Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] discovered that the sewage holding tank was in need of repair. Vsl owner is to submit plans to the local OCMI for approval prior to work. 46 CFR 176.700

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Such hose must overlap metallic pipe ends at the least 11/2 times the pipe diameter and must be secured at each end by clamps. The flexible section must be accessible and as near the upper end of the fill pipe as practicable. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. There is no conductor to protect against the generation of static charge. 46CFR182.445(g)

All fuel tanks must be electrically bonded to a common ground. Diesel fuel tank was not bonded to a common ground.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. MI discovered the port main did not have a guard over rotating machinery. Make repairs to the satisfaction of the attending MI.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following, Inspection of batteries for condition and security of stowage. MI discovered the stbd battery wire connection to be deteriorating. Make repairs to the satisfaction of the attending MI. 46 CFR 176.806

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following, Inspection of all cable as far as practicable without undue disturbance of the cable or electrical apparatus. MI discovered cables in the Engine room on the SSDG to be zip tied at an angle of which is damaging the wires. Make repairs to the satisfaction of the attending MI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations. MI discovered that the windings on the SSDG to be oil covered. Make repairs to the satisfaction of the attending MI. 46 CFR 176.830

07124 - The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests:portable extinguishers must be inspected and maintained in accordance with NFPA 10 and for fixed-gas fire extinguishing systems the owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. All portable fire extinguishers and the fixed fire extinguishing system missed annual servicing. 46 CFR 176.810(b)

12199 - Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. When the fuel shutoffs were activated the valves did not fully close. 46 CFR 182.455(b)(4)

11117 - Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The ring buoy on the forward part of the vessel was not marked with the name. 46 CFR 185.604(b)

03106 - Each window must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. The window on the main deck starboard side, amidships, was cracked. 46 CFR 177.1020

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks. Vessel Operator reported damages to the bow and deck plating of the vessel after a collision with another vessel.

A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel missing 02 of the 06 required distress signals. 46CFR180.68(b)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). New retro tape was needed on 01 life ring. 46CFR185.604(i)

A carbon dioxide cylinder may not be inclined more than 30° from the vertical unless fitted with flexible or bent siphon tubes, in which case it may be inclined not more than 80° from the vertical. CO2 bottle siphon tube line should be installed at the 12 o'clock position as indicated on the bottle to ensure proper discharge of agent. Bottles were reinstalled oriented in different directions than what is required for horizontal installation. Provide documentation from fire suppression company that siphon tube is oriented in appropriate position.

The operator must secure each Type III device in a manner which prevents discharge of sewage. Sewage ovbd discharge valve was found in the open position with no means of securing. Close valve and remove handle or padlock valve in closed position on ovbd discharge valve.

The number and identification of items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Re-label the number of life jackets stowed in each storage compartment.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. Marine inspector found multiple open slots on vessel's switchboard panel. 46 CFR 183.330(b)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Junction box in aft compartment found with missing cover. 46 CFR 183.340(g)

A dry exhaust pipe must be kept clear of, and suitably insulated or shielded from, combustible material. Generator turbo exhaust was missing lagging. 46 CFR 182.430(g)(1)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter. Waterlight found to be installed with incorrect battery type. 46 CFR 180.70(d)(1)

Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter and be readily accessible for use. Ring buoy plug was missing. 46 CFR 180.70(b)(1)(2)

Repairs or alterations to hull, machinery, or equipment that effects sasfety of the vessel must not be made without the approval of the cognizant Officer in Charge Marine Inspection (OCMI) except in emergency. At the commencement of vessel hull and internal structure exam the following worklist items were outlined for repair or replacement as original. Provide repair plan proposal to OCMI for approval prior to commencing. 1. Both port and starboard side of vessel from top of cleat sockets to one foot below the waterline marking (white stripe on hull below chine) from stem to stern including stern corners wrapping around to the stern are to be cleaned down to bare metal including all ‘bondo’ and then gauged for thickness. Due to hull scantlings the allowable percentage of waste not to exceed 15%

Clean to bare metal and gauge where bottom is marked in grid pattern starting in center of marked area expanding out ward in 6” intervals up to 12 inches.

