The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. NCDOT reported "B" end shaft bearing was slipping and sparking during morning checks. NCDOT must conduct repairs IAW manufacturers specifications ans in-kind equipment. Provide proof of repairs to attending marine inspector. If vessel needs to get underway for repairs, notify the MSD Ft. Macon office for request/approval.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. Vessel master made a report stating during fire drill, crew discovered fire main piping located in the engine room had a pin hole leak. Vessel is currently moored at Cherry Branch. NCDOT must provide repair report stating the correction to the vessel and hydro results of the pipe (1.5 times safe working pressure). Vessel master will notify MSD Fort Macon as conditions change.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Retro-reflective tape on forty adult sized life jackets were found wasted. Vessel restricted to 273 passengers.

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Closure for MSD space not properly working with middle louver fin.

All distribution valves and controls shall be of an approved type. All controls shall be suitably protected. Remote control boxes for E/R and EDG Fixed fire extinguishing systems not properly installed. Safety/security glass not secured with second hold down bracket.

Doors shall have a latch with a minimum throw of 3-4 inch which can be operated from either side of the door. The limit mechanism on outboard door handle for starboard side EDG space was found broken. Make effective repairs.

The emergency power source must meet table 112.05-5(a)....Passenger vessels other than Ocean, Great Lakes, or coastwise and not on an international voyage must have a final emergency power source capable of operating for a minimum of 8 hours or twice the time of run, which ever is less. Vessel's representative reported the emergency generator overheats causing the engine to shutdown approximately 15 minutes into running with a load. Provide a final emergency power source capable of operating for a minimum of 8 hours or twice the time of run, which ever is less.

Each machinery space must be ventilated to ensure that, when machinery or boilers are operated at full power in all weather including heavy weather, an adequate supply of air is maintained for the operation of the machinery. Vessel's representative reported that the emergency diesel generator has to operate with the emergency generator space's doors open to prevent the engine from overheating. Troubleshoot and provide adequate ventilation to prevent emergency generator from overheating.

A survey either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its life-saving appliances, fire-fighting, or other equipment. Master reported the #1 steering system gave alarm during transit and automatically shifted to the #2 steering system. No loss of steering was reported. The vessel moored at Cherry Branch. Electricians conducted verification of the system. A fault code for the "B" end transverse thrust was noted. A calibration of the potentiometer and system was completed. After which, the fault was cleared. The system was tested and found working satisfactory; dockside and post underway trails by the crew.

Crew reported during checks of the vessels rescue boat. The rescue boat davit manual winch safety cut out switch (for the handle) was found inoperative, further investigation revealed the switch was corroded. Maintenance replaced the switch in-kind and tested satisfactory. The master provided objective evidence and an attestation reporting satisfactory operation.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Master reported hitting loading ramp while mooring up. MI's must conducted survey of vessel IVO contact area, prior to departure.

The Master shall take all precautions to see that automobiles or other vehicles have their motors turned off when ferry is underway and motors shall not be started until ferry is secured. In addition, vehicles at each end end shall have wheels securely blocked. MI witnessed vehicles not properly blocked and vehicles were started prior to docked.

Each self propelled vessel must have both main and auxiliary steering gear, except when the main steering gear includes two or more identical power units. Master reported the NR1 power unit had an unknown fault causing it to trip offline and automatically switch to the NR2 power unit. Make permanent repairs to steering system. Provide technician's report of findings and repair. Prove proper operation of steering system to USCG Marine Inspector.

Fire damper on A end of EDG bulhead (intake) is missing hardware IAW 46CFR72.05-50(b).

Excesive trash and debris found in bilges IAW 46CFR71.25-45.

Deck plates in E.R. not properly installed IAW 46CFR72.40-20.

Women's head exhaust vent screening painted over IAW 46CFR72.15-20(a).

Alarm Panel on Bridge B-end control station has faulty alarm for "E-Gen Overspeed" IAW 46CFR111.12-11.

Loose Connections on battery terminal in EDG space IAW 46CFR111.60-17.

Missing bracket on bilge suction throughout vessel IAW 46CFR70.20-1.

