MIDNIGHT STAR

While conducting an in-service inspection, MI found the follow dericiencies: 1. Missing EPIRB 2. Gas generator located outside the pilot house with an extention cord running down to the main deck that connected to a second extention cord that was hard wired into a breaker panel. Where the two extention cords connected the plugs where melted and burnt. Owner explained that they where just using it the night before while at the dock while they install a rebuilt generator. MI entered the generator space to fine a generator that was not attached to fuel, exhust, or wires. Owner explained that all the ship navigation systems operated off battery and they only used the installed generator of lighting. 3. Several cable and wires were broken or frayed throughout the vessel 4. Upon starting the main engines, MI found several water and exhust leaks in engine space 5. Fire protection is out of service 6. At least one bilge pump is not operating, MI did not continue to test other bilges due to oily water thoughout vessel Vessel must undergo a full annual inspection prior to movement of the vessel. 46CFR176.25-50

Fire detection and alarm system must be installed in accordance with 46CFR76. Alarm panel not operational.

A submersible electric bilge pump maybe used as a power bilge pump only when vsl carries not more than 49 passengers. Installed bilge system not operational. Vsl allowed to carry 49 pax until installed sys is operational.

Bilge overboard discharge in 2nd Void must be mounted on the hull.

Sewage discharge valve corroded in half open, half shut postion. Install new valve.

Handle on salwater intake valve corroded. Replace handle.

STBD rudder post leaking. Leak was continuos (approx. .5L/Min) and at packing gland.

Taking on water at the STBD chine in the generator space second frame from aft bulkhead. Minor weeping while stationary. Large streams while vsl flexes during port turns.

Replace BA for Life Floats

Replace 02 Clamps securing overboard hose to valve in generator room.

Repair 120V lighting system throughtout vessel

Battery expired on BA floating water marker light.

Secure leaking of STBD oil supply line from oil filter just fore of the STBD engine.

All machinery spaces must be outfitted with fixed woth a fixed fire protection system of an approved type. Fixed CO2 system in machinery has not been serviced. 46 CFR 181.20-5

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC type accepted category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB box was found in non-float free condition, hydrostatic release did not have year of expiration date only month. Replace EPIRB box, mark hydrostatic release with expiration date and year. 46 CFR 180.64 Vessel is restircted to 3nm from shore

An adult life jacket must be provided for each person carried on board a vessel. A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Each life jacket on board must be marked in accordance with 46CFR185.604: Found three (03) adult lifejackets, and three (03) child lifejackets which needed to replaced as well as an additional childrens lifejacket without proper markings. Replace destroyed lifejackets, mark new lifejacket with name of vessel. 46 CFR 180.71

Each ring life buoy with a floating water-light must have a lanyard of at least three (03) feet in length but not more than six (06) feet in length, securing the water-light around the body of the ring life buoy. Life ring buoy floating light was found cracked, line attaching floating light was measured at over seven (07) feet long. Reduced length of lanyard. 46 CFR 180.70

Survival craft and life floats shall be stowed so that when the vessels sinks the survival craft floats free and if inflatable inflates automatically. Light for Port-Aft buoyant apparatus was found lashed down which would prevent the buoyant apparatus from floating free. Tie-water light properly so that it will float free. 46 CFR 180.130

Survival craft and life floats must be fitted with a lifeline, pendants, a painter, and a light. Each light for either lifefloat or buoyant apparatus was found in need of new light battery.

At every vessel inspection, quarters, toilets and washing spaces shall be inspected to determine serviceable and sanitary condition. Vessel head was found to be unsanitary condition and black liquid was found in toilet. 46 CFR 176.818

At each inspection all observed safe practices and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Grip tape on ladder was missing; replace grip tape on ladder as to prevent slips, trips, and falls. 46 CFR 176.830 (a)

Except as provided otherwise in this subpart, all electrical equipment including motors, generators, controllers, distribution panels, breaker panels, etc. shall be drip-proof protected. Breaker panel forward of engines in accommodation locker found loose from bulkhead as well as the panel door had a broken latch. Fasten panel to bulkhead and fix panel door latch. 46 CFR 183.10-10

Replace butt block in STBD side fuel tank space 2 bay from aft bulkhead, 1st plank above chine. Butt block was rotted.

