Demonstrate to attending MI proper testing of installed fire detection in engine room and bow thruster room, in accordance with manufacturer instructions / supplied equipment manual. 46 CFR 118.400(c), 701c

No more than 19 liters (five gallons) of flammable liquids may be stowed in any machinery space. The flammable liquids must be in containers of 3.8 liters (one gallon) or less. No more than 208 liters (55 gallons) of combustible liquids may be stowed in any machinery space. The general construction of the vessel must be such as to minimize fire hazards insofar as it is practicable. Vessel had excessive flammable/combustible liquid storage and storage of solid combustibles in engine room / bow thruster room. 46 CFR 147.45(c)&(d), 46 CFR 116.405(a), 701c

Suitable means, such as a manual damper, automatic damper, or vent cover, must be provided in an accessible location outside the space served by the ventilation duct for shutting off the passage of air through the ventilation duct in the event of fire. A ventilation duct penetrating an A-Class or B-Class fire control boundary must meet the following requirements: (7) Ducts serving cargo spaces, machinery spaces, or vehicles spaces must be fitted with automatic fire dampers. An automatic fire damper must meet the above requirements [46 CFR (g)(1)-(g)(4)] and must be designed to operate at 74 °C (165 °F) for normal locations and approximately 100 °C (212 °F) for locations such as galleys. Engine room ventilation (in STBD aft corner) was unable to be successfully closed. This ventilation shaft penetrates an A-60 deck boundary. 46 CFR 116.610(d), 46 CFR 116.610(f)&(g), 701c

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. 46 CFR 115.830(a) Observed multiple instances of oil soaked lagging in the engine room. 46 CFR 115.830(a), 701c

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Lube oil leaks were observed on all main propulsion engines and generator prime movers. 46 CFR 119.100, 701c

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. 46 CFR 16.230(a) Certificate of enrollment in drug testing consortium was expired. 46 CFR 16.230(a), 701(c)

The general construction of the vessel must be such as to minimize fire hazards. Repairs or alterations to … equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. SFP in AC room on Deck 1 did not meet required boundary based on fire control plan. Submit repair proposal for approval prior to commencing work. 46 CFR 116.405(a), 46 CFR 115.700(a)&(b), 50c

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. Multiple gauges were inoperable in the engine room. 46 CFR 119.410(b), 16c

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. Provide accurate schematic diagram. Review instructions and marking to ensure accuracy. 46 CFR 122.612(a), 16c

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart. … The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Cable/wire located in the engine room on the aft bulkhead/stbd side lacked appropriate metal supports. 46 CFR 120.340(b)(4), 16c

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel’s stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Vessel does not have markings to indicate number of passengers allowed on upper decks in accordance with stability letter.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, flooding, heavy weather, or man overboard conditions. Emergency instructions were not mounted at the operating station. 46 CFR 122.510, 16c

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. Emergency lighting arrangement in liquor storeroom below the main deck did not illuminate secondary means of escape. 46 CFR 120.432(a), 46 CFR 116.500(i), 60c

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Emergency broadcast placard with contents in accordance with 46 CFR 121.510 was not provided at the radio installation. 46 CFR 121.506, 16c

An exposed hazard, such as gears or rotating machinery, must be protected by a cover, guard, or rail. Shafts were not protected in the steering compartment. 46 CFR 116.960, 50c

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placards with type-specific lifejacket donning instructions were absent. 46 CFR 112.516(a), 16c

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. All lifejacket stowage locations lacked an appropriate method to separate child and adult lifejackets.

The owner, charterer, master, or managing operator shall instruct each crew member … at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by §122.510, when applicable, the duties listed in the station bill required by §122.514. Provide copy station bill with any necessary updates and proof of crew training. 46 CFR 122.420, 16c

Vessel's fire detection and alarm system is not of an approved type and must be replaced with an approved system. 46 CFR 118.400(c), 50c

For vessels required to meet the damage stability standards of subpart H of this chapter, when Class 1 doors are installed below a deck the molded line of which at its lowest point at side is less than 7 feet (2.14 meters) above the deepest load line, an indicator light for each door which warns when the door is open must be installed on the bridge. Vessel lacks indicator on bridge indicating status of WTD installed in Frame 41. 46 CFR 170.255, 50c

Each space accessible to passengers or used by the crew on a regular basis, must have at least two means of escape. “Doors in the travel path should swing in the direction of exit travel and should be operable without the use of a key from the egress side of the doors. If a lock is required on a door in the escape path, panic bar hardware should be provided.” One door IWO escape route lacked necessary panic bar hardware. 46 CFR 116.500(a), NVIC 9-97 Ch. 1, 50c

Emergency instructions cited rescue boat and restaurant manager. Instructions were posted behind a window blind. Produce vessel specific instructions and post in a visible location. 46 CFR 122.510(a)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Post upper deck passenger limits IAW stability letter issued in 2016.

A-Class bulkheads must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. Port bow, main deck door does not have latch required to ensure required A-60 boundary. Install door latch. 46 CFR 116.415

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Vessel rep could not produce list of tested crew members. generate list and add to vessel DAPI folder 46 CFR 16.260

Your vessel must submit a written application for a renewal of the Certiifcate of Inspection at least 30 days prior to the COI expiration date. Vessel's COI is currently expired and shall complete a renewal COI inspection prior to carrying passengers.

Demonstrate operation of 480V ground detection. If a grounded distribution system is provided, there must be only one connection to ground, regardless of the number of power sources. This ground connection must be at the switchboard or at the common ground plate, which must be accessible. Vessel is showing 2 amps of ground when connected on shore power. Vessel must not have two ground connection points and eliminate ground fault. [46 CFR 120.376(a)]

Provide 3 light covers in the soda room.

Post COI and emergency instructions in area accesible to passengers.

Determine fire boundary for dumbwaiter on main deck.

4. REMOVE GREY WATER PUMP ELECTRICAL CONNECTION.

5. ENSURE IDENTIFIED WATERTIGHT BULKHEADS ARE WATERTIGHT, MAINLY THROUGH HOLE FROM STORAGE ROOM TO WATER TANK ROOM.

All adult passengers may be carried unitl 720 extended size life preservers are provided or 72 children life preservers are provided. 46CFR117.71(b) PRIOR TO CARRYING CHILDREN UNDER 90 POUNDS.
