Each tankship that has a keel laying date on or after January 1, 1975, must have deckhouse and superstructure ventilation inlets and outlets and other openings to the exterior arranged to minimize the admission of flammable gas to enclosed spaces that contain a source of ignition. Found cargo pump shaft boot was ripped and unsecured on all 04 cargo pumps; did not provide vapor tight seal. Repair or replace vapor tight boots to the satisfaction of the Coast Guard.

Be identified with the legend “High-level Alarm” in black letters at least 50.8 millimeters (2 inches) high on a white background; The vessel overfill alarm was found to not be properly labeled. Mark as required.

Scantlings, materials, and workmanship of cargo spaces, fittings of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Class surveyor found side shell damage of No. 8 WBTP and No. 9WBTP during internal exam and buckling of bilge strake, side shell brackets, transverse members, and longitudinal stiffeners. Make repairs to the satisfaction of Coast Guard and RO at next scheduled drydock.

ach vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. It was observed that the flange on the starboard aft vapor line is missing the permanent stud projection. To be addressed at next schedule drydock.

On a tank barge, each cargo tank must be equipped with an overfill device that- (i) Meets the requirements of 46 CFR 39.2007(b)(2) through (b)(4), and (d)1 through (d)(4) and 46 CFR 39.2009 (a)(2) 46 CFR 39.2007 (d)(3): Activate a visible and audible alarm so that it can be seen and heard on the vessel where cargo transfer is controlled and in the cargo deck area. External audible and visible alarms were inoperable when tested.

The aggregate capacity of the electric ship's service generating sources must be sufficient for the ship's service loads. In accordance with Bentley's report (Proj. ID: EP-2023-0046/Doc. ID: 000048); Nestco's Ballast Water Management Load Analysis (Dwg. No. T4600-S60-2-1/Rev. 0); and original Load Analysis and Short Circuit Analysis dated 8/9/96 (Dwg. No. E3/Rev.0) provided by the company, the vessel's electrical plant exhibits several overload conditions in its current configuration without the Ballast Water Management Treatment System installed. Relatively common operations of the barge may exacerbate these issues (i.e. operating the inert gas system, hydraulic power pack, and additional service loads and auxiliaries while only running the no. 1 or no. 2 generators). Prior to future electrical system modifications, the operator shall submit plans IAW 46 CFR 110.25-1 to Marine Safety Center (MSC) for review. All future modifications shall be approved and inspected to the satisfaction of the cognizant OCMI and Class.

Each pressure-vacuum relief valve must be tested for venting capacity in accordance with paragraph 1.5.1.3 of API 2000 (incorporated by reference, see 46 CFR 39.1005). The test must be carried out with a flame screen fitted at the vacuum relief opening and at the discharge opening if the pressure-vacuum relief valve is not designed to ensure a minimum vapor discharge velocity of 30 meters (98.4 feet) per second. Marine Inspector identified all P/V valves on board were past their required bench test inspection date. Provide proof of successful bench test or conduct required bench test for all p/v valves on board, to the satisfaction of a Coast Guard Marine Inspector prior to loading cargo. 46 CFR 39.2011(b)(2)

The galleys, living quarters, navigation spaces, general cargo spaces, boiler rooms, and enclosed spaces where sources of vapor ignition are normally present, shall be segregated from cargo tanks by cofferdams or pump rooms or tanks, either empty or used to carry liquid having a flashpoint of 150 °F. or above, or deck spaces enclosed or open. Marine Inspector discovered multiple holes in the bulkhead of the cargo prime mover room and multiple holes the generator house bulkhead which are located in the hazardous zone. Repair/renew bulkheads to original vapor sealed condition, to the satisfaction of a Coast Guard Marine Inspector prior to loading cargo. 46 CFR 32.60-10(a)

Vapor collection piping must be electrically bonded to the hull and must be electrically continuous. Marine Inspector discovered cargo/vapor piping on board missing grounding straps. Install appropriate grounding straps to cargo/vapor piping, to the satisfaction of a Coast Guard Marine Inspector prior to loading cargo. 46 CFR 39.2001(c)

