The Company and the ship shall comply with the requirements of the International Safety Management Code. In meeting these requirements, the company should ensure that: any non-conformity is reported; appropriate corrective action is taken; and records of these activities are maintained. PSCO observed multiple SMS policy violations following the discovery of 04 inoperable sea chest valves on 18MAY2020:
1)As per the vessel's SMS, the Master is required to report to port authorities, vessel management and local agents any damage or defect that affects a vessel's seaworthiness. No notifications to the U.S. Coast Guard, vessel company or local agent were made regarding inoperable valves prior to arrival.
2) Per the vessel's SMS, the Chief Engineer is responsible for the efficient and reliable operation of all machinery equipment and installations, including notification to the company of any abnormalities. Chief Engineer did not document any issues related to the sea chest or its associated valves and made no notifications to the vessel's Master or company. Further, no corrective action was taken or documented. Based on objective evidence, PSCO recommends full ISM external audit. 74 SOLAS (14) IX/3.1 ISM Code 10.2
