The master, owner, or agent of the vessel shall notify the officer in charge, marine inspection, whenever the is dry docked regardless of the reason, during a routine yard inspection it was observed that the vessel was dry docked and had been dry docked for over a month with active repairs being conducted to the vessel's structure though no prior notofication was given about the vessel's current dry docking period.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the officer in charge, marine inspection. During discovery of the vessel being in dry dock, many repairs to the hull were in various stages of completion with no prior notice given to the [name withheld] and desist all structural repair work on the vessel until a meeting can be scheduled to the OCMI notice of all repair work and/or alterations being made to the vessel.

Vessel must undergo a dry dock, internal structure exam as set forthe in table 46 CFR 91.40(a). Vessel must undergo a dry dock and ISE exam every 2.5 years or twice in 5 years with no more than 3 years in between. Vessel's COI dry dock and ISE dates reflect 12APR2019, vessel did not notify the CG and therefore went past the required inspection dates. COI removed from the vessel. Make vessel's hull, void, ballast tank and integral fuel tanks available for inspection.
