All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI found fuel leak on the port MDE. Fix leak and notify local OCMI. 46 CFR 182.100

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's port fuel shutoff valve broke during test. Vessel has repaired fuel shutoff but still needs to be tested which is scheduled for 16OCT25. 46 CFR 182.455(b)(4)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. MI found electrical outlet missing the face cover in the engine room. Replace face cover and notify local OCMI. 46 CFR 183.200(c)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. MI found through hull valve in the laz uncapped. Cap valve and notify local OCMI. 46 CFR 182.200

Generator belt guard was found to be not fully secured, permanently bolt guard to the generator.

Label life jacket storage to indicate adult or child.

Line on ring buoy was found deteriorated, replace line.

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. The steering rod connecting the Port and Starboard rudder sheared. Conduct approved repairs and notify the OCMI to witness operational test. 46 CFR 183.30-1 (OLD T)

Steering systems must be properly mounted/secured and ready to be tested by USCG Marine Inspectors at each initial/subsequent inspection for certification. Primary steering ram was not properly mounted causing potential steering casualty while system is engaged. Must make necessary repair to the satisfaction of the USCG Marine Inspector.

Light on ring life buoy was inoperable. Vessel is restricted to day time operations until it is repaired.

Vessels Communications Act Safety Radio Certificate expired on June 11, 2023.

All griddles, broilers, and deep fat fryers must be fitted with a grease extraction hood in compliance with 181.425. A grease extraction hood must meet UL 710 or other standard specified by the Commandant. A grease extraction hood must be equipped with a dry or wet chemical fire extinguishing system meeting NFPA 17 or other standard specified by the Commandant, and must be listed by an independent laboratory recognized by the Commandant. Install grease extraction hood equipped with a dry or wet chemical fire extinguishing system to the satisfaction of an attending marine inspector. In accordance with CID policy letter #20, and PFM 1-99 alternative arrangements may be allowed if the vessel maintains and logs a (At a minimum) quarterly cleaning and inspection schedule. The vessel did not have a cleaning and inspection program.

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to the release of the extinguishing agent into the space. Both port and starboard vent fans do not shut down upon release of CO2.

Vital system piping shall be composed of ferrous materials. The suction and discharge pipes connected to the installed bilge pump are PVC.

A visual indicator must be provided at the operating station to indicate when ANY automatic bilge pump is operating. The vessel has numerous electric submersible bilge pumps installed and the indicator panel in wheel house was found inoperable.

Electrical plans were reviewed and approved for the vessel in 2021 by Sector San Diego. Numerous modifications have been made to the engine room panel and wheel house panel without USCC approval or plan review. Submit new plans for review.

Electrical outlet with USB in the charger in the wheel house was found burnt and poses a fire hazard. Replace outlet.

Provide double hose clamps for generator sea strainer connections.

Prove proper operation of watch alarm system. Roving watch alarm was inoperable. The Operator/Captain did not know how to log in to even test it.

Remove dead ended cable starboard side engine room forward, labeled RSW.

Replace starboard main engine raw water hose, hose was found collapsed and kinked. Provide new hose with double hose clamps.

Remove dead ended cable, between main engines, forward, port side.

Provide all deck hatches with chain / retaining device.

Provide permanent means to secure all vent openings for engine compartment. 03 on aft end of house, 04 on raised engine room hatch access.

Remove dead ended cable in area forward of wheel house console. Cable is located by pump, port side, near bilge.

Provide junction box for electrical connections found in wheel house counsel. Cable splice with wire nuts were found on the port / upper side.

Prove proper operation of public address system.

Provide electrical outlet with cover, located forward end of bunk space / port side.

Properly mount automatic heat detector for CO2 system.

Provide forward escape hatch with proper labeling, 2" letters "EMERGENCY EXIT, KEEP CLEAR"

Electrical power for the anchor windlass was found with taped splice in the cable in the forward void. Provide electrical connections with a proper junction box.

Complete all items from CG-835 issued on 8/1/23. Open defs are #6, #9, and #16.

Starboard marine gear experiencing intermittent clutch failures. Identify source of failure, repair and provide operational test with CG oversight.

