Rudder angle indicator shall determine and display rudder angle from the conning position. PSCO observed starboard bridge wing rudder angle indicator to be reading 4 degree or greater deviation from the true position. 74 SOLAS(20) V/19.2.5.4

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed rescue boat unable to start. 4th engineer and 2nd engineer troubleshot rescue boat and replaced fuel filter and spark plugs. The rescue boat was restored to full operational condition and was ready to start.

THE APPLIANCES SHALL BE KEPT CLEAN, PROPERLY MAINTAINED AND STOWED AND SHALL BE REGULARLY INSPECTED TO ENSURE THAT THEY ARE SAFE TO USE. LINE ON PILOT LADDER WAS FOUND TO HAE PARTED AND LADDER IS NOT IN A SERVICEABLE CONDITION.

An alarm system shall be provided indicating any fault requiring attention and shall be capable of sounding an audible alarm and indicate visually. PSCO observed audible and visual alarm indicators in the engine room were not working, 01 forward of the main engine and 01 at the main engine local control station.
