Vessels hot water heating auxiliary boiler has been replaced with new unit of same design and manufacturer. Boiler is prohibited from use until installation inspection, hydrostatic testing and the witnessing of all safety shutdown devices is completed.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Vessel owner/operator failed to complete or provide record of completion of required inspections and/or maintenance of and fixed fire suppression system. 46 CFR 176.810(b)(2)

The owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. Inspection and testing of all smoke and fire detection systems, including sensors and alarms. Vessel owner/operator failed to complete or provide record of completion of required inspections and/or maintenance of the fire detection system. 46 CFR 176.810(a)(7)

Maindeck port side RLB lifeline is deteriorated. RLB must be replaced.

Engineroom bilges have rags & debris, all other bilges have excessive dirt & debris, all require cleaning.

Anchor stock bent making anchor unusable/ineffective. Must be repaired/replaced.

EPIRB battery is expired. Service EPIRB or replace with new unit. ** Vessel is limited to NMT 3 NM from land until deficiency is corrected.

Port rudder packing leaking. Monitor and continue with approved maintenance practices. If leak worsens, OCMI notification is required. Packing must be replaced no later than at next dry-docking.

Based on reports from the CGC BISCAYNE BAY, THE SENATOR was observed operating in 6-10" of Plate/Rubble ice on a transit from St. Ignace, MI to Mackinac Island, [name withheld] becoming beset by the ice, THE SENATOR had to be assisted by the Coast Guard ice breaker to free them. Based on previous history of damage to this vessel while being operated in ice and reports of the conditions that day, the Coast Guard suspects the possibility of damage to hull plating or structural members that may affect the sea worthiness of this vessel. Prior to the vessel's movement, Coast Guard Marine Inspectors will conduct an internal structural examination, and based on those findings, may require the vessel to be drydocked or taken out of service to assess the extent of any damage found, and to effect any necessary repairs in accordance with 46 CFR 176.600(d). You are reminded, that in accordance with 46 CFR 176.700(a), repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI.

Vessel has sustained structural fractures at the swosh bulkhead vertical support stiffeners in voids 3 and 4, and on the wing overhead transverse frames in voids 3, 4, & in the engine room on the starboard side. Submit a repair proposal to effect permanent repairs to damaged areas. If owners desire to operate prior to permanent repairs, submit proposed interim operating procedures which include loading documentation and a monitoring plan. Meticulous loading logs shall be maintained for all deck cargo transits. Include cargo weights and some form of mapping of the distribution of cargo on deck. Each trip record must be signed by the Master attesting to the accuracy of the record. All transits must be in full compliance with the vessels issued stability letter. Proposal must also include a plan for the company to monitor known damage via weekly inspections and document the findings. Company loading documentation and documented inspection results will be reviewed by Coast Guard Marine Inspectors at follow-up inspections. Any further degradation shall be reported immediately to the [name withheld].

Electrical shore tie cable has a temporary splice made and is kinked. Replace shore tie cable

Lazarette hatch cover T-handle is broken and makes hatch not watertight. Repair/replace T-handle and make hatch serviceable to the satisfaction of the attending Marine Inspector.

Each lifering must have the vessel's name marked clearly in legible block capital letters. The Starboard side fwd lifering does not have the name legibly on it.

Rudder packing is leaking and needs maintenance.

Escape hatch from the engine room and ladder below passenger cabin must be labeled with "EMERGENCY EXIT, KEEP CLEAR" in 2 inch letters.

Vessel needs four more distress signals based on route.

When Class 1 materials are being loaded, handled, or unloaded on a vessel, the vessel must exhibit the following signals: By day, flag “B” (Bravo) of the international code of signals; and By night, an all-round fixed red light.

Fire stations do not have spanner wrenches, install spanner wrenches at each fire station.

All batteries must be properly chocked to prevent shifting.

Replace dilapidated pfd.

The vessel is currently broadcasting (1) Improper Call Sign.

Replace 3 child size PFDs, prior to carrying more than 120 adults and more than 12 children.

Provide Hazmat Employee Training certificates for master and crew prior to carrying hazardous materials in cargo

Removeable gates/deck railing at bow loading area not in use while underway, master believed it was not needed when passengers not aboard. Corrected during inspection.

Improper size portable fire extinguishers has been substituted during recent servicing/inspection by servicing company. Required correction prior to operation, completed during inspection, master was counseled on ensuring proper fire extinguishers are always aboard as per the COI.

Complete temporary repairs to lazarette hull plate fracture per approved repair plan.

Complete permanent repairs to lazarette hull plate fracture.

Replace outdated nav rules, nautical chart, light list and coast pilot.

Post operating instructions for fixed E/R extinguishing systems at control station.

Make RLB strobe light operational.

Post EP's at operating station

Portable fire extinguishers need anti-chafe material between extingishers and mounting bracket to prevent continued wear/corrosion.

EPIRB monthly testing must be logged.

Main deck escape hatch for engineroom needs to be re-stenciled.

Mark upper deck with maximum passengers allowed per stability ltr.

Flexible sections of fuel fill piping must be properly bonded to prevent static electricity build up.

Mark boiler with reduced MAWP at 30 psi for water per 46CFR53.01-10(b)(2).

Make boiler available for examination and 1.25 hydro test in presence of Marine Inspector.
