Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI observed vessel on Oceans Route with fire detection unable to demonstrate means of interconnected installation. Vessel limited to operations 20 miles offshore. 46CFR181.405(c), 46CFR175.110(c)(2), & 46CFR181.450

MI observed expired alcohol swabs. Replace with new ones.

"Life floats and buoyant apparatus must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers." MI observed missing spacers for life floats. Install appropriate spacers for life floats.

"Life floats. Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light." MI observed no presence of life float light. Install appropriate light for life float.

"Grab line. The finished length of the grab line shall be four times the outside diameter of the buoy. The ends of the grab line shall be securely and neatly spliced together, or shall be hand whipped with a needle and both ends securely and smoothly seized together. The grab line shall encircle the buoy and shall be held in place by the beckets. The spliced or seized ends of the grab line shall be placed in the center of the width of one of the beckets." MI observed life ring grab line in unsatisfactory condition. Replace life ring.

"As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible;(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List..." MI observed outdated charts, light list, and coast pilots. Replace with updated navigation equipment.

"All cable and wire must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage." MI observed only plastic zip ties supporting cable wiring along both sides of vessel. Replace with appropriate supports.

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Life float lines are taped together on all five life floats.

Life floats. Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. 10-person life float is inoperable.

Each "L" vessel will be equipped with (1) fire axe located in or adjacent to the pilothouse. Fire axe was not stowed correctly.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Both main engines and port generator have exposed rotating machinery.

Auxiliary machinery shall be suitable in type and design for the purpose intended. Port side generator oil filter hose and gasket around oil pan was found leaking. Repair to satisfaction of attending Marine Inspector. 46 CFR 182.10(a) (Old T)

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel's COI Expired on 22JAN2025, thus rendering it invalid. Vessel must undergo an inspection for a new COI prior to the carriage of passengers. 46 CFR 176.100.a

respective of build date, a vessel to which this subchapter applies must meet 46 CFR 181.405, 181.450, 181.500, 185.364, and 185.420(b), if it is not a ferry, and if it— (1) Has overnight accommodations for passengers; or (2) Is operating on a Coastwise or Oceans route. Outfit vessel with interconnected fire detection system.

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Replacement of existing equipment, not including wire or cable, installed on the vessel prior to March 11, 1996 need not comply with the regulations in this part. Vessel installed a new generator in October 2023 with no Coast Guard oversight. Submit to Marine Safety Center for plan review.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment... IDENTIFIED VARIOUS NON- AIR TIGHT OPENINGS TO ENGINE ROOM. MAKE AIR TIGHT.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve... ATTENDING MARINE INSPECTOR IDENTIFIED THAT THE LIFEFLOAT WATER LIGHT AND AFT PILOTHOUSE WERE INOPERABLE. OUTFIT VESSEL WITH WORKING WATER LIGHTS.

In the design of a control, interlock, or indicator circuit, the disconnect device and its connections, including each terminal block for terminating the vessel's wiring, must not have any electrically unshielded or uninsulated surfaces. ATTENDING MARINE INSPECTOR IDENTIFIED A SWITCHBOARD IN THE AFT ENGINE ROOM BULKHEAD THAT HAD AN OPENING. OUTFIT SWITCHBOARD WITH BLANKS.

Mechanically propelled "S" vessels which are ferry vessels or which carry more than 49 passengers and mechanically propelled "L" vessels shall comply with the provisions of Subchapter F of this chapter. ATTENDING MARINE INSPECTOR IDENTIFIED NONMETALLIC COUPLINGS ON VESSEL'S BILGE SYSYTEM. OUTFIT BILGE SYSTEM WITH METALLIC PIPING.

FORWARD CENTER LINE BULKHEAD ENGINE ROOM IN WAY OF BILGE, FOUND DEFORMED AND SCALING. PROVIDE UT OF BOTTOM 6" OF BULKHEAD.

PORT SIDE DECK DRAIN INTO ENGINE ROOM IS WASTED. PERMANENTLY REPAIR DRAIN.

