The company and the ship shall comply with the requirements of the International Safety Management Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that inspections are held at appropriate intervals. PSCO observed that hourly maintenance was over for the following: M/E safety valves: 4821 hours overdue, M/E turbo charger cooler air side: 1926 hours overdue, D/G general inspection: 700 hours overdue, and D/G cylinder cover inspection: 700 hours overdue. M/E safety valves have been overdue for over one year. Provide corrective action plan from company with concurrence from administration. 74 SOLAS (14) IX/3 17bc

Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator or person in charge must immediately notify the nearest Coast Guard Sector office. On 27 April 2019, the crew identified water on top of the main engine #6 piston. After repairs were completed, the C/E noted that the repairs would not last long and submitted a requisition for a new cylinder head with company. Submitted eNOA that stated all equipment was operational. Provide corrective action plan from company with concurrence from Administartion. 33 CFR 160.216 17bc