Rescue Boat davit has chafed wire on limit switch IAW 46CFR70.28-1.

Master reported a steering fault on "B" end while in route to Cherry Branch, during operations. Determine cause of fault and make proper repairs to the satisfaction of the coast guard. Prior to passengers operations.

Replace or repair "B" control head (lever) is loose not working properly, to the satisfaction of the coast guard.

First aid kit. Pilothouse and engine room expired meds.

Main deck engine room watertight door unable to secure. Adjust dogs.

During testing/operation, the rescue boat died and failed to restart. Prove proper operation to the satisfaction of the attending Marine Inspector

Received report of a throttle/steering failure while carrying passengers. Throubleshoot and report finding to marine inspector (MI) prior to requiring attendance. Once permanent repairs are conducted,test all related systems and contact CG MI for attendance of final operational testing prior to carrying passengers.

After allision with bulkhead, three frames on the underside of the starboard deck on the A end were buckled and one weld was broken. Make permenant repairs to the satisfaction of the attending Marine Inspector.

Womens Restroom door is not closing properly

Determine "A" end upper navigational light. Electrical issue/not operating properly

A and B gauge lines are loose.

Pilot valve CO2 was loose in the fuel tank void.

Loose gear blocking emergency escape void #2A

During testing/Mob rescue boat failed to start. Prove proper operation to the satisfaction of the attending Marine Inspector.

Rescue boat davit failed to retrieve rescue boat after rescue boat was deployed. Prove operation of the davit to the satisfaction of the attending Marine Inspector.

While mooring No. 1 generator oversped and would not maintain electrical load as designed. Make effective repairs and prove proper operation to the attending Marine Inspector. Vessel is restricted from carrying passengers.

Upper deck A end storage locker missing hardware.

Broken Fire Ext. mount rescue boat (securing strap).

Rescue boat securing straps wasted/worn.

Engine room WTD will not properly close due to adjustment/ alignment issues. Prove proper operation.

Crews first aid kit had expired medication.

All water tight doors in the engineering spaces were witnessed as open and unattended, during inspection this date.

PTSP's were not availible for inspection, because vessel was on the run. A requirement for crew to carry out specific testing procedure for voith units and to provide attestation from captain once witnessed as satisfactory.

Portable fire extinguishers in pilot house, #02, #03, and #17 had loose hoses, when spot checked this date.

Change out block tackle on the rescue boat ladder wasted and worn.

Replace the forward and aft cargo netting which was damaged and worn.

Install backflow preventer on the saltwater to fresh water filtration piping.

The Starboard Engine Room door vent closure does not close properly. Make repairs to the satisfaction of attending MI.

Install suitable exhaust lagging IWO MDE exhaust side shell penitrations.

Provide CPR first aid cards.

Vessel has not completed a new deadweight survey as required by MSC letter dated September 13,2018 (serial: H2-1803524) and MSC Technical Note 4-95. The new survey is required due to an aggregate weight change in excess of 2%. According to MSC letter dated February 20,2007 (serial: H2-0700482), M/V NEUSE had receive alterations in 2007 amounting to an aggregate weight change of 1.02%. This previous change along with the voith unit soft patch installations, deck wear plate modifications, and other items completed since 2007 exceeds the 2% threshold. Provide new calculations.

The vessel is currently Not broadcasting (1) IMO number.

EDG space vent closures broken spring and will not close 100%.

B-end escape hatch dog cracked. Yard period in June conduct repairs.

A-end escape hatch leaking

Liferaft straps deteriorated on three rafts.

The #2 SSDG RPM gauge in not working; make the necessary repair and prove proper operation.

#1 SSDG low L/O shutdown did not secure the engine; make repairs and prove proper operation.

The life buoy on the "B" end is failing at the inner seam; replace the life buoy and ensure it is properly marked.

Properly terminate the dead end wires in the #2 SSDG control panel and the EDG control panel.

Ventilation closure IWO the emergency generator compartment is not closing as designed; repair as necessary and prove proper operation.

