Every company should develop, implement a safety management system which includes instructions and procedures to ensure safe operations of the ship. During expanded exam into the failure of the rescue boat davit system, several SOLAS/ISM related discrepancies were noted. The davit system it self would not operate on stored mechanical power and the rescue boat was found to be damaged on the port side, due to nails sticking out of the blocks where it sits.
Several areas of the SMS was found to be lacking clear guidance on; the maintenance of the rescue boat and davit system, procedures on proper operation of the davit system and training for crew responding to emergency situations.

Tested generators to see if one tripped offline due to an increased amperage load if the other would kick on and it did not work. Vessel added many heaters due to going to Alaska, adding to the electrical demand so we tested generator capabilities. Vessel is also a DP-2 so redundancy is required for the generators.
