Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed two of the type "A" relief valves in the CO2 room with the safety pins in place. While PSCO continued with exam chief officer instructed cadet to remove the pins resulting in the CO2 lines for the scrubber system to be energized. 74 SOLAS(20) II-2/14.2.1.2

Means shall be provided to control leaks of flammable liquids. PSCO observed active fuel leaks on several connection points on the #2, #3, and #4 main diesel generators. Additionally, there was oil accumulation under #3 and #4 main diesel generator turbos. 74 SOLAS(97)II-2/4.1.1

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in normal manner. PSCO observed covers missing on switch board buttons in the vicinity of the #3 main diesel generator. Additionally, PSCO observed cracked light covers and exposed wires on several connections throughout the engine room. 74 SOLAS(97) II-I/45.1.3

After any survey of the ship has been completed, no change shall be made in the structural arrangement, machinery, equipment and other items covered by the survey, without the sanctions of the administration. PSCO observed modifications made by the crew to the main diesel generator lube oil centrifugal pump arrangement.

The arrangements for the storage, distribution, and utilization of other flammable oils… shall be such as to ensure the safety of the ship and persons on board. PSCO witnessed excessive oil accumulation near main engine cylinders, auxiliary generators, oil purifying space, and HFO settling tank. Throughout the engine room, oil-soaked lagging was present and active leaks were observed on the auxiliary generators. Lagging over critical machinery components has been painted over, eliminating the possibility to identify if and components are leaking. In addition, numerous oil-soaked rags were removed from concealed locations. The amount of standing oil combined with the presence of excessive combustible materials in the engine room poses an increased probability of ignition within the engine room, thus endangering the crew, the port, and the environment. 74SOLAS(97 CONS) II-2/15.4

Where "A" class divisions are penetrated for the passage of electric cables… arrangements shall be made to ensure that the fire resistance is not impaired.. Penetration for shore tie between tunnel and main engine room found unsealed in vicinity of improperly stowed fuel samples. Penetrations through superstructure identified by FCP as "A" class boundaries found penetrated and not properly sealed near main deck entrance into house. 74SOLAS(97 CONS) II-2/44.1

For determining the appropriate fire integrity standards to be applied to divisions between adjacent spaces, such spaces are classified according to their fire risk as shown in categories (1) to (11) below. PSCOs discovered MARPOL fuel samples stored in ship's tunnel in unapproved storage container. The samples are required to be stored in category (9) spaces with "A" class boundaries. 74SOLAS(97 CONS) II-2/44.2.2

The fire resistance of doors shall, as far as practicable, be equivalent to that of the division in which they are fitted. "A" Class self-closing door between ECR and engine room missing locking mechanism and door handle creating openings in the door. Additionally, the door is warped and does not close on its own. 74SOLAS(97 CONS) II-2/47.1

Lighting in CO2 space was inoperable. Crew is unable to see emergency instructions without use of external light source. SOLAS II-I/43.2.2.3

Every ship… shall carry a garbage management plan, which the crew shall follow. PSCOs discovered cardboard stored in waste receptacle designated for oil-soaked rags in vicinity of ship's incinerator. MARPOL V/10.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Areas where lifeboat and rescue boat rest against the davit are causing dents and scratches due to chocks being made of wood. 74SOLAS(97 CONS) III/19.2

Machinery… associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended… and the working conditions to which it will be subjected. Main engine cooling water line was observed to be leaking. 74SOLAS(97 CONS) II-I/26.1

Satisfactory means in the form of guardrails… shall be provided for the protection of the crew in getting to and from…all other parts used in the necessary work of the ships. PSCOs discovered excessive wastage of railing in multiple areas on starboard side. ILLC (66) Annex I /25.4

The International Load Line Certificate… shall not be delivered to the ship until the officer or surveyor… has certified that the marks are correctly and permanently indicated on the ship's sides. After the survey of the ship, an additional load line marking was affixed to the side of the ship - causing the ship not to correspond with the issued ILLC. ILLC (66) Annex I / 9

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship. PSCO observed fuel soaked rags under and around generators 1, 2, 3, and 4. PSCO observed oil soaked lagging on each generator.

In a ship in whcih oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship. PSCO observed oil soaked lagging and fuel soaked rags in the oil purifier room.

Hydraulic power operated steering gear shall be provided with the following: A fixed storage tank having sufficient capacity to recharge at least one power actuating system. PSCO observed insufficient hydraulic oil in the reserve tank to fill one power actuating system. 74SOLAS(97)II-1/29.12.3

Main and emergency switchboards shall be so arranged as to give easy access as may be needed. PSCO observed stowage of spare parts behind switchboards in ECR and steering gear room. 74SOLAS(97)II-V 45.2

In a ship in which oil fuel is used, the arrangements for the storage,distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship. PSCO observed saturated fuel lines on the #4 A/E. 74SOLAS(97)II-2 15.2

In a ship in which oil fuel is used, the arrangements for the storage,distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship. PSCO observed leaks and non-approved containments, and on the A/E and MDE fuel oil heater and transfer pumps. 74SOLAS(97)II-2 15.2

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. PSCO observed major boiler water leak from level controller. 74SOLAS(97)II-1/33.1

The company and the ship shall comply with the requirements of the ISM code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. PSCO observed collective operational deficiencies indicating a lack of ISM effectiveness, an internal safety audit is required within 90 days. 74SOLAS(20)IX/3.1 ISM code 10.1

A self-contained emergency source of electrical power shall be provided. PSCO observed the emergancy generator run for 30 seconds, then shut off. 74SOLAS(97)II-1/43.1

Internal combustion engines shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. Oil mist detector #1 was displaying a fault from the solenoid. 74 SOLAS (04) II-1/47.2
