The public address system shall be a loudspeaker installation enabling the broadcast of messages into all spaces where the crew members are normally present. Sanitary accommodations shall comply with the following requirements; (a) floors of durable material, easily cleaned and impervious to damp, and properly drained; (c) cold fresh water and hot fresh water available in all wash spaces. When testing the water flow in the sinks it was noted that the water was brown. This was also present in the toilets. Crew reported this to be a problem every time the vessel switches power, causing the system to drain (lose pressure) and then the lines fill with rusty water. The hospital must be suitably separated from other spaces and must be used for the care of the sick and for no other purpose. A crew member was found living in the hospital. 46CFR 92.20-35(b) 50 AC

Public address system shall be available throughout the ship. Lower deck space speaker ws turned down. SOLAS Ch II-2 Part D Reg. 12 10 C

A launching appliance shall not depend on any means other than gravity or stored mechanical power which is independent of the ship's power supplies to launch the survival craft or rescue boat it serves in the fully loaded and equipped condition and also in the light condition. Rescue boat davit unable to operate using stored mechanical power. Demonstrate proper operation of rescue boat davit. SOLAS III/34, LSA 6.1.1.3

Every company should develop, implement a safety management system which includes instructions and procedures to ensure safe operations of the ship. During expanded exam into the failure of the rescue boat davit system, several SOLAS/ISM related discrepancies were noted. The davit system it self would not operate on stored mechanical power and the rescue boat was found to be damaged on the port side, due to nails sticking out of the blocks where it sits. Several areas of the SMS was found to be lacking clear guidance on; the maintenance of the rescue boat and davit system, procedures on proper operation of the davit system and training for crew responding to emergency situations. 74SOLAS (2020) Ch. The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea. The port main engine experienced a low jacket water alarm, with water leaking out of various inspection ports. Identify the source of the leak, make any necessary repairs, and demonstrate proper operation of the engine and its associated systems.

Corroded elbow in hydraulic line to tethering system, allowing hydraulic fluid to leak on deck and into water.

Tested generators to see if one tripped offline due to an increased amperage load if the other would kick on and it did not work. Vessel added many heaters due to going to Alaska, adding to the electrical demand so we tested generator capabilities. Vessel is also a DP-2 so redundancy is required for the generators.

None of the immersion suits had lights on them nor did they have the vessel's name stenciled on them.

Emergency fire pump only generated 45 psi.

Vessel overhauled propulsion engines. There was no SOLAS tape covering the high pressure fuel fittings.

Submit one copy, for each item under Title 46 Code of Federal Regulation part 62.20-3, to replace specific personnel or to reduce overall crew to the cognizant OCMI.

Make vessel available for a minimum of 36 hours for underway inspection using approved vital automation test procedures and provide associated systems performance logs, overtime sheets etc.. reflecting automation system performance for cognizant OCMI.

Prepare and submit for approval automated vital system test procedures in accordance with Title 46 Code of Federal Regulations part 61.40 in the format described under Title 46 Code of Federal Regulations part 61.40-10 to the cognizant OCMI via the Marine Safety Center.
