Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. 03 of the vessel's shutoff valve were not operable.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Both generators had open panel with exposed wires. Properly enclose boxes.

The owner, charterer, master, or managing operator of a vessel carrying overnight passengers shall have a suitable number of watchmen patrol throughout the vessel during the nighttime, whether or not the vessel is underway, to guard against, and give alarm in case of, a fire, man overboard, or other dangerous situation. Vessel watchman system was inoperable, fix watchman system and notify local OCMI. Vessel will have logged rounds every hour until engine room watch system is corrected w/in 14 days.

Electrical equipment must be installed to protect from electrical hazards. Electrical wiring underneath operating station has several exposed and dead-end wires. Properly remove or cover to prevent potential shock or fire hazards.

All vessels must have operable navigation lights in accordance with the Navigation Rules. Stern light was inoperable. Provide navigation lights that are following the Navigation Rules. Vessel shall only operate in daylight hours.

Vessel did not have non-conductive matting in front the switchboards. Vessel is to place non-conducting mats or grating shall be provided.

Whenever a vessel is to carry out major repairs or alterations affecting the safety or seaworthiness of the vessel, it is the duty of the person in charge to report any contemplated repairs or alterations to the Officer in Charge, Marine Inspection (OCMI). A report was received regarding repairs to the vessel’s planks. A Coast Guard Marine Inspector attended the vessel, observed questionable repairs, and identified concerns in the vicinity of the bow. No additional repairs or alterations shall be conducted without prior authorization from the OCMI.

Remove failed flooring support framing in forward storage room to assess damage to planking. Provide repair plan to replaced failed flooring support framing.

Removed decking in way of forward frames in crew bunk room port and stbd sides to assess failed frame fasteners and cracked frame. Provide repair plan to re-strengthen deck clamp areas.

Remove decking in way of forepeak compartment to assess frame-to-deck fastening. Provide repair plan to re-strengthen deck clamp areas.

Provide repair plan to add additional longitudinal stringer from storage room and into forepeak.

Provide repair plan to stop water ingress at various plank seams.

Provide repair plan to address movement of gunwale on bow of vessel.

Each ring life buoy on a vessel must: (1) Be approved in accordance with §160.050 in subchapter Q of this chapter, or other standard specified by the Commandant. Each ring life buoy with a floating watertight must have a 3ft lanyard and be attached with a corrosion resistant clip. Port ring buoy is not of an approved type, waterlight lanyard is to short and does not have a clip. 46CFR180.70

Vessel must have a valid bridge-to-bridge certificate on board. Vessel did not have a bridge to bridge certificate. 47CFR80.1001

Life rafts require servicing. Provide annual servicing report by 26Jun25. 46CFR185.730

Vessel conducted emergency haul out to address multiple areas of water intrusion from the hull. After re-entering the water on 18JUN25, 02 additional areas of water ingress were identified my MSD Santa Barbara marine inspector. New areas of water intrusion identified on 18JUN25 are: Butt-block above chine under bunk 24 / port side AND Butt-block under forward / stbd bunk in forward accommodation space. Vessel departed Ventura Boatyard without marine inspector verifying status of leaking areas identified. Vessel's COI was removed from the vessel on 11JUN25 and is scheduled for a recertification inspection on 25JUN25 at Seaforth Landing. Prior to the 25JUN25 recertification inspection, provide Sector San Diego with objective evidence attesting to any repairs made to address the 02 new areas of water ingress and the current leaking rated of each area.

While at the dock, the vessel is actively making water from a butt-block, stbd side at chine, underneath the forward accommodation staircase. Two frame bays in-way-of butt-block have water trickling downward. Haul vessel out of the water for inspection and submit repair proposal.

While underway, the vessel is making water in the forepeak compartment when the bow digs into swells. Areas of concern are the upper four strakes of planking, both port and stbd sides in the aft end of the compartment. Haul vessel out of the water for inspection and submit repair proposal.

The aft watertight bulkhead (collision bulkhead) is not watertight. Significant pass-through of water to the adjacent compartment was observed while filling the forepeak with water. Identify the cause and submit repair proposal.

Plank seam at stbd garboard to strake B plank is making water in the aft end of the storage room adjacent to the forward accommodation space. Haul vessel out of the water for inspection and submit repair proposal.

Partial butt-block in forward accommodation space, stbd side forward of staircase is seeping water. Haul vessel out of the water for inspection and submit repair proposal.

While conducting steering tests, both rudders were found to be leaking excessively with water accumulation in the lazarette bilge pockets. Make packing gland adjustments to both rudders to reduce the water accumulation.46CFR 182.610

Both main engine sea strainers were found unmounted and were supported by the attached flexible hoses. Provide each main engine sea strainer with proper support to prevent movement in all sea conditions. 46CFR 182.420

Electrical submersible pump found partially disconnected with exposed wiring. Either remove or make permanent installation.

During the fire drill the two deckhands were not familiar with the location of the Halon pull station and thought it was activated with the galley pull station. The vessel was also found without a life-boatman but did not have training on how to launch a life raft. Conduct remedial training on both and once satisfactory, send a statement that you as Captain of the TRIBUTE found their knowledge of emergency drills satisfactory.

