail without the COI required oiler position, with an additional OS/Wiper onboard. Vessel is authorized to temporarily sail without an oiler for 30 days with the following contingencies: 1) additional OS/Wiper is aboard and receives documented training to fill oiler Station Bill responsibilities, 2)work/rest hours are followed in accordance with 46 CFR 15.1111, 3) continue to make reasonable effort to find adequate crew replacement.

Conduct MOB drill with satistfactory launching and recovery of rescue boat within time specified.

Vessel must have fire boundaries insulated with the appropriate structural insulation. Vessel had areas of missing and damaged A-60 structural insulation in the steering flat above the space heater, above the port generator, and above the port Main Diesel Engine. Vessel must make repairs in kind to ensure the space meets the A-60 fire boundary protection rating.

Vessel must support cables with metal clamps. Vessel had multiple areas throughout the engine room where plastic cable ties were used to support cables. Vessel must replace plastic cable ties that are used to support cables with metal clamps.

Vessel's PSTPs for the Gen-sets were not accurate. Vessel tried to demonstrate a Low Lube Oil Pressure Warning Alarm and Shutdown. The PSTP listed both alarm and shutdown would occur at 15 PSI. The starboard gen-set experienced an alarm at 30 PSI and shutdown at 15 PSI. The port gen-set experienced an alarm and shutdown at 25 PSI. Vessel will troubleshoot to either ensure PSTP is accurate and demonstrate accuracy, or submit their PSTP for amendments to reflect how the gen-sets should function and demonstrate new PSTP accuracy.

SM01: Each lifesaving appliance, including lifeboat equipment, must be inspected monthly using the checklists required under paragraph (b)(1)(i) of this section to make sure the appliance and the equipment are complete and in good working order. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel's official logbook. Monthly EPIRB testing is not logged. Monthly rescue boat inspections are not logged. Conduct monthly testing of EPIRB and inspection of rescue boat and document in official logbook to the satisfaction of the marine inspector.

SM02: Weekly inspections and tests: Each survival craft, rescue boat, and launching appliance must be visually inspected to ensure its readiness for use. Each lifeboat engine and rescue boat engine must be run in accordance with 46 CFR 199.190 (d). Vessel crew does not conduct weekly inspections and engine test of rescue boat. Conduct weekly test and inspection of rescue boat and log to satisfaction of marine inspector.

SM03: All portable fire extinguishers must be maintained in accordance with NFPA 10, chapter 7. No evidence provided to indicate the fire extinguisher on Rescue Boat is serviced in accordance with NFPA. Ensure annual and monthly inspection is conducted to satisfaction of marine inspector.

SM04: As appropriate for the intended voyage vessel must carry adequate and up-to-date publications listed in 46 CFR 78.05-5. Vessel unable to produce electronic publications due to software issue. Provide adequate and up to date pubs as required by regulations to satisfaction of marine inspector.

SM05: Vessel is required to have FCC station license, bridge to bridge certificate, and Great Lakes Agreement Inspection. Vessel was unable to produce documentation to indicate aforementioned inspections were completed.47 CFR 80.1005; 47 CFR 80.13; 47 CFR 80.953

Identify and correct the source of the grounds indicated on the emergency generator distribution panel.

Boiler safety relief valves (SRV) must operate at or below the set point. The boiler SRV was set at 30psi and operated at 37psi. The boiler safety relief valve must be renewed.

The air damper for the auxiliary boiler is missing. The damper also serves as a guard for the blower fan. Reinstall the air damper.

The fuel oil day tank high and low alarms failed to function when tested. Prove proper operation of alarm functions.

The trouble alarm for zone 3 of the fire detection system failed to operate when tested. Prove proper operation of fire detection panel trouble alarms.

The breaker label for the Hyde Guardian system is missing. Label breaker as existing breaker nameplates.

The PSTP procedures to test the SSDG high water temp sensors failed to prove proper operation of the sensors. Re-develop procedures to prove operation of the sensors.

The SSDG's are not capable of being operated in parallel operation due to compatibility issues between the automatic voltage regulators and governors. Correct the compatibility issues with the AVRs and governors and prove proper parallel operation of SSDG's.

The boiler high temperature cut-out failed to produce an audible alarm as indicated in PSTP Section 4.2.