A liquid filled pressure-vacuum breaker may be used for vapor overpressure and vacuum protection if the vessel owner or operator obtains the prior written approval of the Commandant. Marine Inspector discovered P/V breaker sight glass leaking voiding prior equipment approval. Repair sight glass to the satisfaction of a Coast Guard Marine Inspector prior to loading cargo. 46 CFR 39.2011(c)

When alteration or repair of boilers, pressure vessels, machinery, safety valves or piping systems becomes necessary, the work shall be done under the cognizance of the Officer in Charge Marine Inspection. Marine Inspectors identified cargo pump prime mover inoperable. No notification given to OCMI regarding failure or repair of prime mover. Repair prime mover to the satisfaction of a Coast Guard Marine Inspector prior to loading cargo. 46 CFR 50.05-10

The master of a tank vessel shall ensure that the instruction manual under § 157.23 is used when the cargo or ballast systems are operated. Cargo Operations Manual, Section 4.1 – General Requirements, states “Stopping the transfer immediately if any discrepancies are discovered a during the transfer and contact RCM Operations!” Additionally, page 7 of the cargo operations manual states “ Communicate to tankerman that they have the ability, responsibility, and obligation to stop work when an unsafe condition, act or behavior may result in an unwanted event.” During cargo loading operations, the barge experienced several electrical casualties including an electrical fire on the no. 2 generator. All 3 generators are currently deemed inoperable. The initial notification to the RCM Operations Manager was around 0330 on Friday morning, 01DEC2023; however, the barge never stop cargo loading operations during the casualty and the nearest Coast Guard office was only notified of the event after the barge had completed loading which was around 1800 on Saturday, 02DEC2023. Company and vessel shall conduct an internal audit of the barge’s cargo operational procedures and submit a root cause of failure analysis report along with a correction action report to Sector Houston-Galveston prior to loading cargo. 46 CFR 157.49

The installation of all electrical engineering or interior communications systems, together with the details of design, construction, and installation, must meet the requirements of subchapter J (Electrical Engineering Regulations) of this chapter. Electric installations on vessels must ensure: (1) Maintenance of services necessary for safety under normal and emergency conditions. (2) Protection of passengers, crew, other persons, and the vessel from electrical hazards. (3) Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. All shipboard generators experienced operational failure due to failed circuit protection and inadequate maintenance (i.e. - paint on generator windings). During generator failures, generator windings experienced arcing/sparking episodes. Provide permanent means to restore required power for shipboard and emergency systems as required by 46 CFR Subchapters D/F/J. Ensure all repairs are approved by flag/class, prior to movement. 46 CFR 32.45-1 46 CFR 111.01-1(a)

The installation of all electrical engineering or interior communications systems, together with the details of design, construction, and installation, must meet the requirements of subchapter J (Electrical Engineering Regulations) of this chapter. Each circuit that supplies a device on a switchboard, except a circuit under paragraph (b) of this section, must have overcurrent protection. Vessel’s generator circuit breakers and control overload protection fuses on the main switchboard failed to open upon overload condition. Power generation system components, such as generators’ excitation fields, windings, automatic voltage regulators (AVRs), and potential transformers (PTs), have failed based on evidence provided by the company. Provide technician's report attesting to the repair, testing, and satisfactory operation of the circuit breakers and overload protection fuses. 46 CFR 32.45-1 46 CFR 111.30-17(a)

The owner, agent, master, operator, or person in charge must, within 5 days, file a written report of any marine casualty required to be reported under § 4.05–1. This written report is in addition to the immediate notice required by § 4.05–1. This written report must be delivered to a Coast Guard Sector Office or Marine Inspection Office. It must be provided on Form CG–2692 (Report of Marine Casualty, Commercial Diving Casualty, or OCS-Related Casualty), and supplemented as necessary by appended Forms CG–2692A (Barge Addendum), CG–2692B (Report of Mandatory Chemical Testing Following a Serious Marine Incident Involving Vessels in Commercial Service), CG–2692C (Personnel Casualty Addendum), and/or CG–2692D (Involved Persons and Witnesses Addendum). At each inspection for certification the marine inspector shall conduct such tests and inspections of the auxiliary machinery and its associated equipment as he feels necessary to check safe operation. The low lube oil alarms for two auxiliary generators were not appropriately set up for proper testing due to manufacturer specific preparations not being met. 46 CFR 61.20-3(a)

Rule 24(b) of the 72 COLREGS states when a pushing vessel and a vessel being pushed ahead are rigidly connected in a composite unit, they are regarded as a power-driven vessel and must exhibit the lights under rule 23. 33 CFR 82.3

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of ABS or other recognized classification society. 3 ventilation closure devices for port generator space found holed on knife edge, 1 missing locking device. 1 ventilation closure device for port cargo pump room found holed on knife edge. Barge requires closed gauging. Sight glass on tank top for cargo tank #6P is shattered. Replace cargo sight glass on tank top #6P.

