Hydrostatic release units (HRUs) for both 50 person IBAs expired this month. Prior to carrying passengers, replace HRUs and properly install to satisfaction of a Marine Inspector. 46 CFR 180.430(a)(2)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Retro reflective material on all 03 ring buoys was not approved for marine use. Replace all retro reflective material to satisfaction of a Marine Inspector. 46 CFR 185.604(i)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Emergency escape from the galley to the main deck was frozen in the closed position and could not be opened from the galley. Crew was able to pry open the escape hatch from the main deck. Recommended greasing the knife edge to ease emergency egress from the space. Also recommended monthly maintenance and opening the hatch each trip prior to taking passengers to ensure operability. 46 CFR 185.100

Each vessel that relies on electricity to power The vital systems listed in 46 CFR 182.710, must be arranged so that the loads can be energized from two sources of electricity. Vessel's port generator was inoperable due to a faulty alternator and unable to provide power to the vessel's fire main. Make vessel's port generator in compliance with regulatory requirement to satisfaction of attending Marine Inspector.

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Power ventilation did not shut down when testing pre-engineered fixed gas fire extinguishing sys in STBD E/R. Ensure satisfactory operation of power ventilation shut down to attending Marine Inspector.

Each vessel certificated to operate on an oceans route in cold water must be provided with inflatable buoyant apparatus (IBA) of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. One (50 Person) IBA is not currently on-board. Vessel COI is temporarily reduced to 108 "Total Persons Allowed" until O/O can provide proof of satisfactory primary lifesaving, to attending Marine Inspector, to cover total number of persons allowed onboard as per vessel's COI.

Penetrations of required watertight decks and bulkheads by any rigid plastic pipe are prohibited unless each penetration is accomplished using an acceptable metallic through bulkhead fitting that is welded or otherwise attached to the bulkhead or deck by an accepted method. Valves in "Library" were found wasted. Repair/replace all wasted fixed bilge system valves to satisfaction of attending Marine Inspector. Vessel is only able to continue operation under the conditions that the vessel stays within 3 NM of the mainland of their current operating zone as stated on the vessel's COI, all installed electric bilge pumps are satisfactorily operating, and rounds are made of the spaces and logged on an hourly basis of all spaces where valves are found unsatisfactory. * NOTE AMENDING AM3 - AM3 Penetrations of required watertight decks and bulkheads by any rigid plastic pipe are prohibited unless each penetration is accomplished using an acceptable metallic through bulkhead fitting that is welded or otherwise attached to the bulkhead or deck by an accepted method. Bilge system valves in "Library" (and throughout vessel) were found wasted. Have comprehensive repair proposal submitted to attending Marine Inspector within 14 days of this amended deficiency for review. This repair proposal shall consist of a drawing of the current vessel bilge system configuration noting/marking valves in need of repair/replacement. All wasted fixed bilge system valves that are accessible without drydocking the vessel shall be repaired/replaced to satisfaction of attending Marine Inspector within 30 days. All other valves determined to need access during drydocking will need to be addressed to the satisfaction of the attending Marine Inspector at the next credit drydocking. Vessel is only able to continue operation under the conditions that the vessel stays within 3 NM of the mainland of their current operating zone as stated on the vessel's COI, all installed electric bilge pumps are satisfactorily operating, and rounds are made of the spaces and logged on an hourly basis of all spaces where valves are found unsatisfactory. AMENDED to an Action 705 (To be cleared during next drydocking).

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the inspection of each portable fire extinguisher. 01 2- A portable fire extinguisher was not serviced as per NFPA 10. Provide proof of servicing to attending Marine Inspector.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Emergency lighting along starboard berthing area egress route was not operable. Repair/ replace to satisfaction of attending Marine Inspector.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kit contents expired. Replace required contents of first aid kit with unexpired items.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have Safety Radiotelephony Certificate on-board. Provide proof of satisfactory Safety Radiotelephony Certificate to attending Marine Inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Have written preventative maintenance plan (developed in accordance with NVIC 02-16) developed and presented to attending Marine Inspector.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in §185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel must update emergency procedures as addressed during vessel's annual inspection to address the particular equipment, arrangement, and operation of the vessel.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Battery (low voltage) sensor for port engine faulty. Have sensor repaired/replaced to satisfaction of attending Marine Inspector by submitting report/short e-mail from technician/repair person explaining cause of issue and how it was repaired.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. While in Drydock vessels mast was unstepped for inspection and serviced. Most of vessels sail rigging was also replaced. Due to vessel schedule the attending Marine Inspector was unable to conduct a sea trial under auxilary sail power. Vessel is restricted to MDE operations only, until a satisfactory sea trial is conducted.
