The bilge pipe piercing the collision bulkhead shall be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. In lieu thereof, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead if it is readily accessible under service conditions. The valve/reach rod was not operable. Prove proper operation within 30 days. Any proposed changes to the piping system IAW the cite, will require OCMI approval. 46 CFR 182.25-5(d) (Old T)

ire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Fuel pipe fitting located in void 3 displayed signs of a slow leak, tighten or replace fitting to ensure proper seal.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Safety drills haven't been logged appropriately the past several months.

Repairs to the hull must not be made without the approval of the cognizant [name withheld] venting for the stern void (#7) has a pin hole; Permanent repair proposal must be submitted for OCMI approval. Permanent repairs to be made no later than 28 February 2026. Vessel authorized to operate with temporary repair measures in place. Condition of temporary repair to be checked weekly during vessel's operational season. Any change in condition of the venting or temporary repair to be immediately reported to the OCMI.

Each vessel must have a stability letter issued by the Coast Guard. As per the vessels stability letter (H2-2301918, dated 24MAY2023), alterations in vessel displacement, VCG, or LCG may invalidate the stability letter. Due to vessel alterations, the total aggregate weight change exceeds 2% of the currently approved lightship displacement and, as outlined in MTN 04-95, necessitates a review of stability criteria by MSC. Vessel authorized to operate with a reduction of 04 passengers allowed to be on the 02 deck which also reduces the maximum passengers allowed on the vessel from 228 to 224.

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. The fire pump gauge was inoperable.

Gages to indicate cooling water discharge temperature and lubricating, oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Several engineering gauges at the operating station were found inoperable. prove proper operation of all.

A voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation. Ammeter in engine room was found inoperable.

A vessel must be operated in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Significant deterioration in the vessel's fore-peak void has reduced hull plate integrity to the point watertight integrity was breached in two areas. Provide permanent repair proposal for marine inspector approval and make repairs to the satisfaction of the attending marine inspector prior to carrying passengers UNDERWAY. Vessel is authorized to carry passengers moored and NOT underway at the Gateway Arch Riverboat facility. The "dockside" operation shall be in full compliance of the vessel's COI and stability letter at all times while passengers are on board.

A vessel must be operated in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Bow's fore-peak/bow void hull plating sustained about a dime sized hole located on starboard side, approximately 7 feet aft of bow stem at about the 4' draft line (approximately 18 inches above the waterline). Provide repair proposal for marine inspector approval and make repairs to the satisfaction of the attending marine inspector prior to carrying passengers.

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. 1. As per 46 CFR 118.115 vessel must comply with 46 CFR 181.20-5 (46 CFR Old Subchapter T) in that fixed fire suppression system must be installed to the satisfaction of the OCMI. As per the USCG approved Kiddie CO2 systems manual, the CO2 system depend on tightness of the space protected. Engine room ventilation dampers failed to close when exercised.

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. As per 46 CFR 118.115 vessel must comply with 46 CFR 176.25-50 (46 CFR Old Subchapter T) in that unsafe conditions must be corrected. A hole in the vessel's 2nd deck leaked water directly into a passenger accommodation space. This deficiency has been extended from prior to carriage of passengers to next drydock, due 28FEB2025. Temporary repairs were verified on 21FEB2024.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. Current COD not on board vessel at time of inspection.

Fire Protection - vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H (Passenger Vessels) of this chapter. Main deck FWD PORT side and 01 deck FWD STBD side have areas of wastage on the doors. Vessel has been authorized to conduct temporary repairs of adding 16 gauge steel sheet coverings on wastage areas meeting the class “A “ bulkhead standard. Vessel is allowed to carry passenger. Vessel shall replace both doors once company receives the new fire doors. A due date of May 12, 2023 is issued.

Navigational lights and public address system must be arranged so that they can be energized from two sources of electricity. Inspection found navigational lights and public address system did not energized on emergence/backup power.

Six (06) doors installed in a class A-0 bulkhead where found without latches. Before purchasing products submit products specifications to the Inspection Office for review.

Gas fire extinguishing system required to be serviced as per CG approved manual. Inspection found fixed gas fire extinguishing system distribution line caps/plugs that extend at least 2 inches beyond the last orifice have not been removed during the bi-annual blow through process. Prove that caps/plugs have been removed and blow through process has been conducted.

Vessel authorized to meet regulations applicable on 3/10/1996. IAW 46 CFR 183.01-5 (Old T). Inspection found electrical outlet has exposed wires due to cover being damaged, location is behind bar area on 02 deck. Prove wires are not exposed.

Bilge high level alarms must be provided. Inspection found two level alarms improperly secured in the bilge. This deficiency was corrected before the conclusion of the inspection.

Vessel authorized to meet regulations applicable on 3/10/1996. IAW 46 CFR 183.01-5 (Old T). Inspection found exposed wires located on A/C compressor's junction box due to cover missing. Prove wires are not exposed.

