A vessel must be provided with a main steering gear that is: (1) Of adequate strength and capable of steering the vessel at all service speeds; Vessel tiller fractured. Repair/Replace prior to carriage of passengers.

Operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit of the class required below. The operator is responsible for the proper operation of the station. It was observed that operators do not have FCC radio operator license. Operators are to obtain required FCC operators permit.

Where life preservers are stowed so that they are not readily visible to passengers, the containers in which they are stowed shall be marked "life preservers" and with the number contained therein, in at least 1-inch letters and figures. This legend shall indicate the separate stowage of children's life preservers. Properly mark all PFD stowage locations.

Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft non-ferrous metal straps with no sharp edges. It was observed your vessel had fuels lines supported to the bulkhead by plastic zip ties. Install with proper support straps.

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gastight throughout its length and shall be properly supported by non-combustible hangers or blocks. It was observed during operation that there was a minor exhaust leak from your exhaust piping. Repair exhaust to the satisfaction of attending MI.

Connections to battery terminals shall be made with permanent type connectors. MI's observed wingnuts on batteries. Replace wingnuts on batteries

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. It was observed your vessels first aid kit does not have instructions provided. Provide instructions for first aid kit.

Keep bilges dry to prevent loss of stability due to water in bilges as per stability letter. It was observed that the shaft packing was at the end of its adjustment capability. Replace shaft packing at next DDX.

Vessel ruptured coolant hose.

Propulsion machinery must be suitable in type and design for the propulsion requirements of the hull in which it is installed at constant marine load under such requirements without exceeding its designated limitations. Vessel lost forward gear reduction and no longer has full operation of main propulsion machinery.

SM01: The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. While underway outbound the Yaquina River, vessel reported that propulsion became inoperable and experienced ingress of water from main engine exhaust. Propose repair plan, make vessel available for damage survey, and conduct repairs to satisfaction of attending marine inspector, prior to departure.

SM02: The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The vessel's shaft packing is dripping at an excessive rate. Conduct repair to packing at next dry dock.

Each vessel certificated to operate on an oceans route in cold water must either: (1) be provided with inflatable buoyant apparatus (IBA) of an aggregate capacity that will accommodate at least 100% of the total number of persons on board; or (2) meet subdivision standards and be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons on board. The IBA currently on board is rated for 20-persons. Vessel shall not carry more than 20 persons onboard.

Make permanent repairs to vessels steering control arm. Repairs to be approved by the attending Coast Guard Marine Inspector, prior to carrying passengers.

Troubleshoot and make permanent repairs to vessel's main engine fuel oil service system to the satisfaction of the attending Marine Inspector prior to carrying passengers.

Replace unserviceable type I PFD's. Vessel reduced to 21 passengers. IAW 46CFR 180.71

Replace expired hydrostatic release unit on EPIRB. IAW 46CFR 180.64

Renew packing in shaft stern tube. IAW 46 CFR 182.200

Replace lifebuoy. IAW 46 CFR 180.70

Repair non-operational high water alarm lights on the console.

Prepare and submit a plan to bring rails into compliance with excursion vessel requirements. Execute once approved.

Vessel lost suction on sea water pump causing engine to overheat and shut down. Determine cause of loss of suction and repair.

Properly repair port/fwd motor mount.

Provide 01 Childs lifejacket. Vessel restricted to 02 children until additional Childs lifejacket is onboard, light is attached and properly marked with vessels name.

Properly repair aft fuel vent screens with 30x30 mesh screen.

Properly support electrical wire below decks under bunk on stbd side.

Properly label lifejacket stowage.

Replace 08 adult and 02 child life-jackets. Vessel passenger count is reduced to 25 adults and 02 children until such time as life-jackets can be replaced.

Provide and post Inflatable Survival Craft Placard.

Properly secure saltwater wash down hose.

Prove proper operation of magnetic compass light.

Prove proper operation of all bilge pumps prior to operating with passengers.

Prove proper operation of visual indicator for all automatic bilge pumps prior to operating with passengers.

Provide copy of Marine Operators Permit.

Provide new life-ring buoy prior to carrying passengers.

Provide updated charts.

Clean excess fuel from strainer bowl prior to carrying passengers.

Clean stbd fuel tank vent screen.

Provide "bright" container for signal flares.

*USER FEES: No Sail CG-835 issued. User Fees documented by USCG FINCEN as delinquent. Vessel is not to carry more than 6 passengers for hire until the USCG FINCEN documents that all required User Fees have been paid.

Renew FCC station license. 47 CFR 80. Correct to the satisfaction of the attending marine inspector.

Make forward bilge pump operate as designed. 46 CFR 182.520. Correct to the satisfaction of the attending marine inspector.

Renew fuel hose using J-1942 type running from filter to MDE injector pump. 46 CFR 182.720. Correct to the satisfaction of the attending marine inspector.

