(1) Cylinders must be retested at least every 12 years. The two CO2 cylinders on board had not been tested within the last 12 years.

A coupling in the fire main (located in the main deck overhead just forward of the main accommodation space's aft bulkhead) failed and prevented the testing of the system. The coupling was replaced and the test was successfully conducted by the end of the inspection.
