All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. STBD engine exhaust pipe and both STBD and PORT engine shaft bulkhead penetrations were not vapor-tight impairing pre-engineered system functionality for designed space.

Observed both generators were designed with an ambient temperature of 40°C , but were not derated to 80% of the full load rating, and the rating or setting of the overcurrent devices weren't reduced accordingly.