33 CFR 164.46 Vessel must have on board a properly installed, operational Coast Guard type approved AIS.

Marine employers shall establish programs for chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who occupy a position, or perform the duties and functions of a position required by the vessel’s certificate of inspection. Vessel was underway with alternate crew members that were not subject to the vessel’s established drug and alcohol program. Vessel is restricted to operating no more than 12 hours away from the dock until the alternate crew are enrolled in drug and alcohol program to the satisfaction of the coast guard. Acceptable evidence of when a vessel operator is under the influence of alcohol or a dangerous drug includes but is not limited to: (a) Personal observations of an individual’s movement, manner, disposition, speech, general appearance or behavior. On May 13th 2021 a passenger witnessed crew members smoking marijuana and drinking alcohol while operating the vessel. Owner/marine employer must ensure reasonable cause testing is conducted. Prior to getting underway with passengers for more than 12 hours, the vessel must conduct a safety assessment and provide results to Sector New York Inspections Office. The safety assessment shall include but is not limited to; operation for conducting overnight passenger voyage, procedures for crew on watch and roaming patrolman, expectations on recording in the bridge log, how to limit electrical hazards like excessive cell phone chargers, actions taken to reduce excessive luggage and personal items from impeding egress, restrictions on alcohol on board, etc. Additionally items to be addressed during the safety assessment are to update the station watch bill to reflect current crew members, provide proof that a prepared voyage plan is occurring verbally or written prior to departure and provide the provisions to accurately record a passenger and crew list. During an at sea USCG boarding it was determined that the vessel was not operating with the required crew compliment on board. Vessel was underway on a voyage exceeding 12 hours with no alternate crew provided not in accordance with the certificate of inspection. Proceed directly to hailing port. Vessel is required to contact local USCG Sector Command Center every 2 hours on VHF Channel 22A.

Damaged fire main piping on port and starboard risers at the main deck.

Each life jacket carried on a vessel on an oceans route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. 32 adult and 02 child size life jackets require new batteries in the life jacket lights.

Each vessel that operates on the high seas, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. NOAA registration for EPIRB was not provided during inspection.

Each distribution panel or switchboard must be totally enclosed to prevent a person from accidentally contacting energized parts. There is an opening in the electrical panel located in the engine room space on the most forward bulkhead.

WIRING JOINTS AND SPLICES SHALL BE MECHANICALLY SECURE AND MADE IN JUNCTION BOXES. WIRING CONNECTIONS IN ALL SPACES FOR HIGH BILGE ALARM WERE NOT MADE IN BOXES.

THE BILGE SUCTIONS SHALL BE FITTED WITH SUITABLE STRAINERS HAVING AN OPEN AREA NOT LESS THAN THREE TIMES THE AREA OF THE BILGE PIPE. STORAGE SPACE BILGE SUCTION LACKED SUITABLE STRAINER.

PREPARE AND POST EMERGENCY CHECKOFF LISTS IN A CONSPICUOUS PLACE ACCESSIBLE TO CREW AND PASSENGERS. EMERGENCY INSTRUCTIONS MUST BE REVISED AND POSTED.

EACH VESSEL THAT OPERATES ON THE HIGH SEAS MUST HAVE ON BOARD A 406 MHz EPIRB INSTALLED TO AUTOMATICALLY FLOAT FREE AND ACTIVATE. EPIRP HYDROSTATIC RELEASE UNIT IS EXPIRED.

ALL MACHINERY AND EQUIPMENT MUST MAINTAINED IN SUCH A MANNER AS TO AFFORD ADEQUATE PROTECTION FROM MACHINERY FAILURE. GENERATOR ENGINE RADIATOR WAS CLOGGED.

LB06: Provide logs of monthly EPIRB testing and lifesaving inspections.

LB07: Seal hole located in bulkhead between the #3 void space and the engine room.

LB01: When the vessel is operating in a no discharge area the MSD system must be secured in accordance with one of the ways described in 33CFR159.7(c).

LB02: Vessel has an oil fired package boiler onboard that is required to be inspected in accordance with Subchapter F. This discrepancy was noted in 2014 when the system was tagged out of service and the crew was informed it would need to be inspected prior to any further use. The boiler was being used and has not been inspected and is not annotated on the COI at this time. Vessel's master was informed that he needs to tag out the boiler until proper tests and inspections can be conducted.

Vessel must provide saftey orientation public announcement to passengers before getting underway.

Life flooat paddles shall be lashed on the sides of the life floats to which they belong.
