Lifeboat and rescue boat release gear must be operationally tested under a load of 1.1 times the total mass of the lifeboat when fully loaded with its full complement of persons and equipment whenever overhauled. The vessel reported significant damage to the port side rescue boat wire. Immediate repairs shall be made to the lifeboat wire. A weight test shall then be conducted IAW 46 CFR 199.190 in the presence of a Marine Inspector prior to the vessel departing Ketchikan. The port side rescue boat shall not be used other than in an emergency. 46 CFR 199.190(h)(5)

Means of escape: Escape routes shall be maintained in a safe condition, clear of obstacles: As currently designed the emergency escape route, leading from the auxiliary machinery space via the enclosed vertical trunk ladder, is obstructed by a large rising stem salt water cooling supply valve. The valve handle and stem protrude into the center of the walkway at a height of Apx 1ft. This protrusion not only imposes a tripping hazard but also an impaling hazard to crewmembers. Rectify hazard to the satisfaction of attending CG Marine inspector prior to next credit dry dock. SOLAS 14 II-2/13-1.2

Automatic propulsion systems, automated electric power management systems, and all associated subsystems and equipment must be capable of meeting load demands from standby to full system rated load, under steady state and maneuvering conditions, without need for manual adjustment or manipulation. The vessel reported the #3 SSDG shutdown and the ship lost power while moored in Ketchikan. Provide repair plan and prove proper operation to the satisfaction of the attending MI prior to completion of next scheduled drydock. Should additional similar failures occur, the Master must immediately notify the cognizant Officer in Charge of Marine Inspections. 46 CFR 62.35-1(b)

Repair reported issue of #2 SSDG generator excitation failure. Provide technician report and prove proper operation to the satisfaction of the attending Marine Inspector prior to completion of next scheduled dry-dock.

#3 generator cooling water pump died, and left the #3 generator inoperable. Fix #3 generator cooling water pump.

Each lifesaving appliance must be in working order and ready for immediate use. Make immediate repairs as necessary and in accordance with the Captain and [name withheld] report of both Lifeboats.

Automatic propulsion systems, automated electric power management systems, and all associated subsystems and equipment must be capable of meeting load demands from standby to full system rated load, under steady state and maneuvering conditions, without need for manual adjustment or manipulation. The STBD M/E would not meet bridge commands under combinator mode. Vessel shall operate the STBD M/E in full RPM mode during all restricted maneuvering operations. Should additional similar failures occur, the Master must immediately notify the cognizant Officer in Charge of Marine Inspections. Provide a tech report and demonstrate proper operation of the STBD M/E from the bridge to the satisfaction of the attending MI. 46 CFR 62.35-1(b)

Automatic propulsion systems, automated electric power management systems, and all associated subsystems and equipment must be capable of meeting load demands from standby to full system rated load, under steady state and maneuvering conditions, without need for manual adjustment or manipulation. The STBD controllable pitch propeller failed and went full astern while maneuvering to the dock. Vessel shall ensure the standby electric hydraulic pump is on during all restricted maneuvering operations. The vessel shall also provide extra manning to monitor the CPP system while the vessel is in restricted maneuvering. Should additional similar failures occur, the Master must immediately notify the cognizant Officer in Charge of Marine Inspections. 46 CFR 62.35-1(b)

Machinery vital to the main propulsion system must be provided in duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. The vessel reported the STBD M/E unable to maintain steady RPM and then shutting down during transit. Demonstrate proper operation of the STBD M/E. 46 CFR 58.01-35

A two-way communications means between the elevator car and a location staffed by authorized personnel shall be provided. Marine Inspector found the two-way communication in the passenger elevator is not operational. Repair communication system and provide local OCMI with mitigation strategy as to how equivalent level of safety will be achieved until system is repaired.

Identified wasted steel on the Officer’s Mess deck above passenger cabin. Provide gauging report of the deck and crop and renew to good steel in accordance with the UT results, and to the satisfaction of attending CG Marine Inspector. 46 CFR 72.01-15

Safety vavles on the waste heate boilers must be stamped in accordance with section I of ASME Boiler and Pressure Vessel Code and stamped with (V) not (UV). Replace safety valves In accordance with 46 CFR 52.01-120, prior to using the waste heat boilers.

Vessel's starboard controllable pitch propeller has a low pressure alarm on the manual pump. Prove proper operation of the controllable pitch propeller system to the satisfaction of the attending Marine Inspector.

