A marine employer shall comply with the chemical testing requirements found in 46 CFR Subpart 4.06. During a DAPI audit, the following requirements were found to be deficient: - security of records maintained. - negative tests are kept for one year. - pre-employment documentation is kept for the entire employment period. - supervisor has received 1-hour EAP training. - employer has substance abuse professional (SAP) name and contact information.

Immediately after the addressing of resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a marine casualty shall notify the nearest Sector Office, Marine Inspection Office, or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting of loss of main propulsion or primary steering. The owner, master, agent, or person in charge shall, within five days, file a written report of any marine casualty. This written report is in addition to the immediate notice required by 185.202. This written report must be delivered to a Coast Guard Sector Office, or Marine Inspection Office. It must be provided on Form CG-2692. On or about 07NOV2025, the vessel experienced an engine casualty adversely affecting its fitness for service while operating with passengers. Vessel representatives did not report the casualty and a completed CG-2692 has not been provided at this time.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have a means to reset devices used to automatically shut down ventilation systems and engines the following equipment at the operating station. The operating station engine override device for the engine room Fireboy system was found to be inoperable.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. The fuel shutoff valve was unable to fully close using the pull cable at the operating station.

Each life jacket carried on board the vessel must be marked in accordance with 46 CFR 185.604. Name of vessel needs to be put on 06 life jackets and lifebuoy.

Each life ring buoy on a vessel must be approved in accordance with 160.050 in subchapter Q of this chapter. Life ring is required to have vessel name written on it.

A space containing propulsion machinery is required to be equipped with a fixed gas extinguishing system. Fireboy over ride switch is inoperable.

An adult life jacket must be provided for each person carried on board a vessel. Adult life jackets have been found to be in a dilapidated state and not fit for service. Until replaced vessel is only allowed 19 passengers on board.

Gauges to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines. Repair/ replace gauges.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request.

Upon completion of repairs to the main engine, an underway sea trial to be conducted to the satisfaction of the attending Coast Guard Inspector.

Fuel shut off values, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. Fuel value must be exercised and be in working order.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing actions to be taken in the event of fire, heavy weather, or man over board conditions.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request.

Sound producing device was broken.

Battery was not covered.

The required water light shall be attached to the body of the life buoy with a corrosion resistant quick release clip.

03 PFDs were condemmned. 02 PFDs ripped when the straps were pulled and 01 PFD was saturated.

The installed fire buoy is a manual setup and vessel rep did not know how to test it in the prescence of the MI. Recommended that when the fire inspector services equipment in next two weeks that he ask and demonstrate to MI. On 20 AUG 2019 vsl rep sent an exemption request to the COTP.

Faded vessel name.

Entire kit water damaged.

Incorrect vessel name on four (4) PFD's

Reflective tape no longer effective as designed on one (1) PFD.