Clean off ‘bondo’ from both port and starboard embarkation decks and the entrance bulkhead IWO main accommodation area doors and framing to access damage.

Repair as original hole in bulkhead IWO bilge suction at keel on aft bulkhead of storage hold aft of forepeak.

Repair deck and bulkhead IWO embarkation areas to inclide hull plate and deck plate replacement as original.

Repair as original holed areas in 02 deck and 02 deck starboard base of bulkhead entering into 02 accommodation space.

Remove paneling interior IWO atrium area to access steel bulkhead condition.

Repair/replace as original the bases for the exhaust “buckets’

Repair / replace as original aft engine room longitudinal and tabs where longitudinal connect to aft bulkhead outboard of port engine.

Replace as original missing Structural Fire Protection in engine room areas where missing.

Remove interior paneling and interferences in way of main deck windows to access wastage to the superstructure around window trim.

Repair as original rotted exterior superstructure trim and drain areas above main deck windows.

Gauge under potable water tank.

Replace fore peak hatch gasket.

Replace Starboard engine R/W suction valve

Replace rubber section of over board drainage line for bar sink overboard drain and add an overboard skin valve.

Remove concrete patch around AC thru hull valve in Storage Void port side. Gauge hull plating IWO of thru hull for thickness.

Repair hole on side shell plate in Storage void starboard forward corner.

Repair / replace as original port aft corner longitudinal and tabs where longitudinal connect to aft bulkhead in steering compartment.

Replace as original lower six longitudinal stringers in fore peak.

Prior to carriage of passengers all repairs shall be completed to satisfaction of Officer In Charge Marine Inspection (OCMI)and a satisfactory seal trial completed.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Excess oil was found to be accumulated under the Generator. 46 CFR 182.100

Glass and other glazing material used in windows accessible to passengers and crew must be of material that will not break into dangerous fragments if fractured. Window in main accomodation area on port side aft was cracked and shattering. Replace broken window. 46CFR177.1010

Repair secondary fire/bilge PTO pump on starboard main engine.

Replace missing cover for starboard main engine computer control box.

Provide CO2 testing report from March 2021.

Provide appropriate tide tables, charts, and coast pilot for intended area of operation (Baltimore, MD).

Repair or replace inoperable marine radar.

Repair inoperable spot light.

Only 04 red hand flares were onboard. Provide a minimum of 06 flares (03 red and 03 orange, or 06 red).

Update first aid kit with ammonia inhalants.

Remove dead end wires in lazarette.

FCC Safety Radio Authorization Certificate is expired. Provide renewed certificate.

FCC Bridge to Bridge Certificate is expired. Provide renewed certificate.

Servicing report for fire systems was not on board during the inspection. Operator was able to provide an email of the report prior to the conclusion of the inspection.

Flares must be stored in a water tight container marked "DISTRESS SIGNALS". Current storage is not water tight.

Vessel is required to have 6 day and 6 night flares. 3 day and 2 night flares are missing.

Add missing blanks electrical panel on main deck.

Electrical wiring not properly secured or maintained in galley, main dining area and engine room.

Replace broken windows on main deck; 04 total.

Provide technician report or satisfactory testing of generator with fire pump running to simulate engine under load. Engine was emitting excessive smoke while under load.Potential turbocharger issue.

Vessel must undergo annual inspection prior to carrying passengers.

stenciling is worn. re-stencile all 3 ring buoys.

Life jacket location marking is out of date. Update the actual amount of life jackets at the storage locations to reflect amount on board.

Permanently mark the vessels name.

Renew P/S strut and P/S shaft tube cutlass bearings.

Renew 03 gussets, one in lazerette on stern starboard side and two in sewage tank space (marked).

Clean prep for inspection identified area of scaling in engine room, port side bottom plating. **Repair proposal submitted to insert an 18 X 18 plate to renew area of wasted hull**

Renew fram in forepeak, starboard side second frame.

Crop and renew to good metal idendified wasted areas in galley deck (marked in lazerette).
