Covered metal trash containers shall be provided in lounge areas and the spaces shall be maintained to minimize fire and safety hazards. MI observed plastic trash containers throughout the vessel.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. MI observed emergency radio instructions that were designed for a commercial fishing vessel and did not match this specific vessel.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. MI observed that child life jackets were separated by a piece of plywood that was not permanently secured, allowing adult and child life jackets to mix.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. MI observed that crew was not testing the EPIRB monthly and were not logging the test.

Windows and other openings at the operating station must be of sufficient size and properly located to provide an adequate view for safe navigation in all operating conditions. 46 CFR 177.1030 (a)

Vessels operated beyond the 20 nautical mile limitation specific in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter. Vessel has MF radio installation without DSC frequency transmitter, and a non-GMDSS satellite phone. Rectify to the satisfaction of the attending MI. - 47CFR80.905(a)(2)

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. Vessels bilge suction was inoperable in several compartments. Vessel was able to clear blockage from manifold and regain suction. 46 CFR 182.25.5

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges. Vessel mast light was inoperable. Vessel is restricted to operate during daylight and clear viability. Rectify to the satisfaction of MI. 33 CFR 83.22

Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Vessel's bow life ring buoy was missing its identification sticker. 46 CFR 180.70(b)(1)

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. All floating waterlights were installed with the incorrect type of battery. 46 CFR 180.70(d)(1)

Two required fixed powered bilge pump shall be self priming and each driven by different sources of power. Vessel has only one fixed power bilge pump installed. 46 CFR 182.25-10 (c)

Each life float must be secured to the vessel by a painter and float free link the strength of the float free link is determined by the combined capacity of the group of life floats. The two forward life floats with the combined capacity of 24 persons had an undersized float free link rated for 11-20 persons. 46 CFR 180.20-1 (a)(5)(iii)

Life preservers for children, when provided, shall be stowed separately. Vessel child size life preservers were stowed in the same location as adult size life preservers. 46 CFR 180.25-10 (c)

Each water light must be of an approved automatic electric type constructed in accordance with 161.010. Usage of non recommended battery may cause the light to sink. Vessels water light had an expired RAY-O-VAC 945 6 volt battery installed instead of a RAY-O-VAC 942 as recommended by manufacturer. 46CFR 180.30 (b)

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. The vessels port main propulsion engine was missing a guard on the front engine pulleys. 46 CFR 177.35-15

All escape hatches shall be marked on both sides using a least 2 inch letters "EMERGENCT EXIT,KEEP CLEAR". Vessels escape hatches were not marked as required by regulation. 46 CFR 185.606

Remote fuel shut off stations shall be marked in at least 1 inch letters indicating purpose of the valve and direction of operation. Vessels remote fuel shut off valves were not marked as required by regulation. 46 CFR 185.608

A visual and audible alarm must be provided at the operating station to indicate a high water level in each unmanned space. The float switch in the space aft of the collision bulkhead was not functioning as designed. 46 CFR 182.530 (c)

Openings for which conductors enter shall be adequately closed. The galley electrical distribution panel had 3 unused stuffing tubes on the bottom that were not sealed. 46 CFR 183.10-20 (f)(1)

All electrical distribution panels shall be constructed to prevent uninsulated parts that could be inadvertently touched by any person. The galley distribution panel is missing a circuit breaker blank spacer. 46 CFR 183.10-10 (a)

All electrical installation shall be to the satisfation of the Officer in Charge, Marine Inspection. Electrical outlet in passenger space on the starboard side above the windows is cracked and unsafe. 46 CFR 183.01-15 (a)

All life floats shall be of approved types (160.027). The life floats installed on the vessel had only two of four permanently attached name plates as required by 46 CFR 160.010-8 as referenced by 46 CFR 160.027-2. 46 CFR 180.10-1

Before getting underway the master of each vessel shall ensure that their is a suitable safety orientation/ public announcement that are provided in a manner that affords all passengers the opportunity to become acquainted with stowage of life preservers, proper method of donning and adjusting the types carried on board, and the location and content of the emergency check off list required by 46 CFR 185.25-5. The vessel safety orientation was not consistent as required by regulation. 46 CFR 185.506

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol testing strips were expired, replace to the satisfaction of the attending Marine Inspector. 46 CFR 4.06-15(a)(1)

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. New cables in engine room were not supported by metal supports and were connected to other cables with plastic tie wraps. Correct to the satisfaction of the attending Marine Inspector. 46 CFR 183.340(b)(4)

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessels with a keel laid date after March 28, 2022, must include plans for the monitoring device(s) within the plan submissions required in 46 CFR 177.202. The Coast Guard will work with the vessel operators to determine a reasonable implementation schedule once the plans are accepted. Vessel has not installed watchmen monitoring device. Install watchmen monitoring device to the satisfaction of the USCG Marine Inspector or remove overnight passenger endorsement from COI. Vessel is restricted from carriage of overnight passengers until rectified.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel does not have an installed interconnected fire detection system. Install interconnected fire detection system to the satisfaction of the USCG Marine inspector or reduced vessel's route to limited coastwise. Vessel is restricted to limited coastwise until rectified. 46 CFR 181.405 (c)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at a constant marine load under such requirements without exceeding its design limitations. Starboard engine is not responding to throttle manipulation.

All vessels of 26' and over shall be fitted with individual bilge lines and bilge suction for each watertight compartment. Collision Bulkhead did not drain the space of water.

Portable fire extinguisher must be installed to the satisfaction of the OCMI, Marine Inspector. Operating station portable fire extinguisher mounting bracket broken.

Remote fuel shutoff stations shall be marked indicating purpose and direction of operation. All three remote fuel shutoff stations do not have markings indicating direction of operation.

Portable watertight containers for distress flares and smoke signals shall be of bright color and marked “DISTRESS SIGNALS”. Container is dull and no markings are legible.