Vessel to be operated in a safe condition free from fire hazards. Inspection found generator windings were saturated in oil. Prove that generator windings are clear and free of oil.

Vessel authorized to meet regulations applicable on 3/10/1996. IAW 46 CFR 176.25-32 (Old T). Inspection found pressure vessel's safety valve was due for testing. Crew was unable to test safety valve. Test safety valve in presence of attending inspector.

Inspection found shackle not moused on passenger gangway rigging. Inspect all rigging on gangway and properly secure shackles/rigging. Prove that all rigging is properly secured.

Each life jacket must be marked with the vessel's name and have retro-reflective tape. Inspection found some life jackets had vessel's name on sticker, which were peeling from the material and retro-reflective tape damaged. Prove that all life jackets are permanently marked and have proper retro-reflective tape .

Vessel authorized to meet regulations applicable on 3/10/1996. IAW 46 CFR 176.25-35 (Old T). Inspection found steering system chain located port side aft had a kink in one of the chain's links and drive gear was visibly askew from being perpendicular to the shaft.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated. Inspection found no signage indicating max number of passengers allowed on upper deck.

Vessel authorized to meet regulations applicable on 3/10/1996. IAW 46 CFR 181.20-15(d) (Old T). Fixed fire extinguishing - simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes. Inspection found no instructions had pull boxes located in the galley.

IAW 46 CFR 80.159 Each ship transporting more than six passengers for hire equipped with a radiotelephone station where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Inspection found the master unable to provide radio operator’s permit.

A means of escape that is not readily apparent to a person from both inside and outside that space must be adequately marked IWA 46 CFR 122.606. Inspection found PORT and STBD doors on the 02 deck are not marked "EXIT".

Fuel tank starboard side bulkhead access plate/gasket leaking fuel from fuel tank into adjacent void. Take necessary action to stop leak and remove fuel from void prior to carrying passengers.

Vessel (Existing T-L) required to meet 46 CFR 182.25-10(Old Subchapter T) minimum bilge pumping capacity of 50 gallons per minute (GMP). During bilge test, evacuation of water from bilge pick using installed electric bilge pump was unsatisfactory from multiple voids. Prove proper operation of a pump capable of meeting bilge (46 CFR 182.25 Old-T) and fire pump requirements (46 CFR 181.10 Old-T) applicable to vessel. Vessel authorized to continue operation with an appropriately sized portable dewatering pump kept on board and ready for immediate use.

Vessel (Existing T-L) required to meet 46 CFR 182.25-10 minimum bilge pumping capacity of 50 gallons per minute (GMP). During bilge test, evacuation of water from bilge pick was well below required rate. Prove proper operation of bilge system to meet GPM rating from each bilge pump or provide an alternative temporary arrangement for consideration/approval.

Vessel required to be operated in a manner as to afford precaution against hazards to the vessel and personnel. Diesel fuel tank access plate located on starboard side of tank leaked fuel into the adjacent void. Repair and prove proper seal of plate gasket and ensure bulkhead in vicinity of plate is sound and leak free.

Vessel required to be equipment with operational navigation lights. Port and starboard running lights were inoperable. Lights bulbs were replaced during inspection.

IAW 46 CFR 122.520 & .540 Crew members are required to participate in man overboard drills (every 3 month), fire drills (monthly), and abandon ship drills (monthly), or, immediately before returning to service if vessel is not operating for a greater time period. Following an extended period vessels was not operating, six individuals had crewed the boat between 22 Mar and Apr 05 without participating in drills as per vessel log. Conduct and document required drills prior to using individuals as crew members.

Waterlight is required to be attached to ring buoy with lanyard. Lanyard attaching buoy to waterlight was significantly weathered and frayed. Lanyard required to be renewed.

Portable light required at entrance of engine room. Portable light was inoperable at time of inspection.

Vessel required to be operated in a manner as to afford precaution against hazards to the vessel and personnel. Emergency lighting fixture in engine room not properly secured to bulkhead.

Provide acceptable documentation showing vessel meets 46 CFR Subchapter S stability requirements.

PFDs required to be marked with retro-reflective tape and the vessel's name. Twelve PFDs required retro-reflective tape and one PFD required to be marked with vessel's name.

Piping to supply water to mechanical fire pump on starboard engine, first union inboard of sea suction valve, leaked during testing of fire pump. Union required to be made leak free.

Mid-body void, fore and aft watertight bulkheads contain alternating fore and aft deflections between vertical stiffeners. Condition occurred over a period of time, however, there is no previous documentation on condition and no current procedure in place to monitor. Current extent of deflections required to be documented to facilitate future monitoring and evaluation.

Aft most transverse bulkhead (superstructure-transom at stern of vessel) found to have defective weld seam. Location of defective weld seam, in bulkhead within exhaust closet, approximately 10 inches above hull plating and adjacent to starboard side of vessel. Defective weld seam required to be re-worked.