Renew or service (2) B-II & (1) B-I portable fire extinguishers IAW NFPA-10. Correct to the satisfaction of the attending marine inspector. VESSEL RESTRICTED FROM OPERATING CERTIFICATED UNTIL SATISFIED.

Renew port #4 wheelhouse window with proper material. 46 CFR 177.1010. Correct to the satisfaction of the attending marine inspector.

Provide service report for repairs completed to engine mounts and a Masters statement, stating operational checks have been completed. To be completed prior to carrying of passengers.

Secure raw water pump. Correct to the satisfaction of the attending marine inspector.

Re-new starboard bilge hose and double clamp all other bilge hoses where applicable. 46 CFR 182. Correct to the satisfaction of the attending marine inspector.

Re-new FCC Safety Certificate. 47 CFR 13. Correct to the satisfaction of the attending marine inspector. VESSEL RESTRICTED TO 20 MILES FROM LAND UNTIL RENEWED.

Re-new FCC Marine Radio Permit. 47 CFR 13. Correct to the satisfaction of the attending marine inspector.

Test/conduct the following to the satisfaction of the attending Marine Inspector: Steering, SSB Radio, MOB & F/F drills, Main Engine operation & associated gauges.

1. Make permanent repairs to stbd external frame guard (aprox. 6 inches aftof bow)

2. Make permanent repairs to port fuel tank and supply line and hydrostatically test as required in 46CFR182.440.

REPAIR RUSTED AND CRACKED FWD STBD EMERGENCY FUEL OIL TANK SHUT OFF VALVE HANDLE.

Install positive shut off valve at overboard in way of fish box.

Install bonding cable on all fuel tanks.

Install flame screens (30x30 wire mesh) on port and starboard fuel vents.

CONDUCT MAN OVERBOARD DRILL.

CONDUCT UNDERWAY OPERATIONAL TESTS OF STEERING AND MAIN ENGINE.

Repair fracture in weld/hull at stern tube (shaft log).

Make drip proof the electrical outlet box on the forward bulkhead of the engine room.

Make proper repair to the rudder pin. Rudder pin had excessive movement.

Prove proper operation of MDE

Re-new flame screen in the port aft fuel tank vent.

Provide 32 adult PFD's as required by the COI

DRILL STOP AND PROPERLY REPAIR THE APPROXIMATELY 3/4" FRACTURE IN THE PORT DECK INSERT (FWD STARBOARD CORNER)._x001C_ _x001C_ _x001C_

WELD THE UNDERSIDE OF BOTH DECK INSERT PLATES, OR CONDUCT NDT TO VERIFY FULL PENETRATION WELDS ACHIEVED.

PRESENT THE AFT FUEL TANK INSTALLATION FOR INSPECTION.

Provide valid registration for 406 EPIRB and renew expired hydro release._x001C_ ** prior to carriage of passengers. **

Provide on board a valid radio operator license and radio station license._x001C__x001C_ _x001C_ _x001C_

Prior to Comercial operation or carriage of passengers, provide on board valid Certificate of Documentation.

OBTAIN VALID RADIO OPERATORS LICENSE.

TEST PROPULSION ENGINE AND CONTROLS AND TEST EACH BILGE PUMP. SEND AFFIDAVIT LETTER TO [name withheld].

REPAIR LIFEFLOAT PADDLES OR PURCHASE NEW ONES.

REPAIR OR RENEW STERN LIGHT FROM WHITE ALL-ROUND TO PROPERLY SECTORED LIGHT.

OBTAIN CURRENT COPY OF 46 CFR PARTS 166 TO 199 (SUB T).

PLUG SW INLET TEE ON STBD SIDE.

PLACE CHECK VALVES IN ELEC BILGE PUMP OVBD LINES.

PROPERLY CONNECT AFT FUEL SHUT OFFS.

PLACE PLUG IN STBD FUEL OIL LINE. FUEL OIL LINE IS PRESENTLY DISCONNECTED.

TIGHTEN OR REPLACE RUDDER PACKING GLAND.

REPLACE RUBBER EXHAUST SECTION, STBD EXHAUST MANIFOLD.

CLEAN AND REMOVE ALL SOOT FROM AFT STEERING AND ENGINEROOM BULKHEADS, OVERHEADS, AND BILGES.

PROPERLY MARK LIFE PRESERVER STOWAGE.

CONSPICUOUSLY MARK THE LIFEFLOATS WITH THE VESSELS NAME.

ATTACH PADDLES TO THE LIFEFLOATS.

MAKE PERMANENT REPAIRS TO CRACKS IN THE OUTER SKIN OF THE TWO 15-MAN LIFE- FLOATS.

INSTALL A BRACKET FOR THE WATER LIGHT ATTACHED TO THE RING BUOY.

REPLACE ALL FLEIXBLE FUEL HOSE WITH APPROVED TYPE.

SERVICE AND HYDRO ALL FIRE EXTINGUISHERS AS NEEDED.
