"The two required fixed power bilge pumps shall be self-priming and shall each be driven by different sources of power." PTO pump removed from port engine, and vessel found with only one electric pump (fire pump) installed. Re-install PTO pump. 46 CFR 182.25-10(c) (OLD T)

"The person in charge shall conduct such drills and give such instructions as are necessary to insure that all hands are familiar with their duties." Vessel did not have any fire, man overboard, or abandon ship drill recorded. Conduct satisfactory drills. 46 CFR 185.25-10(a) (OLD T)

"Means shall be provided for closing all other openings to the space protected from outside such space. In this respect, relatively tight doors, shutters, or dampers shall be provided for openings in the lower portion of the space. The construction shall be such that openings in the upper portion of the space can be closed off either by permanently installed means or by the use of canvas or other material which is normally carried by the vessel." Engine room fire damper modified to allow electrical cable run to pass through while in closed position. Submit repair plan proposal. 46 CFR 193.15-35(c) (OLD T)

"A power-driven fire pump shall be installed on each mechanically propelled "S" vessel which is a ferry vessel or which is permitted to carry more than 49 passengers, and on each mechanically propelled "L" vessel." Power driven fire pump leaks from shaft packing. Repair or replace packing. 46 CFR 181.10-1 (OLD T)

"At all inspections special attention shall be paid to insure that no fire hazards exist and that guards or protective devices are provided in all hazardous places." Protective lagging on aft main deck diesel generator was missing. Replace lagging. 46 CFR 176.25-10(b) (OLD T)

"Batteries... shall be effectively screened by a cage or similar structure to minimize the danger of accidental spark through dropping a metal object across terminals." Battery box near aft main deck generator was found loose. Secure box. 46 CFR 183.05-20(a)

"Suitable safety guards are installed in way of rotating machinery, hazardous cable runs and at other appropriate locations." Flywheel guard missing on aft generator. Replace guard. 46 CFR 189.35-13(2)

"The material specification shall show that the rigid nonmetallic material possesses characteristics adequate for its intended service and environment and shall be approved for use by the Officer in Charge, Marine Inspection." MI observed both ends of fuel hose connecting the aft generator Racor filter and head tank are connected with hose clamps. Repair prior to use. 46 CFR 182.40-1(4) (OLD T)

"At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition and fit for the service for which they are intended." Air intake gasket on aft main deck generator found wasted. Repair gasket prior to use. 46 CFR 176.25-10(a) (OLD T)

"It shall be the responsibility of the master to assure himself that all exposed hatches and other openings in the hull of his vessel are closed, made properly watertight by the use of tarpaulins, gaskets or similar devices..." Hatches for forward void spaces found with gaskets worn and painted. Repair or replace gaskets. 46 CFR 196.15-20(a) (OLD T)

"Penetrations and openings in watertight bulkheads must- (1) Be kept as high and as far inboard as practicable; and (2) Have means to make them watertight." MI observed hole in forward lazarette bulkhead adjacent to engine room. Submit repair proposal within 7 days. 46 CFR 171.114(a)

For semiportable and fixed gas fire extinguishing systems, the inspections and tests required by Table 115.810(b), in addition to the tests required by §§ 147.60 and 147.65 in subchapter N of this chapter. The owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Attending marine inspector did not see any documents showing servicing on fixed fire fighting equipment in the engine room and the galley. 46cfr 115.810

Exhaust gas must not leak from the piping or any connections..... Attending marine inspectors witnessed exhaust leak coming from the port and starboard main engines. 46 cfr 119.430

All piping, valves, and fittings shall be in accordance with good marine practice and suitable for the purpose intended..... Attending marine inspector identified a leak in the piping system between the power take off and the fire main. 46 cfr 181.15-5 OLD T

Battery chargers must have an ammeter connected in the charging circuit. Attending Marine Inspector did not see an ammeter in the battery circuit forward of generator #1 46 cfr 120.350

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Attending marine inspector was informed by captain during bridge examination that the rudder angle indicator was not operational 46 cfr 182.30-1 OLD T

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules... Attending marine inspector identified that the vessel's anchor lights were not operational. 46 cfr 120.420

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited ...the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Attending marine inspector found the valves of the sewage tank to be in an open position without securing devices. Rectify deficiency within 7 days. 33 cfr 159.7

The following records must be kept:Drills and exercises. For each drill or exercise, the date held, description of drill or exercise, list of participants; and any best practices or lessons learned which may improve the Vessel Security Plan (VSP)... Attending marine inspector identified that the quarterly drill and annual exercises were not conducted and logged. 33 cfr 104.235

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural exam at least once every two years... Vessel has lapsed on required hull inspection date. Receive Credit DDX/ISE prior to carrying passengers.

Vessel is required to conduct drills every 3 months. Vessel is required to conduct security exercise once each calendar year. 46CFR104.230

Vessel is required to document crew training and the following information shall be included - date of the training and general description . 46CFR122.420

Vessel is required to log or documents when they conducted abandon ship and man overboard drills. Each member of the crew shall participate in at least one abandon ship drill each month. 46CFR122.520

Vessel is required to conduct at least one fire drill each month and it shall be logged or otherwise documented. 46CFR122.524

Fire extinguishing systems must be maintained IAW table 176.810 (b). Annual service not performed on fixed APC and CO2 firefighting systems. Have service performed to installed FF systems. 46CFR176.810 (b) (2)
