If your vessel is not operated on international voyages and does not meet the conditions in paragraph (d) of this section, it must undergo a drydock and internal structural examination as follows unless it has been approved to undergo an underwater survey (UWILD) per § 176.615 of this part:(1) A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years... Marine Inspector observed that vessel did not undergo required drydock. Vessel is restricted from carriage of passengers until satisfactory drydock examination. 46 CFR 176.600

Provide repair plan for loose framing and failed structure around forward hatch.

Machinery installation must be suitable for its intended use. Provide repair plan for hydraulic lines going from Engine Room to Anchor Windlass that are beyond acceptable corrosion limits. 46CFR182.100

Piping for vital systems must be composed of ferrous materials. Provide repair plan for three sections of bilge piping in the forward are of the engine room with soft patches. 46CFR182.710

Machinery installation must be suitable for its intended use. Provide repair plan for inoperable fuel tank room bilge suction that failed to operate due to failed check valve. 46CFR182.100

A positive shutoff valve must be installed at the hull penetration. Provide overboard valves for both submersible bilge pumps located in the lazarrette. One did not have a valve installed and the other's handle was wasted off. 46CFR182.520(e)(7)

Provisions shall be made for closing all supply and discharge openings for a space protected by a gas system. Provide ventilation closures for engine room. 46CFR182.465(g)

Secure reach rod to handle of collision bulkhead securing valve.

The vessel will have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel did not have emergency instructions. 46CFR185.510(a)

Each life float may be used as long as it is in good and serviceable condition. Life float is wasted and not serviceable. Remove and destroy to satisfaction of Marine Inspector. 46CFR180.15

Replace expired Asprin in medical kit.

Fire Boy engine shutdown and override test, Underway Test and Drills, and fuel tank room bilge suction must be test prior to issuance of COI.

Each life float/buoyant apparatus shall have lifelines and pendants IAW 46CFR160.010. Lifelines and pendants are deterioratied. 46CFR180.175

Ring life buoys shall have a lifeline on at least one and be IAW 46CFR160.150. Lifeline and grab-lines were deteriorated and the life buoy showed signs of damage/cracking. 46CFR180.70

Electrical connections shall be made in an enclosure. Multiple connections were not in enclosures, to include all bilge pumps/high level alarms. 46CFR183.340(g)

Vessel must create a company drug and alcohol policy and keep on board with other paper work.

Vessel must get alcohol testing devices and keep on board.

Vessel needs new coast pilot, light list,and corrected and up to date charts of the area of operations.

Vessel needs an all round white light to meet 33CFR84 regulations. (2 mile visability)

Provide first aid kit IAW 184.710.

Provide Random Drug Testing results.

Two kapok life jackets failed. Replaced on the spot.

Paint aluminum lifefloat w/ highly visible (orange) paint.

Replace 100' painter for lifefloat.

Replace 03 rejected lifejackets. Pax count reduced to 48 until completed.

Address weep in speed seam in both the fwd rake & void #2 on the stbd side. Weep is 3 drops / minute.

Address leak btwn fwd (crash) bulkhead and void #2. Leak is 0.5 cup / minute approximately.

Replace retroreflective tape on 20 of Model # 601 (adult) and 04 of Model # 603 (child) life jackets. LEGACY COMPONENT DESC: Marking

PROVE PROPER OPERATION OF THE FIXED FIRE FIGHTING SHUT DOWN FOR GENERATOR.

Vessel is not to be operated with passengers for hire after March 11, 1999,until the above requirements have been cleared to the satisfaction of an attending U.S. Coast Guard Marine Inspector.

Submit plans for review and approval for the installation of a Fixed Fire Fighting System in the machinery spaces in accordance with 46 CFR 181.115(b) and 46 CFR 181.400

Install a Fixed Fire Fighting System in machinery spaces in accordance withU.S. Coast Guard approved plans.

Make vessel available to CG MI while out of the water to inspect #3 void, stbd side planking IWO aft 5 vertical frames and chine. Slight weepage noted below butt blocks in this area. Planks appeared sound with no workingnoted.

Seal leaking port engine exhaust flange where it penetrates the transom. Item to be hose tested or cleared while waterborne. Major leakage noted when vessel was backed down. No sail item.

Tighten rudder packing stbd side.

REMOVE THE DEBRIS AND WATER FROM COMPARTMENT NUMBER 2. DRY OUT WITH MECHANICAL HEATER, APPLY PRESERVATIVE.

RENEW 30X30 MESH SCREEN ON FORWARD STARBOARD FUEL AND OIL VENT.

REPAIR OR REPLACE HANGER BRACKET ON THE AFT EXHAUST PIPE.

REMOVE OLD MARINE SANITATION DEVICE SYSTEM.

RENEW NUMBERS 1 AND 5 BILGE SUCTION VALVES.

SEAL BULKHEAD PENETRATIONS IN COMPARTMENT NUMBER 2.

REMOVE ALL DEAD ENDED WIRING THROUGHOUT THE VESSEL.

