At each inspection for certification, periodic inspection and at other times necessary, the inspector will determine that all fire-extinguishing equipment is in suitable condition and may require any tests necessary to determine the condition of the equipment. Vessel reported a 2" crack in the fire main piping in aft section of starboard walk tunnel. Crew placed temporary fiberglass patch on piping. Vessel is to make permanent repairs upon arrival to Duluth, MN OOA 08Jul2026 to the satisfaction of the attending Marine Inspector.

No vessel may be navigated unless it has in its service and onboard the crew complement required by the COI; vessel does not currently meet the minimum manning specified on the COI. Vessel is currently short (01) AB from COI requirement. Vessel is permitted to transit from Duluth-Superior Harbor to Marquette, MI to bring on board replacement AB. 46 CFR 15.515(a)

Except as provided by 46CFR15.725 of this part, no vessel may be navigated unless it has in its service and on board the crew compliment requied by the COI. Vessel sent a request to the MSU Toledo OCMI to sail short 01 First Engineer. The vessel will bring on a Second Engineer to fill the role of First Engineer. The request is temporary and the vessel is authorized to sail with this replacement until 01JAN2026. Prior to the expiration of this deficiency the vessel/company shall provide proof that they are operating in accordance with the COI.

08JUN2025: Vessel ran aground while transiting the East Neebish Channel in the St Marys River. Vessel is actively taking on water via the #7S ballast tank and aft heavy fuel oil tank. Conduct extensive damage survey and submit a transit plan for OCMI approval. Vessel is not to be moved without approval from the OCMI. 14JUN2025: MI's attended vessel at anchorage in St Marys River. Pollution threat has been minimized and vessel is cleared to sail to Fraser Shipyard in Superior, WI to effect permanent repairs under the oversight of Class and cognizant [name withheld] degradation or pollution shall immediately be reported to the Coast Guard.

Vessel has shifted from HFO to Diesel fuel as primary requiring calibration of the main's governors. Vessel's Chief was able to make adjustments to get from anchor to the dock. Company is flying in a tech rep to oversee the changes made and will report the vessel as being safe to sail when complete. Tech rep will make the transit to Superior, WI and fine tune once the ship is under load outbound from the St. Marys River. Once initial adjustments are confirmed make a report to the [name withheld] Lakes and upon final have tech rep submit report.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this sub chapter. Vessel removed port shaft generator from vessel to conduct repairs, vessel plans to reinstall port shaft generator. Make port shaft generator available for inspection and testing once installed.

While underway in Lake Huron the vessel experienced a loss of propulsion, the port main engine had a high oil mist detector alarm and shut down the port main. After cleaning the oil mist detector, and finding no other issues the engineers put the engine back on-line and it is operating as normal. Chief Engineer shall provide attestation of incident, repairs, and that all associated systems are fully operational.

The installations of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F of this chapter. The material, design, construction, workmanship, and arrangement of main propulsion machinery must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Governor actuator on the port main engine experienced an oil leak due to a plug failure. Afterwards the port engine was unable to be clutched back in. Meanwhile the starboard engine gauges in the pilothouse were showing that the fuel line was not delivering fuel to the starboard engine. Make repairs and test to the satisfaction of the U.S. Coast Guard Officer In Charge of Marine Inspection. 46 CFR 96.03-1(a) / 46 CFR 58.05-1(a) / 60 / c

All engine installations shall be suitable for the intended service, and designed and constructed in conformance with subchapter F. Vessel Master made notification of a high temperature alarm on the turbocharger of the starboard engine with possible relation to a broken valve guide on the intake side of head #4.

Vsl reported a pinhole leak in the firemain system piping and made temporary repair while en route to NPOC Marquette, [name withheld] repair shall be made to the satisfaction of attending surveyor or marine inspector.