Barge over-ballasted, submerging load line by approximately 1 meter. Corrected while CG inspectors were onboard during cargo discharge operations.

Trunk for Slop Tank #9S had evidence of product leaking down trunk wall onto the deck. Determine cause of leak and demonstrate tank is vapor tight.

Life raft painter line not connected to the weak link, life raft was not in a float free configuration. Corrected on the spot.

All barges are required to have sufficient piping systems including pumps, valves, etc for the safe operation of the vessel. During discharge, the #2 cargo pump failed. The #4, #5, and #6 P & S cargo pumps lost suction and could not maintain suction due to suspected holes in the cargo piping below deck and improper packing. Vessel is to make permanent repairs to pumps and piping.

Vessel's gaging pipes, sight glasses, and cargo hatch covers are not vapor tight. At least eight sight glasses & six gauging pipes were leaking IG/cargo vapor mixture to atmosphere. Vessel is to be gas-freed and a vapor tightness test conducted. All components found to be leaking shall be repaired/renewed and a vapor tightness test repeated until the vessel is vapor tight.

The IGS system is not fucntioning as designed. The scrubber injectors clog up due to apparent excessive rust in the feed water line causing the system to shut down rendering the system inoperative until the injectors can be cleaned. Company must provide a stand-by means to maintain the inerted atmosphere in all tanks until all cargo tanks are empty and gas-freed.

After new inert gas system was installed; Entire approved procedures including testing/alarms and opreating of the system. Crew couldn't operate/start IG system, IG Tech. shall be onboard until the crew can show competency in the operation of the system to attending USCG Marine inspector and or ABS surveyer prior to cargo operations.

The #2 Generator over-speed trip is inoperable. This generator operates the Inert Gas System.

Sewage was discovered in the aft peak starboard side tank. Company had space pumped out and tagged out the MSD. Vesssel is required to have third party verify the sewage system is functioning properly prior to departure from New Orleans shipyard.

The liferaft painter was connected to the bracket vice the weak link, making the liferaft not float free.

Vessel has not conducted crew drills or training regarding fire fighting.

Fuel tank drain line gate valve is inoperable. Crew installed a threaded plastic cap to prevent further discharge of fuel on deck.

Sea Chest valve stem was wasted through causing the hand wheel to break off from the valve.

Generators were missing guards over belt.

Port side ballast pump was leaking excessive amounts of water duing operation.

The crew of Bouchard 245 was unfamiliar with both fire and life saving drills and had not performed any drills since the vessel came from shipyard in Tampa, Florida on November 24, 2017

The Bouchard 245 was found in operation with multiple marine engineering discrepancies.

The Bouchard 245 was found in operation with a flammable cargo (jet fuel) onboard with a non compliant Inert Gas System.

The crew of the Bouchard 245 were unable to use portable gas detection equipment onboard the vessel due to inoperable or missing calibration gas/equipment.

Multiple pieces of machinery equipment onboard the Bouchard 245 were found with missing or inadequate safety guards.

The Bouchard 245 was found with sewage being improperly stored in a void due to malfunctioning marine sanitation device & piping.

The crew of the Bouchard 245 was observed performing metalic grinding/filing in an atmosphere previously determined to be leaking flammable vapors.

Numerous areas onboard the Bouchard 245 were observed with inadequate or missing electrical bonding, explosion proof covers, or intact insulated wiring.

The crew of the Bouchard 245 did not perform any security drills since leaving the shipyard in Tampa, FL on November 24, 2017.

The Bouchard 245 was found with numerous non compliant cargo pumps, pipes, guages, valves, etc.
