A fire hose with a nozzle must be attached to each fire hydrant at all times. Aft fire station hose was missing the nozzle.

An adult life jacket must be provided for each person carried on board a vessel. 143 adult life jackets were onboard and not the required 156. Vessel is restricted to 143 total persons onboard until all required life jackets are place onboard the vessel. 46 CFR 180.71

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Port bridge wing ring life buoy needs replaced due to cracking, upper deck aft ring life buoy needs retro reflective tape replaced and main deck aft ring life buoy needs to be cleaned and properly marked with the vessels name. 46 CFR 176.808(b)

Each first-aid kit must meet the requirements in ISO 18813:2006(E) paragraph 4.12. First aid kit items with expiration dates were not properly marked on the outside of the container.

Bilge high level alarms.(a) On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. High level audible alarm for the STBD engine room bilge is not working at the operating station. 46 CFR 182.530

A vessel of 20 meters or more in length shall be provided with a bell in addition to a whistle. Vessel's sound signaling device is not operational. 33 CFR 83.33

Auxiliary machinery shall be suitable in type and design for the purpose intended. In the STBD generator space, MI discovered degradation in the exhaust hose that leads from the generator to the muffler and from the muffler to outside. 46 CFR 182.10-1

Diesel engine exhaust cooling. (a) Installation shall be in accordance with 182.15-15. STBD aft engine missing lagging. 46 CFR 182.20-15

Materials and workmanship. The materials and construction of fuel lines, including pipe, tube, and hose,must comply with the requirements of this paragraph. STBD main engine fuel return hose is showing signs of deterioration. 47 CFR 80.159

All piping, valves and fittings shall be in accordance with good marine practice and suitable for the purpose intended. MI observed leaking discharge line on fire pump. Suction for fire pump has a short bend/ kink. 46 CFR 181.15(c)

Vessels shall be inspected for compliance with the standards required by this subchapter. For items not covered by standards in this subchapter, inspections shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. Electrical wiring for the STBD side fuel tank fuel sensor needs to be secured & Port side jet space steering signaling electrical box needs to be covered. 46 CFR 176.25-1

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements:(1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Master did could not locate his Marine Radio Operator Permit. 46 CFR 80.159

(f) Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. PSCO found zero drills logged for the month of July and August. Drill logs to be inspected at next annual. 46 CFR 185.520

The construction and arrangement of a vessel must allow the safe operation of the vessel IAW the terms of its COI...Operations manager reported a collision between the ST. GREGORY and ST. ACTINEA. ST. GREGORY sustained damage to her port quarter. Complete ISE and repairs to the satisfaction of the CG prior to carrying passengers.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Port aft accomodation door latch mechanism did not function properly causing the door to remain open and swing. 46 CFR 176.800

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight. Port and starboard jet room hatch gaskets were found missing or damaged. 46 CFR 179.360

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Port fuel tank void alarm would not reset after being tested and the starboard engine room high water audible alarm was not functioning. 46 CFR 182.530

Clean and descale bottom plate IVO generator sea chest valve through hull for reinspection.

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: A hatch on a watertight trunk that extends at least 305 millimeters (12 inches) above the weather deck. The port side engine room hatch was missing the gasket. 46 CFR 179.360(a)(1)

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. The starboard forward emergency escape door latch sticks and does not operate properly. 46 CFR 177.500(h)

Each small battery installation must be located in a well ventilated space and protected from falling objects. Battery installations in engine rooms were not protected from falling objects. 46 CFR 183.354(b)

AT EACH INSPECTION FOR CERTIFICATION AND AT EVERY OTHER VESSEL INSPECTION ALL OBSERVED UNSAFE PRACTICES, FIRE HAZARDS, AND OTHER HAZARDOUS SITUATIONS MUST BE CORRECTED AND ALL REQUIRED GUARDS AND PROTECTIVE DEVICES MUST BE IN SATISFACTORY CONDITION. DURING "FOLLOW ON" INSPECTION ON THIS TIER 1 VESSEL, A MAJOR FUEL LEAK WAS OBSERVED ON THE #3 MDE. FUEL WAS SPRAYING OUT OF THE TOP OF THE FUEL INJECTION PUMP AND ATOMIZING THROUGHOUT THE ENGINE ROOM. DIESEL FUEL WAS OBSERVED THROUGHOUT THE OUTBOARD SIDE OF THE #3 MDE AND THROUGHOUT THE BILGE. THE ADJACENT INSULATION AND #4 MDE AIR INTAKE FILTER WAS SOAKED IN FUEL OIL. IDENTIFY AND MAKE REPAIRS TO THE FUEL PUMP, REMOVE OIL SOAKED INSULATION AND REMOVE EXCESSIVE FUEL THROUGHOUT THE ENGINE ROOM AND BILGE TO THE SATISFACTION OF A COAST GUARD MARINE INSPECTOR. 46 CFR 182.100