Provide a cover for the junction box under the "A" end console on the bridge.

The loss of lube oil pressure shut down for the "A" end main engine did not function as designed, make the necessary repairs.

The breaker panel index card in the storage room located within the passenger accommodation space is not properly labeled with amperages; make the necessary corrections.

Provide approved type hose (SAE J-1942) for the following machinery; (1) on the emergency generator, (1)on the "A" end main engine, (1) on the "B" end main engine, and (3)on the "B" end generator.

Verify that all required equipment on the E-bus panel in the pilothouse are operating as required or properly labeled as needed.

During exam we were unable to see either main diesel engines shut down during low lube oil test as required. They only gave a visual/audible alarm. Vessel is to come into compliance and demonstrate proof of low lube oil audible/visual alarm and shutdown for both main diesel engines in the presence of a Coast Guard Marine Inspector along with meeting the requirements of 46 CFR 58.05-10.

Louvers to EDG need to operate properly while EDG is running and shutdown to include closing properly. Until cleared Chief engineer to log in engineer's log anytime EDG runs that both louvers are closed and verified closed after EDG is shutdown.

Properly terminate dead end wires under bridge steering consoles

Have technician troubleshoot steering control transfer system

Test steering control transfer system IAW approved periodic safety test procedures.

Provide instructions for the manual operation of the Fixed Co2 System in the engine room and label the control valves accordingly.

Demonstrate proper operation of ENG room ventilation shut-down near ENG room door.

Provide lanyard for rescue boat engine.

Provide proper identification placards IWO propulsion control system (operating stations).

Resubmit propulsion control operations manual to reflect as built controls, indicators and configuration.

Re-submit DVTP to address missing MDE load signal and shaft speed inputs.

Provide proper sound powered phones IWO local voith control spaces.

Submit as built drawings IWO propulsion control system.

Adjust fire door IWO galley entrance. The door is not self closing and does not latch. The BLKHD is reported to be warped, precluding sufficient adjustment. Repair within the next 30 days to the satisfaction of the attending marine inspector.

On or before the next dry dock, install main engine shut downs at specified locations IAW 46 CFR 62.35-5.

Modify E/R vent closures to ensure secure fit (fitted plates are loose).

Properly stencil the emergency escapes through out the engineering spaces as needed, approx. four not marked.

Post operating instructions for the fixed CO2 system local operation to include labeling of the control valves.

Prior to the carriage of vehicles on the tunnel side driveway repair the leak in the supply piping for the vehicle deck sprinkler system.

B-end main diesel engine over speed when activated shut down the engine by means of fuel rack only, the air box shut down solenoid did not operate as designed. Repair/renew shut down solenoid, test and provide documentation to USCG.

A-end Life Jacket storage locker door wasted IWO bottom securing dog; dog was difficult to operate and allow access to L/J - repair/replace hardware as needed to allow door to properly operate.

While operating on the Southport, NC to Fort Fisher, NC ferry route, vessel must have at a minimum 1 Master, 1 Chief Engineer, 2 Deckhands and 1 Oiler. The Oiler may be non-certificated. LEGACY COMPONENT DESC: Minimum Safe Manning

VESSEL SHALL HAVE ON BOARD AN APPROVED RESCUE BOAT WITH REQUIRED EQUIPMENT AS REQUIRED BY 46 CFR 199.630(i) AND 46 CFR 199.620(j), PRIOR TO PLACING THE VESSEL INTO OPERATION.

VESSEL SHALL INSTALL AND PROVE PROPER OPERATION OF STEERING ALARMS IN THE PILOT HOUSE AS REQUIRED BY 46 CFR 58.25-25(d).

VESSEL TO HAVE ON BOARD A TRAINING MANUAL AND OR ADUIOVISUAL AIDS TO COMPLYWITH 46 CFR 199.180.

VESSEL SHALL HAVE ON BOARD AND POST AN APPROVED STABILITY LETTER AS REQUIRED BY 46 CFR 170.120. REQUIREMENT SHALL BE SATISFIED PRIOR TO PLACINGTHE VESSEL INTO OPERATION.
