1) The company should establish procedures to ensure that the ship is maintained in conformity with The provisions of the relevant rules and regulations, and with any additional requirements which may be established by the company. Multiple fuel oil leaks were observed in the engine room. E/R log states rounds are being conducted every four hours but there is no guidance as to which items are to be checked and at what frequency. SMS states oil leaks shall be cleaned, but multiple catch basins were observed throughout E/R, including approximately 2 inches oil under M/E shaft seal near cylinder 4 and 5. None of these fuel leaks have been reported to the company for rectification. Based on the objective evidence sited above, the vessel is not in substantial compliance with the relevant conventions, this questioning the adequacy and overall implementation of the vessels SMS. An external audit conducted by the flag or RO to determine the ship is operating in accordance with the ISM Code is required. 74SOLAS(20) IX/3.1 ISM CODE 10.1

2) In a ship which oil fuel is used, the arrangements for the storage, distribution, and utilization of the oil shall be such to ensure the safety of the ship. PSCO observed fuel oil leaks in numerous locations including, but not limited to, Main Engine Shaft Seal near cylinder #4 and #5, M/E crankcase, pooled oil in m/e containment, lube oil purifier pump, and aux generator piping. Crew was observed removing manufactured catch basins full of oil and oil soaked rags in areas where oil leaks were observed. In addition, Excessive amounts of pooled fuel oil were discovered under step of main engine, coming from M/E shaft seal near cylinder #4 and #5. 74SOLAS(97) II-2/15.2

The Administration of a party may allow any fitting, material, applinmace or apparatus to be fitted in a ship other than procedures and shall communicate to the organization for a circulation to the parties particulars thereof. Scrubber system operates properly. However, RO has not been notified of alternative arrangements.

The ORB part 1 shall be completed on each occassion on a tank to tank basis whenever there is collection and disposal of oil residue (sludge). Sludge tank cap was removed and 200L of water from scrubber system gets pumped into the sludge tank unrecoreded.

PSCO requested the vessel's daily checklists for engine room maintenance. The daily work report for 9/20/21 stated the engine room was cleaned and painted and the daily work report for 9/18/21 stated the main engine crank case was inspected. Additionally, permits to work for 9/21/21, 9/16/21, and 9/15/21 stated cleaning was being conducted in the engine room. Due to objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under the ISM Code. An external audit conducted by the Flag or RO is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port. 74 SOLAS (20) IX/3.1 ISM Code 10.1

07126 - In a ship which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil shall be such to ensure the safety of the ship. PSCO observed fuel oil running down both sides of the main engine, fuel oil leaking from the gasket on crankcase #2, oil soaked rags stuffed in numerous spaces on the main engine and all 4 generators, oil splattered on lighting in the vicinity of the main engine, fuel oil leaks and pooled oil around all fuel pumps for the main engine, and fuel leaking from the injector on the #6 cylinder. Additionally, there were fuel oil leaks from multiple crank cases on all 4 generators, oil leaks and pooled oil on top of the generators, and leaking pipelines around generators. 74 SOLAS (97) II-2/15.2

02108 - Main and emergency switchboards shall be so arranged as to give easy access as may be needed to a apparatus and equipment, without danger to personnel. The sides and rear, and where necessary, the front of switchboards shall be suitably guarded. PSCO observed openings on front and rear of main switchboard in ECR. 74SOLAS(04)II-1/45.2

02105 - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed hydraulic oil leak, with a rag soaking up the leak, on the steering gear. 74SOLAS(20)I/11(a)