SUITABLE COVERS, GUARDS, OR RAILS SHALL BE INSTALLED IN WAY OF ALL EXPOSED AND HAZARDOUS PLACES SUCH AS GEARS, MACHINERY, ETC. #1 MDE WAS MISSING THE GUARD FOR THE FLYWHEEL. 46 CFR 177.35-15 (OLD T)

BATTERIES SHALL BE EFFECTIVELY SCREENED BY A CAGE OR SIMILAR STRUCTURE TO MINIMIZE THE DANGER OF ACCIDENTAL SPARK THROUGH DROPPING A METAL OBJECT ACROSS TERMINALS. BATTERIES COVERS WERE LOCATED AT THE TOP OF THE ENGINE ROOM HATCH AND NOT ON THE BATTERIES. REPLACE MISSING COVERES FOR ALL BATTERIES. 46 CFR 183.05-20 (OLD T)

EACH RING LIFE BUOY MUST BE MARKED WITH TYPE II RETROREFLECTIVE MATERIAL APPROVED IN ACCORDANCE WITH 46 CFR 164.018. AFT UPPER DECK RING BUOY WAS MISSING ALL RETROREFLECTIVE MATERIAL. REPLACE MISSING RETROREFLECTIVE MATERIAL. 46 CFR 185.604(h)

AT LEAST ONE RING LIFE BUOY MUST BE FITTED WITH A LIFELINE. AFT MAIN DECK RING LIFE BUOY WAS NOT FITTED WITH A LIFELINE. THE LIFELINE WAS HANGING NEXT TO THE RING LIFE BUOY. CREWMEMBER FITTED THE LIFELINE TO THE BUOY. 46 CFR 180.70(c)

EACH SURVIVAL CRAFT MUST BE SECURED TO THE VESSEL BY A PAINTER WITH A FLOAT-FREE LINK PERMANENTLY ATTACHED TO THE VESSEL. THE UPPER DECK 6 MAN IBA SCREW PIN FOR THE SHACKLE, ATTACHING THE PAINTER TO THE HYDROSTATIC RELEASE WAS COMPLETELY BACK OUT AND NOT SECURELY FASTENED. MASTER REATTACHED THE SCREWPIN FOR THE SHACKLE. 46 CFR 180.130

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Locate minor fuel leak and make repairs on the #1MDE and conduct sea trials in the presence of a Coast Guard Marine Inspector. 46 CFR 182.100

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. O/O applied for a new COD in March. The application was submitted with 1 digit incorrectly listed for the Official Number. NVDC did not inform the O/O of the error until the O/O inquired about the status of the new COD. A new application for COD was submitted to NVDC with the correct official number listed. Provide copy of new COD once received. 46 CFR 67.313

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Replace the missing cover on the port side AC condenser in the fuel tank space that has exposed wires.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Fore and Aft generator covers are being held on with 1 screw and duct tape. Replace the missing hardware for the generator covers.

A vessel must have one or more ring life buoys as follows: Be approved in accordance with § 160.050 in subchapter Q of this chapter. Replace discolored ring life buoy on the aft main deck.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. The PA system is inoperable from the bridge or at fixed installation location. Repair and conduct satisfactory test. 46 CFR 184.610(b) and (c)

Port aft engine had a radiator cap seal leak. Repair to original condition.

The Vessel's captain and crew were not familiar with the vessel's fire suppression system on board (or lack thereof). The crew has been logging that they check the FM200 fire system daily, but none is installed on this "Old T" exempt vessel.

The port forward engine had a note on top of it indicating that it is out of service. The vessel captain knew about it but didn't know how long it had been out of service. The coast guard was never notified. Passenger service continued without properly documenting the engine failure and gaining approval to continue operations.