Fire hose nozzle seized, replace with one IAW 46CFR181

Ring buoys must be maintained in accordance with 46CFR160.050. Retro reflective tape was missing from ring buoy on the starboard side of the vessel.

Vessel's must have a life jacket for every person on board. Vessel has 2 un - serviceable life jackets taken out of service and another 7 that needed new retro reflective tape. Vessel is restricted to 41 passengers.

Vessel must have a valid bridge to bridge certificate. Vessels bridge to bridge certificate was expired.

Machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure and personnel injury. Exhaust has multiple pinholes on muffler and must be replaced.

A hatch cover must be attached to the hatch frame by hinges, chains or other devices of substantial strength to prevent its loss. Until replaced ensure hatch stays secured at all times.

Integrated smoke detectors were not integrated and did not all set each other off. Troubleshoot and ensure the smoke detectors were integrated. Roving watch was corrected at the time of the inspection but documented.

Vessel was unable to start its MDEs or generators. Identify and repair the cause of the failure.

Secondary means of escape from forward, lower bunk room was blocked by two rubber mats, impeding escape. Remove mats and keep escape hatch free of obstructions.

Port side DC distribution panel behind accommodation bulkhead found to be missing cover. Install cover.

Vessel decking IWO port side damaged area found to have significant rot and fastener wastage. Remove deck coating for further inspection.

Vessel found to have damaged area and wood rot IWO port bow rub rail. Submit repair proposal.

Vessel required to conduct fastener pull in vicinity of damaged area to the satisfaction of marine inspector.

All vessels on an Oceans Route are required to carry twelve (12) distress flares. Vessel only had six (6) rocket parachute flares on board at time of inspection. Vessel route is reduced to a Lakes, Bays and Sounds until six (6) additional rocket parachute, red flame or smoke flares are provided.

Each overnight accommodation space on a vessel with overnight accommodations for passengers must be fitted with an independent modular smoke detection and alarm unit. Forward passenger berthing spaces on the port and starboard side did not have the smoke detection systems. Overnight passenger count is reduced by six (6) persons until deficiency is rectified.

A fire hose with a suitable nozzle must be attached to each fire station on board the vessel. Fire hose and nozzle on the port side were damaged and need to be taken out of service. Must replace in kind to the satisfaction of the CG Marine Inspector. Starboard side hose is still operable and can reach all sections of vessel.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed. Crank case re-breather on the starboard main engine was not hooked into the air intake as required by engine manufacturers. Must correct to the satisfaction of the Marine Inspector.

All emergency exits must not be blocked and allow easy access for all personnel on board. Emergency egress from forward crew space was blocked by rubber floor grating in the galley. Must remove grating of cut around emergency egress spot prior to the next overnight trip.

Install port fuel tank bonding wire. Due 30Apr2021.

Install double hose clamps on fuel transfer pump hose. Due 30Apr2021.

Secure port side A/C pump to foundation. Due 30Apr2021.

Remove fuel from transfer pump containment. Due 30Apr2021.

Install metallic plugs on fuel filter drains. Due 30Apr2021.

Tighten bridge A/C panel cover to eliminate air-gap. Due 30Apr2021.

At each initial & subsequent inspection for certification, reinspection and every other inspection, the MI shall require that all observed unsafe practices & hazardous situations be corrected. Vessel to make forepeak/stem of vessel available for inspection. Make proper repairs to ensure watertight integrity of the hull, vessel forepeak & Aft Watertight Bulkhead in Forepeak as indicated. 46 CFR 176.25-50

Provide operational approved fixed fire fighting for engine room. Make testing of system available to USCG.

Provide certified marine chemist certificate showing engine room is safe for entry.

Vessels emergency lighting did not actuate upon loss of power to the vessel. Provide passenger and crew accommodations with adequate emergency lighting.

Re-locate emergency escape labeling to actual hatch.

Remove dead-ended cables from all bunks on vessel.

Remove all hard-wired power strips from vessel. Restore to original outlet configuration.

Properly mount and provide blank covers for all junction boxes under pilot house console.

Label all breaker loads in all electrical panels.

Vessel has been identified as having various A/C electrical modifications since initial certification. Sector San Diego does not have vessels electrical plans on file or current installations do not match what we have on file. Provide a one-line electrical drawing for all A/C switchboards and distribution panels. Include all circuit loads, conductor sizes, over current protection, and max panel ratings in amps. Include generator and shore power breaker ratings.

While underway or drifting at sea, every vessel shall at all times maintain a proper look-out by sight and hearing. The designated watchmen required by 46 CFR 185.410 cannot preform both roving patrolling and look-out watch simultaneously. Vessel has been instructed to operate with a look-out and watchman while conducting drift operations.

Provide alcohol test strips to accommodate 100% of crew testing.

Prove proper operation of anchor light.

Relocate EPIRB to open area to facilitate float free.

Relocate spanner wrench for fuel shutoff to outside of machinery space.

Install captive rope/chain on MOB ladder to prevent loss of ladder.

Repair starboard side ventilation closure clasp.