VESSEL UPGRADED REVERSE POWER RELAY ON PORT AND STARBOARD UNLOADING GENS. OBTAIN APPROVED DVTP AND PSTP AND COMPLETE TESTING AS REQUIRED PRIOR TO OPERATING WITH GENERATORS IN PARALLEL. 46 CFR 62.20-1 (a)(7)

Periodic safety tests must be conducted at intervals specified by the USCG to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Company employee designated to conduct PSTP's on the generator does not follow the approved PSTP's. PSTP's must be utilized. Either follow or obtain approved amendment for PSTP's to reflect actual conditions for testing. Specifically, procedure B9-B and B9-C for the generator are not being followed. CLOSURE FOR OPENINGS ABOVE THE FREEBOARD DECK MUST BE AS STRONG AS THE STRUCTURE TO WHICH THEY ARE ATTACHED AND MUST BE WEATHER TIGHT. MAKESHIFT WOODEN BOARD AND VENT ARE IN PLACE IN STATEROOM(S). SUBMIT CAR. 46 CFR 45.143

Where mechanical ventilation is provided for spaces which are protected by a carbon dioxide extinguishing system, provisions shall be made so that ventilation system is automatically shut down with the operation of the system to that space. Upon testing of ventilation louvers in the upper engine room, it was found that the port side louvers did not sufficiently close.

An inspection either general or partial depending upon the circumstances shall be made whenever any important alterations are undertaken. IAW NVIC 10-82 ABS must provide the OCMI with a letter stating approval any any outstanding requirements or limitations from the midship winch, new installation. Until such time as the OCMI receives this information, the midship winch is not approved for use by the USCG.

Where the general emergency alarm signal cannot be heard over background noise, there must be a red-flashing light or rotating beacon, in addition to the general alarm signal. No visual means of alarm is provided on the bottom deck of the engine room.

Each generator arranged for parallel operation must have reverse power or reverse current trips. Upon testing of the starboard generator reverse power relay, it was found to be inoperable. Crew is troubleshooting. Starboard generator not to be placed in parallel operation until repair is made and satisfactorily tested.

Piping, including valves, pipe fittings, and flanges, conveying vapors, gases or liquids whose temperature exceeds 150°F, shall be suitably insulated where necessary to preclude injury to personnel. Steam outlet piping for the economizer safety relief valves were missing insulation.

The port main engine shutdown as a result of a low lube-oil pressure condition, due to failure of the #4 bearing, as reported by the vessel's Master. Make repairs to the port main engine to restore normal functionality prior to departure from the Port of Duluth/Superior. Contact MSU Duluth Duty Marine Inspector upon completion of repairs to witness engine test(s) and provide service technician's report.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering). Fire main in the engine room has a leak with a temporary patch. Effect permanent repairs, provide class report attesting to repairs.

Port main engine shut down while transiting downbound Lake St. Clair around light #10 due to the #4 Cylinder mist detector alarm.

Vessel has a leak in the dredge piping for the #7 port ballast tank.

Vessel reported a hot bearing alarm forward side of the port engine. Vessel anchored south of Whitefish Point to allow port engine to cool and troubleshoot. The engineers reported the issue was a tripped sensor. If moisture is sensed in the oil, then the sensor will trip due to the heat, creating steam. Port engine is up and running in good working order.

Port side fire main has pin hole leak, temporary soft patch applied. Effect permanent repairs and provide proof of successful repair and applicable testing.

Port clutch damage

Conduct inspection/testing of stbd-side open lifeboat IAW the USCG Concentrated Inspection Campaign for Open Lifeboats (MSIB Number: 04-18). Port-side lifeboat has already been satisfactorily inspected/tested (stbd-side lifeboat was unavailable for testing at that time due to docking arrangement). Inspection/testing must be witnessed by USCG Marine Inspector at the next feasible port or place.

Starboard side fire main line near frame 116 in the tunnel had a 6 foot crop and renew needed due to pin hole leak. Due to wastage.

Small drip was identified on Fire main. Permanent repairs were requested in Detroit. Fire Main was operationally tested by vessel to ensure pressure could be held.