46 CFR 122.315 states that the vessel master is required to determine that the vessel complies with all applicable stability requirements in the vessel's stability letter prior to departure. Vessel master could not provide an adequate procedure when requested. Required vessel operator to provide procedures for master to meet requirement.

Main deck, deck plating just forward of aft most transverse bulkhead (at stern of vessel) wasted along length of bulkhead, all instances less than 12" forward of transverse bulkhead. Wasted deck plate to be cropped to good metal and renewed by no later than 10FEB2021. In interim, plating to be visually examined for deterioration or change in condition at least once a month and logged in the vessel's bridge log AND changes to condition are to be promptly reported to the Marine Inspectors (USCG Sector UMR).

Crop & renew fractured hull plating in bottom hull plate of stern void between the 4th and 5th longitudinal frame from the port side of vessel and approximately 4 feet forward of the transom. Until permanent repairs are effected the vessel is prohibited from passenger vessel service UNLESS the following measures are in place: - Temporary repair (pressure patch) is in place and effectively secures ingress of water, - One portable de-watering pump is on board vessel, - A crew member checks for water in stern void no more than 30 minutes before & after each passenger cruise, - Hull plating fracture promulgation at fracture is checked (will require removal and re-installation of temporary repair) at least once a week, - Water checks & fracture checks are logged & made available upon request(date/time/person/condition), and - changes in condition of fracture and/or any uncontrollable flooding are immediately reported to the local OCMI. (Sector UMR Command Center phone number is [phone withheld]) At all times the vessel is to be operated within limits of vessel's stability letter (3'3" mean draft with no more than 6" of trim).

46 CFR 183.10-15 (Old Sub T) as referenced by 46 CFR Subchapter K, Part 120: Place electric insulating matting in front of switch board in engine room.

Clean oil from ship's service generator (in engine room) in vicinity of air intake and turbo charger.

Replace expired visual distress signals.

Replace expired items in ship's first aid kit.

46 CFR 181.20-15(d) (Old Subchapter T) Post operating instructions (at local controls) for fixed fire extinguishing installation (in space immediately forward of engine room).

Properly secure oil leak at after cooler (for gear box lube oil) on inboard side of port main diesel engine.

Fuel oil filter assembly not located in engine room as required by 46 CFR 182.20-40(5).

Senior Deckhand letter of designation not signed by on duty vessel master. COI allows for licensed mate to be substitued with a Senior Deckhand, so designated in writing. Vessel to in full compliance with COI as per 46 CFR 115.110(c).

Make necessary corrections to the deficiencies identified on page 1 of Enclosure (1) listed under "A. Electrical Deficiencies Observed". See attached documentation.

Once item 1) from above is addressed, make necessary corrections to the approved electrical plans (Drawing # 17-2298)for items listed on page 1 & 2 of Enclosure (1) under "B. Electrical Plan Edits". Upon completion, re-submit to the MSC for approval. See attached documentation.

Provide, by means of a PE letter, attestation that all systems identified in 46 CFR 120.310(a) on-board the vessel can be powered from two sources of electricity.

Identified a deficit in one of the supply bottles for the vsls fixed CO2 system for the E/R. Ensure that the vsls fixed CO2 bottles weight loss does not exceed 10% of the weight of the charge.

Significant deterioration was observed on the starboard forward and port aft stairwells adjacent to the pilothouse. Effect repairs to the affected stairwells.

Submit and obtain plan approval for all modifications made to the vsl's electrical arrangement.

Prove proper operation of the bilge system. O/O advised MIs that several manifold valves were not seating properly and required replacement.

Make repairs to fuel leak (return line) in the E/R in the overhead IWO of the stbd generator.

Provide metal supports for all wiring in the E/R where previous supports were removed for stbd MDE overhaul.

Provide repair proposal for hole in the collision bulkhead (between peak tank and #1 void). Upon reciept of OCMI approval, effect repairs.

Ensure navigation lights are powered by two independent power sources.

Conduct satisfactory man overboard drill in the presence of the attending MI.

Provide satisfactory proof of testing of the safety relief valve for the vsls installed pressure vessel.

Make necessary repairs to the starboard MDE.

Employer did not have record of required Supervisor EAP training. Training reqoured as per 46CFR16.401.

Bulkhead on starboard side of fuel tank (integral) in bow void has a leak. Prior to operating, submit repair proposal to local OCMI.

Overhead in port bow void (deck plating) has two (2) epoxy patches. Submit repair proposal to local OCMI prior to conducting repairs.

Stern void compartment, port side, under exhaust piping has a leak. Submit repair proposal to local OMCI for approval, prior conducting repairs.

Emergency instruction list is not mounted at the operating station. Post proper emergency instructions at operating station, prior to carrying passengers.

Public address system not working on battery backup power. System is either configured wrong or battery is low on power because system trips offline within seconds of taking the load. Repair or replace battery backup prior to carrying passengers.