Temporary lighting is required in passenger & crew quarters, other spaces in accordance with 46 CFR 112.15-1. Identified 83 temporary lighting not working in crew & passenger cabins. Provide 02 additional watchman IAW 46 CFR 78.30-15 (and flashlights for occupied cabins), to assist passengers/crew in an emergency, until the lights have been repaired. Demonstrate to attending CG Marine Inspector. 74 SOLA Amended II-1/41.6 & 46 CFR 112.15-1

The test procedure (PSTP) for 9.6 Power Failure Alarms are not written properly re-write and demonstrate to attending CG Marine Inspector. (30 days)

A steam condensate system of adequate cooling capacity shall be installed to ensure that the installed waste heat boilers can be maintained within the parameters established by the manufacturer at all engine loads.

Provide Kongsberg technicians report after his inspection of the reduction gear system.

The main steering gear must be operable from the pilot house by mechanical, hydraulic, electrical, or other means . This gear and components must give full follow up control of the rudder. Currently the the STBD steering gear control system is not operating as designed and system is not correctly answering commands from pilot house. Vessel must correct problem and demonstrate proper operation of steering system to the satisfaction of attending CG Marine Inspector.

To assist in casualty investigations, ships, when engaged on international voyages, subject to the provisions of reg 1.4, shall be fitted with a Voyage Data Recorder (VDR). The VDR installed is not operating properly. Make permanent repairs and provide a technician report to MSD Ketchikan.

The vessels structure in the number 8 starboard ballast water tank is no longer in compliance with the ABS SVR. Vessel conducted weld repairs to two holes on the tank, one 1/2 inch, one 1/8 inch, where water was leaking when the tank was full. Temporary repairs in effect until next scheduled dry dock for month of October. 46 CFR 72.01-15/ABS SVR Part 3

Primary automatic control systems for propulsion is required to be stable over the entire range of normal operation, without the need for manual adjustment or manipulation. As reported by Master, Matanuska port propulsion control system disconnects randomly. 46 CFR 62.35-1(b)

On board electronic stability computers may be used as an adjunct to the required booklet, but the required booklet must contain all necessary information to allow for the evaluation of the stability of any intact condition that can be evaluated by use of the computer. The computer program was not updated appropriately after the last vessel incline. Update the program or use the T&S book and calculate everything by hand. 46 CFR 170.110 (f)

Provide the port reduction gear lubrication system for inspection to verify successfully, measures have been taken to eliminate oil/water emulsifcation caused by the heat exchange failure.

06. The fire detection system is showing trouble in the galley stores dry sprinkler system (Supervisory L3-005 Low Pressure E126P). Make permanent repairs to the fire detection system and provided technician report. 46 CFR 76.05

Make permanent repairs to the Anchor Windlass VFD and provide technicians report.

The exhaust gas temperatures produced by the propulsion engines appear to be higher than the design temperatures indicated on drawings for this waste heat boiler installation. Provide documentation that the source of heat to your waste heat boilers does not exceed the maximum design temperature of the boiler components exposed to the exhaust gas. This temperature must be verified by testing or by the manufacturer of the engine or other equipment producing the exhaust gas. 46 CFR 52.25-20

Make temporary repair to the stbd side cafeteria exit door, muster station C (kick plate). In addition, replace the stbd side sprinkler zones door, the old video game room door and the cafeteria door at the next dry-docking of the vessel with approved doors.

Make the Auxiliary boiler mountings ready for inspection in accordance with 46 CFR 61.05-10. Last date inspected was 19 June 2009 the interval permits 10 years, an inspection is required.

Make the old video game room door weather-tight. If the door is considered an emergency escape in accordance with vessel plans make the door operable and accessible to passengers.

The call back emergency comunication system is not operable at all stations. Repair the installed call back system as required by SOLAS 74 (2017 Amend.) III/6.4 and 46 CFR 113.30-3

Vessel must provide an approved Periodic Safety Test Procedures (PSTP) for use on board the vessel. The final approval of the PSTP will be conducted by the Coast Guard's MSC upon the receipt of all verified test procedures, as submitted per the contract between Vigor Shipyard and Alaska Marine Highway System (AMHS).

Submit repair proposal for items identified during hull walk including holes in the vicinity of the bottom plating around the rudders, pitting on rudders, localized pitting on port/starboard bottom shell plating as detailed in the call out sheet, bow thruster pitting, and gauging on the side shell plating on the boot top forward of the car deck doors both port and starboard.