Portable and installed fire equipment was found with expired fire servicing records. Provide all portable and installed fire extinguishing equipment with annual servicing.

Generator belts and pulleys were found open and without a guard. Provide engine with proper guard to prevent personnel injury.

Generator salt water cooling piping was found with PVC fittings and non-approved flexible hose. Provide generator vital system piping with approved fittings, hoses, and clamping.

Starboard reduction gear foundation mounting bolts were found to missing and never re-installed after recent gear work. Provide reduction gear with adequate fastening to engine bed frames.

Starboard reduction gear lube oil cooling hose (type SAE J-1942) was found with a flared copper fitting. Provide vital system with proper SAE J-1475 fitting.

Generator fuel lines were found with replacement lines installed and without proper support brackets. Install fuel line support clamps in accordance with engine manufacture.

Starboard reduction gear cooler was found unmounted and unsecured. Properly mount red gear cooler.

Generator sea strainer was found unmounted and unsecured. Properly mount sea strainer.

Numerous electrical panels were found with missing breakers and unidentified circuits. Provide labeling for all circuits and properly enclose electrical panels.

Battery disconnect switch between main engines was found unmounted. Properly secure/mount switch.

Bilge pump connection terminal between main engines was found unmounted. Properly secure/mount terminal.

Port main engine salt water cooling hose was found chaffing against engine mount. Prove hose is still serviceable and provide chafe protection.

Machinery start batteries were found unsecured and without battery box covers. Properly mount and secure covers.

Multiple dead ended cables were found in the engine room. Remove dead ended cables or terminate inside a junction box.

Saltwater pump / motor electrical junction box was found unsecured and not watertight. Properly secure electrical junction box.

Multiple modular smoke detectors were found uninstalled and/or disabled. Re-install all smoke detectors and prove proper operation.

Vessel owner or operator must comply with lifesaving arrangements located in part 180 of this chapter. A rescue boat or suitable rescue arrangement must be provided to the satisfaction of the [name withheld] to the OCMI that the ladder used during MOB could successfully allow a crew member to recover an unconscious passenger or replace the lifesaving device.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens of 30x30 corrosion resistant mesh or flame arresters.

Provide current FCC station license for marine radio

Plans for the installation and use of monitoring devices to ensure the wakefulness of the watchmen were due to the OCMI 90 days after publication of the interim rule published December 27, 2021. Plans must be submitted to the OCMI.

Prove proper operation of high water alarm visual indicator for steering and machinery space. Both visual indicators illuminate at station #5 (steering space) on the bridge, machinery space should have a visual indicator at station #4.

Mount fixed bilge pump to pedestal to prevent shifting. 46CFR183.320

Remove PVC and non-rigid hose from bilge system. Install rigid non-metallic hoses and metallic connections (check valves/stop valves/unions) along with double hose clamps throughout entire bilge system. 46CFR182.40-1 and 46CFR182.40-5 (Old-T)

Install valve handle on port fuel tank, forward connection shutoff valve.

Install battery box covers on port side machinery space.

Must show enrollment into Drug and Alcohol Prevention Program for all crew members.

Provide 07 USCG approved life jackets. Passenger count reduced by 07 until deficiency is rectified.

Install corrosion resistant clip for Ring Life Buoy water light.

Secure oxygen cylinder located next to life raft.

Must get underway with CG Marine Inspectors present and conduct drills.

Each hatch used as an emergency means of escape must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening must be obvious, rapid and of adequate strength. Emergency exit from below deck found sealed shut by marine sealant. Hatch opening internal T-handle found extremely difficult to turn by one person. Remove all sealant, conduct maintenance, hose test hatch seal and mark T-handle opening direction of turn.

Repairs or alterations to the vessel equipment (electrical wiring) must not be made without the approval of the cognizant OCMI, except during an emergency. During dry dock examination, at least 6 alterations to electrical system were found in progress. Owner stated new wiring and outlets were being installed. Vessel has outstanding CG-835V due 14JAN2020 for submission of electrical one-line drawing to OCMI. Provide complete electrical one-line diagram of all A/C electrical circuts (new & existing) for review/approval.

Properly secure fixed gas fire extinguishing bottle. Bottle was found unsupported.