The boiler remote emergency shut down failed to produce an audible alarm as indicated in PSTP Section 4.5. Prove proper operation of audible alarm as indicated in the PSTP's.

The man overboard drill was not conducted due to the vessel being moored port side to pier. Conduct MOB drill within 30 days.

The valve for Fire Hose Station #7 located in the main level passageway is leaking. Service or replace leaking valve and demonstrate proper operation.

Replace missing nut on bow cargo hold hatch on main deck.

Install missing structural insulation in cargo hold overhead at passenger viewing area (below bow fire station).

Tighten loose battery terminals in EDG space.

EPIRB Battery expired.

Rudder assembly packing was leaking excessively. Repair to satisfaction of attending marine inspector.

Hose nozzle on starboard pax deck hose was leaking excessively while bonnet was closed. Repair or renew to original to satisfaction of marine inspector.

The fixed CO2 system outer pilot bottle cylinder pull mechanism is misaligned in respect to the pull cable pipe that connects the two pilot bottle pull mechanisms (due to improper length of the cable pipe). Resize pipe to proper length so as to provide for proper alignment, and operation, of all system components.

Vessel must prove proper operation of CPP controls to the satisfaction of attending CG Marine Inspector.

On July 6, 2018, vessel suffered casualty to CPP system while underway. Vessel shall submit technical report attesting to cause of casualty, repair, and full functionality of the CPP system, prior to carrying passengers, to the satisfaction of the attending marine inspector.

Verify accuracy of FCP and get approval from local OCMI. 46CFR71.65-1

Launch and recover rescue boat and perform MOB drill. 46 CFR 199.180(d)(7)

Pt & stbd rescue boats are missing survival craft equipment, including searchlight & fire extinguisher. Outfit rescue boats with required equipment.

Pt & stbd resuce boats are missing retro-reflective material. Install Type II retro IAW IMO arrangement.

EPIRB battery is expired. Install replacement and conduct test. To satisfaction of attending MI.

Bridge ring lifebuoy waterlight lanyards (port & stbd) are degraded. Replace lanyards with buoyant line resistant to sunlight deterioration.

Immersion suits stowed in crew accomadation spaces ladderwell are not marked with accurate number number. Replace missing suits or correct markings.

Log of lifesaving equipment inspections and maintenance is missing. Maintain log of all maintenance and inspections/tests for livesaving equipment.

No alcohol testing devices are onboard and vessel is unable to meet 2 hour test requirement for SMI. Provide a sufficient number of test devices on board.

Identify and properly label each light on the emergency power circuit.

Update emergency muster station notices in staterooms to reflect use of life rafts as opposed to life boats. Also, ensure proper muster location is identified.

Adjust door to engine room and cargo deck to ensure self closing to latched position.

Evaluate required crew for emergency situations and correct WQSB to reflect number of crew required on COI. Every aspect of emergency drills should be addressed by required crew on COI.

Use of an Oil Record Book is mandatory.

Remove dead end wires. Replace questionable wiring with proper guage boat cable. Remove all alligator clips and make all battery connections permanent.

Repair fire detection and alarm panel to provide both audible and visual indicators at operating station.

Remove incorrect splice and wire nuts from power supply to feedwater pump between AUX Boiler and water heater. Make proper connections and enclose in a junction box.

Machinery space is protected by a fixed CO2 system which is manually pulled from the passageway adjacent to the machinery space. Current configuration has the MDEs shutting down once the CO2 system is activated. However, the two SSGs which share the space continue running. These also need to shut down with the CO2 system activation.

Current configuration is for an independent pull station for mechanical ventilation shut down in the machinery space. This should be combined with the CO2 activation so that a single pull shuts down ventilation and stops the MDEs and SSGs.

A Coast Guard approved rescue boat and coast guard approved launching appliance shall be installed and all required plan approval and testing completed prior to the vessel initiating operations in the 2017 shipping season.

Provide evidence of crew participation in drug and alcohol testing.

Provide current approval letter for non-tank vessel response plan.

Execute MOB/Rescue Boat drills by launching and recovering boat at least once every 3 months wearing anti-exposure suits as required in cold water.

Operator not permitted to carry passengers until COI exam is complete and temporary certificate issued.