Port side aft main engine oil supply line to turbo was completely loose causing oil to flow out all over the engine and turbo. Engine was immediately shut down to prevent further damage.

Oil leak found on starboard forward engine's turbocharger.

Exhaust leak found on starboard fwd engine.

Prove proper operation of Port FWD. engine prior to operating this engine with passengers onboard.

Secure bridge batteries to protect them from shifting

Repair split open drain pipe from cabin top to 2nd Deck to prevent crew and passengers from being cut.

Vessel must conduct and log drills every month if reasonable , but at least once within a 3 month period before getting underway with passengers

Passenger count is reduced by 6 until 6 man IBA is reinstalled

Repair stairwell hand rail on the port side heading to the second deck

Properly secure and repair or remove A/C compressor and tank in the #2 starboard void.

Properly install safety seal on portable fire extinguisher as per NFPA 10

Passenger count is reduced by 54 until all lifejackets are repaired or replaced

Reinstall fire/smoke block on wire run from port #2 void to main cabin

Repair leak on Fwd sewage pump valve

Repair all holes in aft sewage tank prior to using the tank.

Properly install lagging on the aft inboard engine in the port engine room.

Install starboard IBA so as to allow for floating free

Replace or Hydro test to prove satisfactory starboard engine room entrance fire extinguisher

Repair diesel fuel piping in #3 Void port and starboard to eliminate weeping at the flange

Replace non-metallic through-hull fitting on both aft main engines raw water discharge line with a suitable metallic fitting

Crop and renew hull plating in #2 void port side in the area from the front of the sewage tank to the first frame fwd. on both sides of the keel and remove water from under sewage tank to allow gauging of pits under the tank

Crop and renew hull plating in both the starboard and port jet rooms on the inboard side shell to the hull to the inboard jet and any area in the cross tunnel plating

Crop and renew wasted plating in both the port and starboard engine room fan trunk.

Crop and renew deck plating around the port engine room and jet room hatches.

Repair hole in port engine room fwd bulkhead center side line.

Crop and renew hull plating in port engine room from the fwd bulkhead from center line of keel outboard four frames and two frames aft and inboard from center line out four frames aft to the front of the aft engine.

Proper attach aft port side deck light

Fix port side fiddle light cover.

The vessel must maintain an up-to-date stability letter to account for changes in the weight and LCG. Following a main engine removal, the configuration of the vessel is not reflected in the current stability analysis. Vessel is only provisionally authorized to operate in this condition.

Vessel is allowed to temporary operate with portable fire pump installed. Vessel shall train and document all crew members on the operation of portable fire pump prior to being crew members on-board the vessel. All repairs to the fire pump prime mover shall be to the satisfaction of the CG Marine Inspector attending.

Properly install all IBA's onboard in a float free arrangement.

Properly maintain, calibrate, & train personnel on use of onboard alcohol test equipment or have approved test strips onboard the vessel.

Properly secure bilge piping in the stbd jet room. Bilge suction piping is outside of the support bracket allowing it to move and potentially crack the piping.

Repair or replace defective starter on the No. 4 MDE. Prior to operating with passengers since this engine drives the vessels only fire pump.

V & weld minor crack in aft exterior deck on stbd side just aft of main deck entrance.

Clad weld minor 1/4" hole at top of aft lower stbd stairs above the jet room.

Ensure that fire detection system is electronically supervised.

REPAIR SECOND RADAR ON BRIDGE.

PROVIDE PROOF OF RANDOM DRUG TESTING PROGRAM, INCLUDING COPY OF CONTRACT WITH CONSORTIUM, PROOF OF ENROLLMENT FOR ALL CREW, AND A COPY OF ANNUAL MISLETTER.

PROVIDE PERMANENT SEAL FOR PORT AND STBD LAZ THROUGH-HULL PENETRATIONS WHERE PREVIOUS MSD VALVE/PIPING WAS INSTALLED.

Demonstrate that jet drive shaft penetrations in the aft port and starboardengine room bulkheads are watertight. Plans shall be submitted to the CoastGuard prior to making any modifications to the existing jet drive bulkhead penetrations.

Provde sound signaling device (bell) as per navigation rules in 33 CFR 86.

Provide trash receptacles constructed of non-combustible materials as per 46 CFR 177.30-7.