Provide generator salt water piping with ferrous materials. Generator piping was found with PVC piping installed.

Prove proper operation of the starboard main engine's fuel system. Engine was found with fuel soaked absorbent cloth wrapped around the piping and residual fuel settled on the fuel pump assembly.

Prove operations of main engines, generator, bilge alarms, and emergency lighting.

Provide covers to distribution panels in engine space.

Must have all radios inspected by FCC license inspector and and receive current FCC Radio Safety Certificate.

Prove proper operations of radar.

Prove proper operations of all around and mast navigation lights.

Properly mount EPIRB.

Cap all dead end wiring aboard vessel.

Replace bad retro reflective tape and lights on life jackets.

Provide machinery guards over shafts and generator serpentine belt.

Replace piping in thru-hull to transom

Reinstall handrails at entrances on port and starboard side.

Properly install lagging on port main exhaust.

Provide lagging specifications on starboard exhaust.

Change out weld cable on port and starboard batteries.

Properly bond fuel-fill on starboard tank.

Air intake filters must be properly installed on main diesel engines and generator sets.

Vessel has been identified as having various A/C electrical modifications since initial certification. Sector San Diego does not have vessels electrical plans on file or current installations do not match what we have on file. Provide a one-line electrical drawing for all A/C switchboards and distribution panels. Include all circuit loads, conductor sizes, over current protection, and max panel ratings in amps. Due 14Jan2020.

While underway or drifting at sea, every vessel shall at all times maintain a proper look-out by sight and hearing. The designated watchmen required by 46 CFR 185.410 cannot preform both roving patrolling and look-out watch simultaneously. Vessel has been instructed to operate with a look-out and watchman while conducting drift operations. Cleared on the spot.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from passenger and crew accommodation spaces below the main deck. Vessel did not have lighting at forward escape hatch. Install emergency lighting at forward escape hatch. Due 14Nov2019.

Install smoke detector in galley space. Due 14Nov2019.

Install the following: 1. Doors on breaker panels as identified, 2. Cover on breaker in accommodation closet, 3. Remove tape from breaker panels and install proper blanks. Due 14Nov2019.

Relocate outlet adjacent to air conditioning unit in accommodation space such that condensate from the air conditioning unit does not drip into the outlet. Due 14Nov2019.

All escape hatches and emergency exits used as means of escape must be marked on both sides in clearly legible letters that says "EMERGENCY EXIT, KEEP CLEAR". Emergency exit hatch from accommodation space was not marked on the top side on the main deck. Mark emergency escape as directed. Due 14Nov2019.

Install valve handle on head suction valve. Due 14Nov2019.

Replace broken bridge window. Due 14Nov2019.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with §80.59 once every five years. Provide copy of safety certificate. Due 14Nov2019. Ref: 47CFR§80.903

Properly mount steering ram (4 bolts and bracket) to ensure free movement. Replace port side jockey bar mounting pin to fail safe mode (rotate 180 degrees).

Repair exhaust leak on starboard main engine.

Repair 06 defective life jackets (1 light and 5 clips).

Provide second master's credentials.

Provide proof of Certificate of Documentation renewal application.

Provide valid FFC Radio Safety Telephony Certificate.

Replace welding cable on batteries with battery cable.

Prove proper operation of high water alarms (visual and audio).

Install bonding wire at flex hose tank cross connections on port side fuel tank array.

Install blank over motor controller opening.

Install blanks over open circuit breaker panel.

Install lagging on starboard main engine exhaust up to jacket water inlet.

Install wire in panel board in engine room (aft bulkhead).

Install gasket material around ventilation covers.

Mount sea strainers with bracket.

Mount compressed gas cylinder to permanent structure.

Provide waste management plan that is specific to the vessel.

Vessel is overdue for annual inspection. Complete satisfactory annual inspection with USCG Marine Inspector.

Provide FCC Marine Radio Operator Permits for licensed crew members.

Prove proper operational on forward watertight hatch and captive chain

Install cover on pilothouse 110 panel

Install cover on crew berthing 110 light

Prove proper operation on P & S E/R vents

Properly install 04 battery box covers
