STBD engine exhaust pipe and both STBD and PORT engine shaft bulkhead penetrations were not vapor-tight impairing pre-engineered system functionality for designed space.

Observed both generators were designed with an ambient temperature of 40°C , but were not derated to 80% of the full load rating, and the rating or setting of the overcurrent devices weren't reduced accordingly.
