At a minimum, vessel operators must conduct an internal examination and take random hull gaugings internally during the hull condition assessment, unless waived by the Officer in Charge, Marine Inspection (OCMI). The OCMI requires the annual hull assessment to attest to the depth of the identified areas of pitting to ensure the corrosion is arrested. The areas of pitting must be included in the required preventative maintenance reports to include permanent markings on the inside of the hull structure to accurately identify the location of the pits for future inspections and ultrasonic/non-destructive testing. 46CFR176.660(a)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below. (b) Steel hull vessels: ABS Steel Vessel Rules. A complete application for an original proof that the applicant passed all applicable vision, hearing, or medical, and/or physical exams as required by subpart C is required by subpart C of this part or a valid medical certificate issued by the Coast Guard. The medical caertificate for the merchant mariner credential for the captain is not up to date. Captain may not operate commercially prior to updated medical certificate is provided. 46 CFR 10.225(b)(7)

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. The cable runs from batteries to the engines in the two aft voids need to made watertight. 46 CFR 179.320

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. The bilge strainers in the vessel's two aft voids need to be made such that the suitable strainer has an open area 3x the area of the bilge pipe. 46 CFR 182.510(b)

To continue to participate in the AHE program, vessel operators must conduct an annual hull condition assessment. Vessel needs to provide an annual hull gauging report for the continue enrollment in the AHE. 46 CFR 176.660(a)

The breaking strength of each link for the life float must be between 900N and 1200N for links intended for life floats and buoyant apparatus of 11 to 20 persons capacity. The vessel did not have the appropriate weak link for one of their life floats. 46 CFR 160.073-10 (b)(2)

The lifeline and pendants must be furnished by the manufacturer with the approved life float or buoyant apparatus. The pendant line for the life float was severely chafed and needs to be replaced. 46 CFR 180.175(f)(1)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking. As per 46 CFR 160.027-2, each life float must have Type II retro-reflective material meeting subpart 46 CFR 160.018 on each side and end. Life float retro-reflective tape wasted an requires renewal.

Batteries - general: Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Battery connection to terminals made with wingnuts, which are not a permanent type connector. Replace with nuts.

Charts and nautical publications - as appropriate for the intended voyage, a vessel must carry adequate and up-to-date. Coast Pilot and Light List missing.

An EAP training program must be conducted for the employer's crew members and supervisory personnel. Missing documentation of training given to crew members and the employer's supervisory personnel. Provide proof of training.

The owner or managing operator shall make sure that maintenance is carried out in accordance with manufacturer's instructions. The 02 life floats are beginning to show signs of deterioration. Conduct monthly maintenance IAW manufacturer's instructions. 46 CFR 185.702(b)

Each item of lifesaving equipment with an expiration date must be replaced during the annual inspection if the expiration date has passed. The batteries in 02 water lights are expired. Replace IAW manufactuer's instructions. Restricted to daylight operations until rectified. 46 CFR 185.726(c)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at a constant marine load under such requirements without exceeding its designed limitations. The No. 3 outboard motor is currently inoperable. The vessel does not usually operate with this motor under normal conditions.
