PSCO observed lock-out/tag-out procedures not being followed as per Company SMS procedures. #1 diesel generator was not tagged out in accordance with SMS as breaker was not tagged out in the open position and not tagged out in the open position and start air valve was not tagged out in closed position. No person may cause a vessel to enter into or get underway on the navigable waters of the United States unless no more than 12 hours before entering or getting underway, the following equipment has been tested:Primary and secondary steering gear. The test procedure includes a visual inspection of the steering gear and its connecting linkage. PSCO observed significant hydraulic oil leaks on both steering gear systems as well as insignificant oil in reserve tank during inspection after vessel was moored. Condition was not identified on vessel checklist prior to port entry. 33 CFR 164.25 (a)