Effect permanent repairs to forepeak stem hull plating fracture in vicinity of 25 foot mark. Repairs shall be completed prior to departure from NPOC.

Port Engine #5 high pressure fuel injector line failed. Make proper repairs to fuel line iaw 46 CFR 56.50-75 to the satisfaction of the attending marine inspector.

Launch and exercise the Port Lifeboat.

10CM RADAR IS NOT OPERATIONAL

01. Complete repairs to starboard engine block seal prior to departing next port of call or provide documentation from class society attesting to operating for extended period of time on one engine.

Affect permanent repairs to fire main system to the satisfaction of the cognizant OCMI.

Submit proposal to ABS technical for review & approval to reflect the acceptability of additional brackets to be fitted under the cargo hold screen bulkheads to eliminate potential nuisance cracking of the ballast tank/cargo hold slope plating as required by 46CFR91.45 & ABS Class Survey Report SB2577087, dated 27MAR2014. See USCG MILSE activity 4819231 for specific details & ABS Survey Report.

To the satisfaction of an attending CG Marine Inspector, repair 3 foot vertical hull fracture at the bow stem. Fracture was discovered by vessel crew and reported to the CG on 28MAR14. Master reported that the damage was caused by thick plate ice as the vessel making way out of Escanaba, MI.

Effect repairs to the 4" fracture, No. 6 ballast tank starboard side frame 170 at junction of tunnel and cargo hold slope sheet.

Wing ballast tanks #1 starboard and #3 port contain cement box patches. These patches are considered temporary repairs and permanent repairs must be made prior to completion of next credit dry dock exam due 30 April 2013. Presently the vessel has no other cement box patches or temporary hull repairs.

Provide proof of proper operaion of oil distribution box prior to departure.

Provide guards for steam reduction stations fwd of main diesel engines. Stations are at chest height and in a location that can easily burn transiting crew.

Prove main engine start air receivers comply with 46 CFR 54 by providing a copy of the U1A Report and Drawings stamped by a [name withheld] stating the pressure vessels meet the requirements of Division 1 of section VIII of the ASME Code as modified by 46 CFR 54.

Submit repair proposal for cracks in the Stbd #4 ballast tank IWO frame 115, 3 feet inbd from sideshell and frame 105 10'-12' inbd from sideshell. Repair proposal and repairs to be completed prior to next cargo load.

Conduct soundings in the stbd #4 ballast tank once every 4 hrs. until vsls cargo is unloaded. Notify cognizant OCMI if condition worsens

During transit, should seas reach 8' or winds in excess of 30kts vessel shall seek the nearest safe shelter until wx subsides.

Prior to operating donkey boiler, prove proper operation of hand lifting device for relief valves..

Perform weight test to 110% on both lifeboats/falls. Document results for review during annual SIP Audit.

Open auxiliary boiler mounts for inspection. To be completed prior to next vessel lay-up period.

Remove doublers and make permanent repairs to port main boiler bottom blowdown overboard spool piece at next dry dock.

Remove cement box patch and make permanent repairs to evaporator blowdown spool piece at next dry dock.

Drive shaft gears failed rendering bow thruster inoperable.

Install retroflective tape on ring buoy, port side aft. Note: Checked other ring buoys & found to be in satisfactory condition. Master noted to install retro. tape on 1 ring buoy.

Tested E-generator by simulating loss of power. First test, E-generator did not come on line. Tested a second time, and automatic operation worked correctly. Spoke w/C/E who said they will look further into adjusting crank timer accordingly. Issued 835 on 26 DEC 2003 w/COTP Order 05-2003.

Immersion suits found to have retroflective tape peeling. Replace as necessary.

Within 15 days, conduct lifeboat drills using the port and starboard lifeboats, exercising the crew at oars. This requirement does not have to be completed in the presence of a USCG Marine Inspector. Notify this officeby letter, signed by the Master upon completion, and no later than 18APR91.
