Light. The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. Port aft life float light did not work when tested. Repair life float light to the satisfaction of the attending marine inspector.

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Vessel had combustible waste receptacles on board. Remove combustible waste receptacles and replace with metal or Sub chapter Q approved non combustible type.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. During the dry dock examination the following items were found and need to corrected before issuance of dry dock credit. Forward keel showed delamination from the hull structure. Fasten delaminated section of forward keel to the hull. 46 CFR 176.700

During the dry dock examination the following items were found and need to corrected before issuance of dry dock credit. Approximately a 6"x6" section of exposed wood was found above starboard propeller. Remove any rot and fill. 46 CFR 176.700

An individual may not be engaged or employed, including self-employment, on a vessel in a position as master, operator, or person in charge for which a credential is required by law or regulation unless all crewmembers covered by this section are subject to the random testing requirements of this section. Vessel master was not listed on the consortium covered crew list. Provide documentation that master is subject to random testing for the company.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety radio Telephony Certificate (installation certificate) was not available for inspection. Provide valid FCC safety radio telephony certificate.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Vessel was unable to dewater the crash locker or the lazarette. Demonstrate the dewatering of all watertight compartments to the satisfaction of the attneding USCG marine inspector.

Each vessel must be equipped with a public address system which be must be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Aft deck public address speaker not operable. Demonstrate public address system for all required spaces.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Passengers not permitted on upper deck, but no markings were present. Mark upper deck as restricted to crew only.

The Rules concerning shapes shall be complied with by day. The lights and shapes specified in these Rules shall comply with the provisions of Annex I of these Rules (33 CFR part 84). Vessel was not carrying day shapes. Provide day shapes.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. The alcohol testing devices need not be carried on board each vessel if obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Alcohol testing devices not on board. Provide alcohol testing devices.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. 8AWG cable running on deck stringer above the inboard side of the port engine found without metallic supports. Provide proper cable supports.

Except as allowed by paragraph (d) of this section, each vessel constructed of a material other than wood certificated to operate on a limited coastwise route in cold water must be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. (c) Except as allowed by paragraph (e) of this section, each vessel certificated to operate on a limited coastwise route in warm water must be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. Vessel;s life raft servicing is overdue. Vessel is prohibited from carrying passengers or cargo until it is provisioned with primary lifesaving meeting the type and capacity of 46 CFR 180.205.

Anchor light inoperable. Demonstrate proper operation of all navigation lights.

Vessel is required to have three 2.5 gallon fire buckets, but only has two. Provide additional fire bucket.

Vessel's genset was not properly grounded causing arching and sparking upon starting.

Provide FCC station License, Safety certificate, & Operator's permit.

Provide updated DAPI program including MIS report.

Provide current charts, coast pilot, light list and tides/currents.

Provide record of monthly EPIRB testing.

Provide hydrostatic release for EPIRB showing expiration date.