RENEW PORT AND STARBOARD FUEL AND OIL HOSES DUE TO ROT.

INSTALL STUFFING TUBE ON ELECTRICAL CABLE GOING INTO GENERATOR BOX.

REMOVE ROTTED WOOD FROM STARBOARD SIDE FRAMES 1 THRU 7 AND PRESERVE.

PROVE PROPER SEALING OF VALVE DISCS ON THE SEA VALVES.

REMOVE ROTTED DECK CLAMPS STARBOARD SIDE FRAMES 1 THRU 7, RENEW AS ORIGINAL

RENEW FORWARD DECK BEAM STARBOARD SIDE WITH STAINLESS OR GALVANIZED PLATES IN WAY OF ROT.

REMOVE OLD STEERING SYSTEM.

MOUNT ELECTRICAL BLOCK HEATER BOX AND REEFER BOX TO BULKHEAD.

REMOVE SPRING CLIPS FROM BATTERIES.

CLEAN BOTH MAIN ENGINES BLOWER BREATHER INTAKES.

PROVIDE PLANS FOR FOR FIXED FIRE FIGHTING SYSTEM, INSTALLATION MUST BE COMPLETED PRIOR TO 10 MAR 1999.

PROVIDE PROOF OF ANNUAL FIRE EXTINGUISHER SERVICING.

DOUBLE CLAMP ALL RAW WATER AND EXHAUST HOSES.

RELABEL FUEL AND OIL SHUTOFF VALVES.

RESTENCIL BUOYANT APPARATUS WITH NUMBER OF PEOPLE, AND THE VESSELS NAME.

PROVIDE WATERLIGHT AND BRACKET FOR BUOYANT APPARATUS.

RENEW WORM SHOE AS MARKED BY ATTENDING MARINE INSPECTOR.

DOUBLE CLAMP 02 OVBD DISCHARGE HOSES LOCATED OUTBOARD OF THE PORT MDE ON THE PORT SIDE. AND 01 OVBD DISCHARGE HOSE OUTBOARD OF THE STBD MDE.

REMOVE TEMPORARY ALIGATOR CLIP FROM THE FWD CENTERLINE BATTERY AND INSTALL A PERMANENT CONNECTOR IN ITS PLACE.

INSTALL NEW RETRO ON LIFE RING

CLEAN ALL FLUSH DECK SCUTTLE GASKET SURFACES TO MAKE THEM WEATHERTIGHT.

LABE BILGE MANIFOLD VALVES AS TO WHICH COMPARTMENT THEY SERVICE.

LABEL THE CHILD SIZE LIFE JACKET STOWAGE AREA SO IT IS MORE VISIBLE TO PASSENGERS.

PROVIDE A COPY OF 46 CFR SUBCHAPTER T

PROVIDE AN 18 INCH ANCHOR BALL DAYSHAPE IAW COLREGS.

DOUBLE CLAMP ALL BILGE MANIFOLD HOSES IN THE ENGINEROOM AT CENTERLINE FWD.

INSTALL NEW RETRO ON B/A AND RE STENCIL CAPACITY AND NAME OF VESSEL.

PROVIDE GUARD FOR BELT DRIVE FOR BILGE PUMP IN WAY OF PORT MAIN DIESEL ENGINE TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

REPAIR HOLE(S) ON PORT MUFFLER TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

PROVIDE 7 APPROVED ADULT LIFE PRESERVERS AND 4 CHILD LIFE PRESERVERS. VESSEL RESTRICTED TO CARRYING 43 ADULTS AND 2 CHILDREN UNTIL THIS REQUIREMENT IS COMPLETED.

REPLACE OR REPAIR 2 CHILD PFD'S, WHERE TORN OR DETERIORATED.

PROVIDE 2-MILE SIDE RUNNING LIGHTS, WHERE PRESENTLY 1-MILE LIGHTS STAND.

PROVIDE 3-MILE MASTHEAD LIGHT, WHERE PRESENTLY 1-MILE LIGHT STANDS.

PROVIDE 2-MILE STERN LIGHT, WHERE PRESENTLY 1-MILE LIGHT STANDS.

REPLACE OR REPAIR 30 ADULT PFD'S WHERE TORN OR DETERIORATED.

REPAIR OR REPLACE 11 ADULT PFD'S WHERE TORN OR DETERIORATED. VESSEL MAY NOT CARRY NO MORE THAN 40 PERSON UNTIL ABOVE REQUIREMENT HAS BEEN CLEARED.

RENEW STEERING QUADRANT CONNECTING RODS, WHERE DETERIORATED.

CONDUCT AN UNDERWAY EXAM OF VESSEL INTERNAL COMPARTMENTS, NOT CONDUCTED DUETO FOUL WEATHER.

PROPERLY INSTALL DECK HATCH ATTACHMENTS TO PREVENT HATCHES FROM COMING ADRIFT AS PER 46 CFR 178.35(D).
