Vessel's follow up steering located at the main operating station had a malfunction that put the rudder hard over. The cause of the malfunction was a broken spring inside the follow up joy stick which recenters the rudder.

All machinery and equipment must be installed and maintained in such a manner to afford adequate protection from causing fire, explosion, machinery failure and personnel injury. Vessel's port reduction gear output shaft flange has failed. Make reduction gear available for examination by an attending Marine Inspector to ascertain the cause of the failure, before effecting repairs to the satisfaction of the attending Marine Inspector. Vessel is authorized a one time transfer from Onset MA to Fairhaven Shipyard in New Bedford MA for repairs. Notify this office prior to departure from Onset, and upon safe arrival at the shipyard.