Next haul out re caulk Port and STBD Chine. Weeping was noticed coming from both Port and STBD chine. 46 CFR 176.20-1 THIS WILL REMAIN PENDING UNTIL THE DDX.

Demonstrate proper operation of all water tight hatches and reinstall safety chains. Vessels watertight hatches were painted over and no gasket was installed. 46 CFR 176.25-40

Provide full insulation on both Port and STBD MDE exhaust pipe to water injector. Missing insulation on both exhaust piping. 46 CFR 182.15-15

DK01 Ansil fire detection panel at the pilot house needs to be repaired or replaced, inoperable.

DK02 Operator unable to start the port engine, demonstrate proper operation.

DK03 Replace four adult life jackets and one child.

DK04 One of the BA are no longer serviceable, replace with a LF.

DK05 generator compartment port side first frame bay from fwd bulkhead, first butt block below chine tighten through bolts.

Provide proper operation of the fixed bilge pump; did not work during inspection.

Provide proper rudder post packing to prevent excessive water ingress. Both rudder posts are leaking excessively.

Provide proper operation of all nav lights. Not all lights operational during insp.

Install hydrostatic release on EPIRB; expired. Prior to operating more than 3 NM.

Install float free light on ring buoy.

Install hatch seals on all weather deck hatches.

Properly seal all bulkhead penetrations throughout vessel.

Remove kink from lazarette electric bilge hose.

Provide proper operation of stbd MDE RPM gauge.

Provide proper operation of generator PSI gauge.

Vessel lost steering during sea trail due to hydraulic hose failure.

Provide CO2 protection from accom spaces; ER protected and not tight.

Address all hull and structural concerns listed in DD/ISE inspection activity #6006920 prior to operating with passengers. Hull/ISE not given.

Complete credit DD/ISE and clear all outstanding CG-835s prior to operating w/passengers

Vessel's stbd shaft log was severely corroded and hole was discovered. Vessel actively taking on water. Vessel is to drydock and make all repairs necassary to correct wasted shaft log. Port shaft log is to be inspected as well to determine if it is wasted as well.

Generator is not permanently mounted to the vessel. Vessel is to mount generator permanently in generator space.

JK01: Both rudder posts are leaking and pouring water into the lazarette. Vessel must re-pack both rudder posts. 46 CFR 179.210.

JK02: Vessel must seal all watertight bulkhead penetrations where wire or pipes pass through. 46 CFR 179.320 & .310.

JK03: There is a crack in the stbd side shear clamp, void 2, 4th frame fwd from aft bulkhead. Vessel must repair or replace. Owner must submit a repair proposal prior to conducting any work. The shear clamp in this space is sistered to rot; this is not good marine practice. 46 CFR 176.700

JK04: There are numerous wires throughout the vessel that are not properly ended. Wire connections must end in a junction box. Vessel must make all wires safe. 46 CFR 183.220.

JK05: The vessel's fuel fill flexible lines are collapsed and not properly grounded. Vessel must repair lines and grounding. 46 CFR 182.445(g).

JK06: Vessel must provide bilge strainers for each missing bilge line. 46 CFR 182.510.

JK07: Vessel must prove proper operation of the fwd bilge alarm. 46 CFR 182.530.

KK01 - Replace EPIRB hydrostatic release unit.

KK02 - Replace all portable fire extinguisher brackets w/ approved type.

KK03 - Install metal drip shield for generator fuel filter.

KK04 - Replace 20" life rings with approved 24" model.

KK05 - Repair inoperable power driven bilge pump. Corrected on the spot.

KK06 - Repair high bilge alarms to be audiable and visial.

Recaulk weeping seams in lazarette and aft void space on stbd side in way of the chine.

Repair and replace leaking fuel piping in the generator space. current configuration has a non metallic hose clamped to exsisting copper tubing. Additionally, reroute the return and supply fuel oil piping from the generator space.

Clean and remove excessive oil and fuel in the engine room and generator room bilges. Ensure bilge overbaord hose is clear of oily mixtures.

Electrical connections for all bilge pumps and float switches need to be placed in watertight box.

Generator has various salt water leaks that are in need of repair. Repair all leaks on generator.

Vessel has several dead end wires throughout the engine room and generator room. Properly terminate all dead end wires.

Replace gaskets on hatches for the lazerette and aft void space. Both gaskets are severly worn.

All electrical connections throughout the vessel need to be connected to a commoon ground.
